1 Are you a high school student in need of volunteer service hours? Are you looking to expand your resume or college applications? The Port of Los Angeles VolunTEEN Program may be just the program for you! The Port of Los Angeles occasionally needs volunteer assistance at community events. We are looking for high school students interested in assisting at these events as VolunTEENS. VolunTEEN duties may include, but are not limited to the following: Working in a Port information booth distributing brochures or promotional items Children s activities, such as, games, face painting, craft activities, etc. Assisting with the TransPORTer, the Port s mobile exhibit Line monitoring and crowd control assistance On occasion, may include lifting and moving of event equipment and materials Other duties as asked by a Port representative The Port of Los Angeles invites interested and qualified high school students to submit a Port of Los Angeles VolunTEEN application. Applications will be kept on file and students will be selected and contacted as-needed to assist. In order to be considered for a VolunTEEN opportunity, you must: Be a high school student currently enrolled in grades 9-12 Complete the Port of Los Angeles VolunTEEN application Read, sign and date and obtain your parent or guardian s signature on the Rules & Responsibilities form Include 2 references on your VolunTEEN application. One reference must be provided by a teacher or faculty at your school. Complete, sign and date and obtain your parent or guardian s signature on the Photography Release Form Submit your application by to edtours@portla.org, fax to or mail to Port of Los Angeles, Public Relations Division, Educational Coordinator, 425 S. Palos Verdes Street, San Pedro, CA PLEASE NOTE: Students who submit applications are not guaranteed service hours. Students will be selected and contacted as-needed to be offered VolunTEEN service opportunities.

3 RULES AND RESPONSIBILITIES To be considered as a VolunTEEN, you must agree to represent the Port of Los Angeles in a professional manner including the following: Follow the attire restrictions provided to you by a Port of Los Angeles representative It is NOT appropriate to wear short shorts, flip flops, or other beach attire While wearing a Port-issued shirt (if applicable) remember that you are representing the Port of Los Angeles and must maintain professional at all times even if it is before and after your shift Port-issued attire should not be altered in any way Arrive and report to a Port representative at your scheduled shift on time, in appropriate attire and ready to work Do assigned work to assist the community or staff it s not the time to just hang out Do not talk on cellular phones or text message during your shift Speak with Port staff and community members respectfully. No profanity or other inappropriate language. Friends or family members are not allowed to remain with you during your shift If you receive a question from a community member that you are unable to answer, advise them that you are a student volunteer and that you will locate a Port representative to assist them If rules or responsibilities are broken or a Port representative ends your shift for any reason you must exit in a professional manner Please sign below to indicate that you understand and agree to follow the above rules and responsibilities. 5 PORT OF LOS ANGELES SPECIAL EVENT STANDARD PHOTOGRAPHY RELEASE FORM MINOR RELEASE In consideration of the engagement as a model of the minor named below, and for other good and valuable consideration herein acknowledged as received, upon the terms hereinafter stated, I hereby grant ("Photographer"), his/her legal representatives and assigns, those for whom Photographer is acting, and those acting with his/her authority and permission, the absolute right and permission to take, copyright and use, reuse, publish, and republish photographic portraits or pictures of the minor or in which the minor may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations from time to time, in conjunction with the minor's own or a fictitious name, or reproductions thereof in color or otherwise, made through any medium at his/her studios or elsewhere, and in any and all media now or hereafter known for illustration, promotion, art, editorial, advertising, trade, or any other purpose whatsoever. I also consent to the use of any published matter in conjunction therewith. I hereby waive any right that I or the minor may have to inspect or approve the finished product or products or the advertising copy or printed matter that may be used in connection therewith or the use to which it may be applied. I hereby release, discharge, and agree to save harmless and defend Photographer, his/her legal representatives or assigns, and all persons acting under his/her permission or authority or those for whom he/she is acting, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy. I hereby warrant that I am of full age and have every right to contract for the minor in the above regard. I state further that I have read the above authorization, release, and agreement, prior to its execution, and that I am fully familiar with the contents thereof.
