If all information is not included, the application will be considered incomplete and deemed ineligible. 2 PROVIDE THE FOLLOWING SUPPORT DOCUMENTATION ALONG WITH THIS APPLICATION 1) A copy of the organization s Constitution and Bylaws including the mandate of the organization. 2) Proof of non-profit status. This must be a copy of your organization s Corporate Registry Profile Report. 3) A copy of the generally accepted set of rules for your sport (e.g. rulebook). 4) A detailed listing of all the rental space booked each month for each distinct program from January to December. This must be copies of actual rental contracts for the rental space for the outlined time period. 5) A listing of the number of participants registered in each distinct program, level and/or activity (e.g. beginner, novice, elite, etc.). 6) Membership list(s) identifying each participant s name, address and date of birth. 7) A copy of your last year s financial statement that clearly identifies the amount spent on facilities required to deliver your program(s). In terms of the facility/space being used, how many participants, on average, are involved during a practice/training session? If this number is dependent on the different programs/levels (i.e. beginner, novice, elite, etc.) please identify and explain: C.
