1 Landstown High School Governors STEM Academy Step 1: Read and Sign Volunteer Guidelines and Ethics Agreement Sign this agreement once each year, at the beginning of the school year, or at the beginning of your volunteer experience. 2 Landstown High School Governor s STEM Academy Step 1: Each Volunteer will complete the Volunteer Information, and read and sign the Guidelines and Ethics Agreement once a year. Return to PTSA Mailbox School Volunteer Information and the Guidelines and Ethics Agreement forms are located with each Staff/Sponsor of Clubs, Sports or Events, and through the Volunteer in Education Coordinator. These forms will be online for all staff and sponsors. Forms shall also be available at the Security Desk, and in the office in the blue Volunteer Folder. Step 2: Staff and Sponsors at LHS provide accountability and safety for Volunteers by a written record of volunteer presence, attendance and service. Volunteers agree and are required to sign in/out for all LHS endorsed activities before, during, and after school. During School Hours: The Visitor Book is located at the front security desk. Each volunteer signs in and out and wears the visitor badge. Before and After School Hours: Under the guidance of Staff and Sponsors, each volunteer shall record his or her daily arrival, departure, and location through sign-in/out log books during any after school LHS supported function. These completed log sheets are to be submitted by the staff/sponsor on the 1 st day of every month to LHS PTSA mailbox. These Sign In/Out sheet forms shall be located online for staff and sponsors, and may be found at the front Security Desk in the Volunteer BLUE Folder, and in the office Volunteer BLUE Folder. Step 3: Landstown High School Governor s STEM Academy recognizes and celebrates all volunteers for all contributions which support our students, staff, administration and community. Each volunteer agrees to log his or her hours online. Each Volunteer will log-in his volunteer hours from a mobile device or computer: Visit this site: Please record hours as soon as possible. At the end of each month, the monthly report is sent to LHS Volunteer Coordinator. o Volunteers activity choices are Partner, Individual Mentor, Program Mentor, Tutor, Classroom, Club, Sports, PTSA, Golden Circle or Other. Sponsors and Staff may need to provide access to an electronic device for some volunteers.
