5 Section A Instruction to Bidders In submitting proposals, bidders must comply with all instructions contained in this RfP document. Failure to submit a proposal containing all the specified information and documentation (incl. All completed forms and templates, and a declaration that all the specified ToR will be accepted) within the stated submission deadline will lead to rejection of the proposal. Article 1 - Tender Process Deadlines Deadline for requesting clarifications from the HAAD Last date for issuing clarifications by the HAAD Clarification Meeting Deadline for submission of proposals August 9,010 August 31,010 N/A September 05,010 Article - Packing and Labelling of Proposals Each submitted proposal must comprise a technical offer and a financial offer, each of which must be submitted separately. Each technical offer and financial offer must contain one original, clearly marked "Original", and copies, each marked "Copy". Box 5674 nd floor Note: Proposals submitted by any other means (i.e, fax or ) will be rejected. Any deviation from these instructions (e.g., unsealed envelopes or references to price in the technical offer) is to be considered a breach of the rules, and will lead to rejection of the proposal. The outer envelope should carry the following information: a) The address for submission of proposal indicated above; b) The reference code of the bid to which the bidder is responding (6/G/IT/HAAD/PT/010) c) The name of the bidder. 6 Article 4 - Proposal Contents A. Technical offer The Technical offer must include the following documents: *Letter of Submission on Vendor s letterhead signed and stamped by the person in charge or the Vendor s authorized representative acknowledging the Vendor s agreement to the terms and conditions of this RFP and certifying that all information offered in the submitted proposal are true, accurate, and complete. Full contact details of the key person in the company in case of any clarification requirements. Table of Contents, including page numbers. List of quoted items (UNPRICED) according to Annex II BoQ complete project Plan for the Wireless network implementation. Catalogues of quoted products where applicable A copy of the data sheet, and complete technical specification for whatever will be delivered from products. Show the model number of all the delivered equipments such as but not limited to hardware, software, licenses, services, cables, etc. Submit the Escalation procedures with required contact names, E mail s and mobile numbers in order to solve any technical and non technical problems throughout the life cycle of the project and warranty period. Testing and commissioning procedures for the delivered solution. Any hardware equipments or software s required for implementing those testing and commissioning procedures is the sole and full responsibilities of the Vendor. The bidder should have a minimum of three years experience in UAE Market Copy of valid trade license/ Legal registration documents Business references the overall services experience of the vendor, including customer references for services delivery of a similar nature and volume in the UAE that verify that the bidder has a satisfactory performance record and demonstrate that the bidder has the capability of meeting the project requirements. The references should specify the bidder s relevant contribution to each listed project. References can only be considered if the bidder clearly lists a point of contact in the client organization for that project (name, address, telephone number, etc.). *Detailed CVs for the key experts and project s staff o Bidder should show the name and the technical experiences and qualifications of the engineers and technicians who will be involved in designing and implementing the project solution. In case the Vendor decide to change one of the approved staff after the delivering of the quotation or during the life cycle of the project he has to submit first his technical experiences and qualifications and it has to be approved by HAAD before he will be able to join the team. HAAD reserve the right to request changing any one of team in case they see him not competently qualified and/or has communication problem and/or has behavior problem. 7 Bidder must provid his Support strategy according to the requirements stated in Article 5 Section B. Soft Copy of the Technical proposal. *Bid Bond * Note: After the bid opening and in case bidder did not submit the required documents stated above, the proposal will be administratively rejected without further consideration for review B. Financial offer The Financial offer should be quoted in U.A.E currency (AED) according to Annex II ( BOQ), inclusive of all costs and all applicable tariffs and /or taxes. Bidders should use the financial template (Annex II) in submitting the financial proposal. Notes to bidders in preparing the financial offer: A. It is bidder s responsibility to examine and consider all the RfP requirements while preparing the financial proposals for this tender. B. Quoted prices are not subject to change after proposal submission to HAAD. Article 5 - Bid Bond The bidder must submit a bid bond with an amount of 19,000 AED (Ninteen Thousand Dirham) enclosed in the technical proposal. The bond shall be unconditional, valid for 10 days starting from the closing date of proposal submission and addressed to the Health Authority Abu Dhabi. Priority for issuing the Bid bonds in UAE should be from Abu Dhabi Banks. The bid bond will be returned back to bidders after the evaluation and awarding processes are completed. Article 6 - RFP Terms and Conditions Failure to meet the specified terms and conditions of this RFP at the time of award will result in disqualification of the Bidder. Article 7 - Validity of Proposal Proposals must remain valid and open for the acceptance of the HAAD for 10 days from the RFP closing date. Proposals specifying a shorter acceptance period will be rejected. Article 8 - Incomplete and Late Offers: Incomplete and late proposals will not be accepted. It is the bidder s responsibility to ensure that the proposal is submitted complete, on time and in accordance with the RFP terms and conditions. Late proposals shall be returned to Bidders unopened. Article 9 - Inquiries Bidders may submit questions in writing either through fax or e mail to the following address and before the deadline stated in the table of tender deadlines (Article 1/ Section A). No further clarifications will be given after the stated date. Note: Any clarification issued by HAAD will be communicated in writing to all the bidders & will be published on HAAD website: Any prospective bidders seeking to arrange individual meetings with HAAD or any of its employees concerning this contract during the bidding period may be excluded from the bidding procedure. Article 10 - Alteration of Proposals Bidders may alter their proposals by written notification prior to the deadline for submission of proposals stated in this RFP. No proposals may be altered after this deadline. Article 11 - Eligible Bidders Bidders considered eligible to submit proposals are defined as the entity /organisation that is legally registered in the UAE to do business and can provide a valid certificate of legal registration/ trade registration license. Article 1 - Proof of Non-profit Status Bidders claiming non profit status must provide certification from the registering body with their proposals. Article 13 - Costs for preparing proposals Under no circumstances will the HAAD accept liability for any costs incurred in connection to the preparation and submission of proposals even if the HAAD decides to reject all the proposals or cancel the tender altogether. Article 14 - Clarification During the evaluation process, the HAAD may request additional information from bidders with regard to the submitted proposal if deemed necessary by the tender evaluation committee. Article 15 - Evaluation of proposals The evaluation process will identify and recommend the proposal which is technically superior at reasonable price. The weight of the technical factors = 70 % The weight of the cost = 30 % Only proposals with average scores of at least 70 points in the technical evaluation criteria set out at Annex I qualify for the financial evaluation. 9 C. Technical evaluation of bids The technical quality of each bid will be evaluated in accordance with the evaluation criteria specified in Annex I of this RfP document. No other award criteria will be used. The award criteria will be examined in accordance with the requirements indicated in this RfP. D. Evaluation of financial offers Upon completion of the technical evaluation, the financial offers of those bidders who passed the technical evaluation stage will be opened. Article 16 - Amendments During the proposal submission period, if the HAAD decides to modify/ change any requirement/s of the RfP, [the modification/s shall be released through the issuance of an amendment to the RfP.] Any amendment will be issued in writing and will be sent to all bidders. Article 17 - Confidentiality The entire evaluation procedure is confidential and all proposals are for official use only and may be communicated neither to the bidders nor to any party other than the HAAD. Article 18 - Ownership of Proposals The HAAD retains ownership of all proposals received as part of this tender. Consequently, bidders have no legal right to have their proposals returned to them. Article 19 - Bid Cancellation The HAAD has the right at any stage in the bidding process to cancel the whole bid without justification to any of the bidders. In the event, Bidders will be notified in writing of the cancellation by the HAAD. Article 0 - Discussion/Negotiation HAAD may initiate discussions should clarification or negotiation be necessary. Bidders should be prepared to provide qualified personnel to discuss technical and contractual aspects of the proposal. Article 1 - Award HAAD reserves the option of contracting only for a portion of the specified project scope or of not awarding a contract to any bidder. Final approval to enter into a contract, and the scope of services to be provided pursuant to the contract, rests with HAAD. A contract may be awarded to more than one bidder based on the quality of the proposals and HAAD s needs. Article - Performance Bond The selected Vendor shall be required to secure a performance bond equal to 10% of the total award value within 10 days from the notification of Award and to be in effect for the duration of the Contract and the warranty period of the supplied items. The performance bond may be used to satisfy penalties for late or lack of delivery and/or loss incurred in the event of the Vendor s failure to deliver or perform according to the requirements of the contract. 10 Section B Terms of References Article 1 - General background and HAAD objectives Health Authority Abu Dhabi (HAAD) is a Government Authority for the Emirate of Abu Dhabi with the mandate to regulate the delivery of healthcare services for the residents of the Emirate of AbuDhabi. The primary functions of HAAD are: Enhance the quality of the healthcare services, within the Emirate of Abu Dhabi and to develop policies, standards, strategic plans, capacities and service levels; Establish and enforce regulations governing all aspects of healthcare provisioning for public and private healthcare facilities through the process of licensing and inspection; Issue licenses to all healthcare professionals wishing to practice in the Emirate through credentialing and examination process; Regulate the relationship between health insurance providers, healthcare facilities and patients; Regulate scope of services, premiums & reimbursement rates of providers and payor Determine eligibility, and arrange, for U.A.E. national patients to obtain medical treatment abroad. Article - Scope of Work The Scope of Work includes delivery, Installation at the delivery site stated here in this RfP, configuration and support for the equipments given in Annex II (BoQ) here in. The configuration is not limited to basic but includes almost all features of IOS and Hardware including QOS. Article 3 - General Requirements Bidders who quote for Cisco products must be silver or gold partner. The deliverables must be mentioned clearly in detail with a complete project Plan for the Wireless Network implementation. Bidder should assign a project manager to handle the project which will be the main contact person for HAAD for the entire duration of the project. The project manager name, his technical experiences and qualifications, , and mobile number must be submitted in the proposal. In case the Bidder decide to change the project manager after the delivering of the quotation or during the life cycle of the project he has to submit the new candidate name and technical experiences and qualifications and it has to be approved by HAAD before he will RFP #: 6/IT/G /PT/HAAD/010 Page 10 of 16

11 be able to join the team. HAAD reserve the right to request replacing the project manager in case they see him not competently qualified and/or has communication problem and/or has behavior problem. Throughout the life cycle of the project, Vendor project manager must provide regularly on weekly basis and upon requested status and progress reports on the achievement of the project. Throughout the life cycle of the project, HAAD representatives will have the right to request regular and non regular meetings any time to follow up with the Vendor project manager on the achievement of the project. Any regular or upon requested meetings by HAAD representatives will be held in HAAD and/or any other places as designated by HAAD representatives. All Management software must be Microsoft Windows Based and should support the latest Microsoft OS releases, Servers specifications are already mentioned, Microsoft Windows OS will be provided by HAAD. Article 4 - Delivery, Installation & Configuration: Requested items must be delivered, installed, functioning and tested as per the tender requirements. All the access points should be deployed through the controller and should be operable centrally from the controller. If there any extra software, like for detecting rouge access point, checking the coverage area of an AP etc. the advantages of software and its cost should be mentioned by the bidder as an optional item. Vendor is responsible for providing the licenses required for the function of the delivered items. It specifies proper licenses for number of access point specified and security features like encryption (18 bit or MD5), integration with external authentication server etc. and even guest portal, which vendor thinks will enhance the standards can be specified along with proper description. Cabling must be properly pulled, terminated tested and labeled as per HAAD standards. Cabling solution must include trunks, pipes, cable trays, patch panel. Any civil works requires must be clearly mentioned. Equipments must be properly labeled as per HAAD standards and as requested by HAAD project Manager. Delivered Solution must be tested and commissioned as per Vendor delivered procedures before handing over the project completion. Any hardware RFP #: 6/IT/G /PT/HAAD/010 Page 11 of 16

12 equipments or software s required for implementing those testing and commissioning procedures is the sole and full responsibilities of the Vendor. All Management software (Whichever management software for wireless is provided should be operable from Microsoft Windows) must be Microsoft Windows Based and should support the latest Microsoft OS releases, Servers specifications are already mentioned, Microsoft Windows OS will be provided by HAAD. Article 5 - Support & Warranty Conditions Support & Warranty must include but not limited to: 3 years warranty with On Site support for all the products of the solution whether hardware or software Warranty of Cabling must be minimum 15 years. Quarterly Periodical maintenance. On call maintenance response time two hours, rectification time four hours, 4hours a day, and 7 days a week. Onsite, , and telephone support. Resolving any problem related to installation, configuration, integration, software and hardware OS bugs, etc. Delivering, installing and configuring software patches, upgrading software and hardware OS to minor releases, IPS signatures updates, etc Vendor must provide in their quotations the software and hardware OS upgrading strategy whether it s minor or major releases. Warranty of the equipment will begin after the project delivery, testing and operation acceptance is signed by HAAD. A completion letter must be signed by both parties HAAD and the Vendor. Only after signing the completion letter it will be consider the beginning of the warranty period. During the warranty period for each visit whether periodical or on call whether preventive or corrective maintenance the Vendor contact person must provide HAAD before he leaves the site a technical report of the work which has been achieved and must be signed by HAAD contact person and Vendor contact person. Vendor must assign a dedicated, on call, non resident engineer for after project completion support and warranty. Vendor must submit in their quotations the name, the technical experiences and qualifications, E mail, and mobile number of the support engineer. In case the Vendor decide to change him after the delivering of the quotation or during the warranty period he has to submit first the new candidate technical experiences and qualifications and it has to be approved by HAAD before he provide service. HAAD reserve the right to request changing the engineer in case they see him not competently qualified and/or has communication problem and/or has behavior problem.
