Please review each item below and check the box beside to signify understanding of the eligibility requirements: The nominated business must have operated in Medicine Hat & District for a minimum of three years (unless otherwise specified in the criteria for a particular category.) The nominated business must employ Albertans to produce goods and/or provide services. The nominated business must be operating at the time of nomination and at the time of presentation of the award. No nominee may have anyone in their management, on their board, or in their employ serving on the judging committee for the category in which they seek to be nominated. Directors of the Medicine Hat & District Chamber of Commerce are asked to refrain from accepting or submitting self-nominations. 4 BUSINESS ETHICS AWARD The Business Ethics Award is given to the business or organization that best demonstrates a commitment to advancing marketplace trust through ethical business practices. The nominee must have operated in Alberta for a minimum of one year. In addition to the general eligibility requirements, nominees must be in good standing with BBB; however, BBB Accredited Business status is not a requirement for consideration. Applicants must provide additional information outlined below along with three letters of reference or supporting documentation. Please provide as much information as possible, along with at least three letters of reference and/or support, as applications are judged on the information submitted. Self-nominations are not permitted in this award category. 1. Applicants will be evaluated on the extent to which they meet each of the Eight Standards of Trust.

5 COMMUNITY INVOLVEMENT AWARD The Community Involvement Award is given to the business or organization that has best demonstrated outstanding commitment to community involvement through partnerships, volunteerism, or other means of contribution to the local community. In addition to the general eligibility requirements, applicants must provide additional information by answering the questions below. Please provide as much information as possible, along with at least three letters of reference and/or support, as applications are judged on the information submitted. Self-nominations are not permitted in this award category 1. Describe how your organization has made significant and ongoing contributions to supporting and building south-eastern Alberta communities. 2. Describe how your organization has demonstrated outstanding civic leadership by providing records or examples of community service, volunteerism, generosity, or advocacy through active participation in communication efforts. 3. Please provide no less than three letters of support or thanks illustrating your company's involvement with locally-based non-profit community initiatives. 4. Please provide examples of policy that exists within your organization to promote the adherence to high ethical standards. 5. Why do you feel your company is a good candidate for this award? 6. 6 CUSTOMER SERVICE EXCELLENCE AWARD The Customer Service Excellence Award is given to the business or organization that has best demonstrated outstanding customer service in their everyday business, and leadership in setting high customer service standards for our region. In addition to the general eligibility requirements, applicants must provide additional information by answering the questions below along with along with three letters of reference or supporting documentation. Please provide as much information as possible, along with at least three letters of reference and/or support, as applications are judged on the information submitted. Self-nominations are not accepted in this award category. 1. How do you provide customers with optimal choices in products, services and means of delivery? 2. How do you provide means to satisfactorily address customer inquiries and/or complaints? 3. How do you develop and implement innovative systems, procedures, or techniques that reduce costs, eliminate paperwork, enhance efficiency, and contribute significantly to quality service to customers? 4. Describe how you have improved customer service to your customers over the past three years and how this has improved your business. 5. Why do you feel your company is a good candidate for this award? 6. Provide any additional information that you think might help us evaluate your submission.

7 EMPLOYER OF PERSONS WITH DISABILITIES AWARD The Employer of Persons with Disabilities Award is given to a business or organization demonstrating creative leading-edge practice in hiring, training, and developing employees with disabilities. This award is given to an employer who has been instrumental in removing barriers to employment for persons with disabilities. In addition to the general eligibility requirements, applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Demonstrate how your organization employs persons with disabilities and provides them with career-oriented activities. Career-oriented activities can include career guidance, training, job shadowing, mentoring etc. 2. Provide information on the impact/success, of providing employees with disabilities with careeroriented activities. Include at least one success story. 3. Explain and describe your organization s success in one or more of the following: a) Eliminating systemic workplace barriers to hiring and/or retaining persons with disabilities, by making appropriate physical adjustments in the workplace OR b) Meeting the needs of persons with disabilities by removing any artificial barriers in work descriptions. Describe changes your organization made in work descriptions that removed and/or refined tasks that allowed you to hire persons with disabilities. 4. 8 EMPLOYER OF YOUTH AWARD The Employer of Youth Award is given to a business or organization that has demonstrated outstanding achievement in hiring youth and providing them with career-oriented opportunities. In addition to the general eligibility requirements, applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Demonstrate how your organization employs youth (age 25 years and under) and provides them with career-oriented activities. Career-oriented activities can include career guidance, training, job shadowing, mentoring, etc. 2. Provide information on the impact/success of providing youth with career-oriented activities. Include at least one success story. 3. Demonstrate success in one or more of the following: creating new career pathways for youth; providing entrepreneurial support for youth; assisting youth, (youth with disabilities, and/or atrisk youth and/or Aboriginal youth) to get, keep and progress in their job in their organization; expanding vocational, technical and/or apprenticeship training for employees age 25 years and under. 4. 9 EXPORT AWARD The Export Award is given to the organization that has demonstrated outstanding achievement in exporting products or services outside of Alberta's borders. In addition to the general eligibility requirements, applicants must have: Exported products, services or technologies that have Alberta content or Alberta value-added contribution. Employed an innovative and/or creative market entry strategy. Made increases in exports over the past three years, or maintained their position in export markets during difficult economic times either for the company or in their geographic or industryspecific market. Applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Please provide a synopsis of your export plan (do not provide a copy of the whole plan), explaining: a) Why your company exports b) Your market entry strategy into your current markets AND any markets you are planning to enter in the next two years c) Your export growth strategy 2. What products, services or technologies does your organization export? 3. Where does your organization export? (current export markets) 4. List your export sales for the past three fiscal years. What is the proportion of export sales relative to total sales over the past three years? (ratio of export sales to total sales) 5. 10 FRANCHISE BUSINESS OF THE YEAR The Franchise Business of the Year Award recognizes a franchise business in the Southeast Alberta region who best reflects the outstanding spirit and success of our region s business community, has demonstrated significant achievement within the last three years and has contributed to community activities and professional associations. In addition to the general eligibility requirements, applicants must: Business must be independently owned and operated. Business must be owned and operated by an Alberta resident(s). Major business activity must take place within the province of Alberta. Business must be owner-operated for a minimum of three years. Applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. How does your organization embody the Alberta Advantage? (e.g. leading edge thinking, risk taking, entrepreneurial practices, environmental responsibility, global focus, fun and friendly attitude) 2. What percentage of your business activity takes place within the Province of Alberta? 3. Describe your business achievements in the past three years. 4. Describe your active involvement in business and/or professional associations. 5. Describe your organization's ongoing and consistent involvement in community events and organizations locally and/or provincially. 6. Demonstrate the ways that your organization contributes to the economy of the local community (buying locally, hiring locally). 7. Describe your brand recognition strategies. 8. Describe your customer service performance. 9. Describe how your franchise provides leadership in the franchise community including encouraging excellence through sharing best practices, speaking engagements, being willing to mentor new franchisors, and raising the profile of the franchise business model. 10. 11 INNOVATION AWARD OF EXCELLENCE The Innovation Award of Excellence will be awarded to a business or organization that is creating commercial value through the development (or application) of better or more effective products, processes, services, or technologies. In addition to the general eligibility requirements, applicants must: Have been in operation for at least one year. Have a significant portion of the innovation and/or commercialization that has taken place over the last two years. Applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Please describe the innovative product, process, service or technology you have developed. 2. Describe the commercial application(s) for this innovation: Does this innovation result in a new or better product that can be sold? Does this innovation make existing processes more efficient? Does this innovation enable your business to access new markets? 3. Describe the process and timeframe for developing and commercializing the innovation: Are you currently marketing, selling, or using the innovation? When did the development start/finish? Which aspects of the development took place in Southeast Alberta? If the innovation has not yet been commercialized, describe your plans and timeframe to bring it to market and/or integrate it into your processes. 4. Describe the originality of the product, process, service or technology: Why is it innovative? Why is it better or more effective that what is currently on the market? Is it a new concept or a variation of an existing idea? Is it unique or has it been adapted from others? Was it developed independently or in cooperation with others? 5. Describe how technology relates to the innovation: If the innovation is a new or enhanced technology, what impact will it have on other companies, products, and markets? Has the technology been tested? If the innovation is a product, process, or service, describe how it leverages technology? 6. Please provide information on the measurable benefits of the innovation: Does it result in measurable revenue increases? Are there measurable cost savings? Are there measurable efficiencies in operations / marketing? Will it enable or increase measurable exports to international markets? 7. 12 LARGE BUSINESS OF THE YEAR The Large Business Award is given to the business or local branch/office/store/etc. that best reflects the outstanding spirit and success of our region's business community in a large, established company. In addition to the general eligibility requirements, applicants must: Have 51 or more full-time equivalent employees and/or have more than $5 million/year in sales revenue. Be profitable in each of the last three years. Demonstrate significant achievement in business. Applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Describe your business achievements over the past three years. 2. What percentage of your business activity takes place within the Province of Alberta? 3. Describe your active involvement in business and/or professional associations. 4. Describe your involvement in your community at-large. 5. Why do you feel your company is a good candidate for this award? 6. Provide any additional information that you think might help us evaluate your submission.

13 MARKETING EXCELLENCE AWARD The Marketing Excellence Award is given to the business or organization that has best demonstrated excellence, innovation, and originality in traditional or new-media marketing. In addition to the general eligibility requirements, applicants must have implemented a new marketing strategy within the past two years, and must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Describe your marketing objectives for 2016 (e.g. increased market share, developing new markets, introducing new products/services, etc.) 2. Please provide a copy of your 2016 Marketing Plan. 3. Describe the promotion tools or marketing streams that you utilized to meet your plan's objectives (mass media advertising, sales promotions, direct mail marketing, telemarketing, trade shows, internet, social media, etc.) 4. Please provide information regarding the methods used to evaluate your marketing strategy, the outcomes of strategies outlined in the plan, evidence supporting objectives being met or the ongoing progress of strategies currently in play. 5. Please provide some analysis as to why you feel that the results that you achieved were significant, given the time, effort, and funds you expended putting your marketing plan into action. 6. 14 NEW BUSINESS OF THE YEAR AWARD The New Business of the Year Award is a long-standing award that recognizes the vital importance of new business start-ups in our community. Businesses must be in operation a minimum of one year to a maximum of three years as of October 13, The business' head office must be located in South Eastern Alberta. Major business activity must take place within the province of Alberta. In addition to the general eligibility requirements, applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Outline your company s history and how the company got started. 2. Briefly describe your company s products and/or services. 3. Did you develop a business plan and if so, how closely are you following it? 4. List any significant business achievement(s) within the last three years, memberships, involvement in business associations, clubs and organizations (e.g. awards, operational achievements, sales achievements, volunteer work etc). 5. After being in business for over one year, do you consider your business viable? Explain. 6. Describe the level of customer service that your business provides the customer. What do you do that makes your business stand out from the competition? 7. What are the future plans for your business? Where do you see yourself in 3-5 years? 8. Why do you feel that your company is a good candidate for this award? 9. Please attach any information that supports financial claims of successful business operation. Supporting documents may include: income statements, year-end financials, in-house financial statements, year to date financials, etc. Applicants who provide evidence of financial success will significantly increase the value of their award application. All financial information will be kept strictly confidential and shall only be reviewed with the designated Adjudication Committee members. 10. 15 SMALL BUSINESS OF THE YEAR AWARD The Small Business of the Year Award is given to the small business that best reflects the outstanding spirit and success of our region's small business community. In addition to the general eligibility requirements, applicants must provide details for each requirement as it relates to your organization. Be independently owned and operated. (Note: joint owners are eligible for this award) Be (primarily) Alberta owned and operated. Have 50 or fewer full-time equivalent employees. Have less than $5 million/year in sales revenue. Be profitable in each of the last three years. Demonstrate significant achievement in business. Franchise businesses are not eligible for this award. In addition to the eligibility requirements, applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Describe your business achievements of the past three years. 2. What percentage of your business activity takes place within the Province of Alberta? 3. Describe your active involvement in business and/or professional associations. 4. Describe your involvement in your community at large. 5. Why do you feel your company is a good candidate for this award? 6. Provide any additional information that you think might help us evaluate your submission.

16 SOUTHEAST ALBERTA SHINES AWARD This award recognizes a business, organization or group that has demonstrated outstanding achievement in putting Medicine Hat and/or Southeast Alberta 'on the map' as a great place to live, work and/or visit. The recipient will have demonstrated innovation, commitment, and leadership resulting in the success of a local product, program, service, event or attraction that increases quality of life for residents or attracts visitors to the area. In addition to the general eligibility requirements, applicants must have: Been in operation for at least one year (the product, program, service, event or attraction can be a new initiative within the last year, but the submitting organization must have been in operation for a full year as of October 13, 2017). The core services, products, or the impact of your business/organization be local to, and impact positively on, the South East Alberta Region. Applicants must provide additional information by answering the questions below. Please provide as much information as possible as applications are judged on the information submitted. 1. Describe your product/program/service/event/attraction. 2. Describe how your product/program/service/event/attraction has improved the allure of the local community for residents and/or visitors. 3. Describe how your product/program/service/event/attraction came about (ie: community need, revenue source, uniqueness to area, etc.) 4. Has your product/program/service/event/attraction grown or changed since its inception? If so, how? 5. Describe marketing strategies for your product/program/service/event/attraction and their outcomes. 6. Describe how your organization demonstrates outstanding leadership and innovation. 7. Describe any other recognition or proof of customer/client satisfaction your product/program/service/event/ attraction has received such as awards, comment cards, testimonials, letters of support, etc. 8. Provide examples of any partnerships your organization has formed with other businesses/organizations. 9. 17 YOUNG ENTREPRENEUR AWARD The Young Entrepreneur Award recognizes outstanding achievement by young entrepreneurs. The award is given to the business that best exemplifies the qualities of effective leadership, innovatively applied know-how, and excellent potential for growth; that shows a high level of social, cultural, and environmental awareness and a demonstrated commitment to the wider community; that engages in continuous learning or other improvement opportunities designed to advance the economic, social, and environmental prospects of the enterprise. The business must have been primarily established and continue to be operated or lead by an individual, or individuals between the ages of 18 and 35. To be a valid entry in the Business Awards the business must Have operated in Alberta for a minimum of three years. Be operating in Alberta and continue to do so at the time of the awards presentation. Employ Albertans to produce goods and/or provide services. Not have won in the same category in the past three years. SPECIFIC ELIGIBILITY REQUIREMENTS: The entrepreneur(s) must be between the ages of 18 and 35. The entrepreneur(s) must have played a direct and primary role in the planning and establishment of the business and currently be operating it in a leadership capacity. The business must demonstrate financial viability and sound potential for growth. In addition to the general and specific eligibility requirements, applicants must answer the following questions (where applicable) in as much detail as possible, keeping in mind the eligibility criteria above as applications are judged on the information submitted. 1. Describe the business, its history, products and/or services, financial and other performance indicators, markets and plans for growth, etc. 2. Describe the direct role and contributions of the entrepreneur(s) in the planning, establishment, and ongoing operation of the business. 3. Discuss challenges the entrepreneur(s) encountered in planning, establishing, and operating the business, and explain how they were overcome. 4. Describe any unique or innovative aspects of the business and/or the entrepreneur(s) such as: commitment to education and lifelong learning; workforce development initiatives; environment and sustainability measures; leaders commitment or workforce programs designed to encourage good citizenship, community development, or volunteer activities. 5.
