Introduction The objective of the Stichting IplusANBI is to serve the general purpose of social, cultural and moral well- being of the society by contributing to the accessibility of health care for both individuals and communities in Low and Middle Income countries. The objective of the Stichting IplusANBI is closely related to the i+solutions Mission and Vision: universal access to healthcare, and empowering countries to improve their health supply chain systems. Funds of the Stichting IplusANBI could therefore be used to finance activities and projects executed by and in the interest of the future of i+solutions. Whereas i+solutions generates income by implementing donor- supported supply chain management ( SCM ) projects, the Stichting IplusANBI will be a vehicle to attract additional funding to address the gap in access to health. The Stichting IplusANBI has more freedom to invest in initiatives that have long- term capacity building targets, because its activities are not formulated by institutional donors and governments, and their project requirements. The Stichting IplusANBI can also invest in innovative projects as an early adopter that may be considered risky for donors/funders or other important priorities for which currently no funding is available or are overlooked because of political or other factors. In the coming years, in line with the ANBI strategy, IplusANBI will focus on its spending on the following categories: 1. Subsidy for individuals and organizations in developing countries for training and capacity building. 2. Investment in local capacity building via i+solutions regional offices. These offices are meant to connect with health decision makers in selected countries with whom we co- create capacity building solutions for the future. 3. Innovative projects with game changing impact on health supply chain systems. Page 2 of 11
3 2. Actual Stichting IplusANBI budget This section will describe the anticipated investments of Stichting IplusANBI funds, and how these investments align with the future of i+solutions. Most activities in 2016 have been implemented. The planned activity to develop strategic trainings aimed at senior level employees of Ministry staff has been postponed until 2017 (Activity 4). Below is the budget indication for the activities planned in Seq. With the large international procurement contracts for HIV and malaria being covered in existing long term projects (USAID funding into the GHSC- PSM project managed by Chemonics, and the Global Fund funded into the PPM project managed by PFSCM/i+solutions) we intend to develop our expertise in non- communicable diseases (NCD) within i+solutions. We believe that new Procurement Service Agency (PSA) and consultancy contracts are likely to come in the field of NCDs. To prepare the organization for proposals in this area, both sourcing and demand expertise must be developed. We have been exploring cooperation initiatives with Medicus Mundi partners such as Cordaid in In 2017 we have concrete plans to build on our relationship with Cordaid and start pilot activities in one African country. Objective: Develop the expertise in non- communicable diseases (NCD) within i+solutions and build relationships with other stakeholders. By the end of 2017 we would like to have one project in NCDs and submit at least three proposals in this area. Activity plan: 1. Conduct analysis of selected NCDs in this/these pilot country(ies) with regards to magnitude, interventions, partners, existing gaps etc. 2. Identify and engage with different key actors in the field of selected diseases with particular focus in selected pilot country(ies) Page 3 of 11

To increase the use and visibility of the i+academy, a scholarship program was launched in In 2017 the i+solutions Management Team would like to continue this scholarship program. This year s program will give preference to government and locally- owned NGOs in specified priority countries. The scholarship programme provides participants without the financial means, access to quality training on supply chain management, thereby strengthening local health systems. The number of applicants in 2015 was 86, in 2016 this was 102. 5 2.3 Pharmacy internship program in SCM, Tanzania Though this intervention is a step in the right direction towards building human resource capacity in supply chain management, a gap still exists between theory and actual practice. The gap exists primarily because the focus of the compulsory pharmacy internship is usually clinical/hospital pharmacy practice alone, which does not cover pharmaceutical supply management in detail. It is good to note that a sizeable number of pharmacists work in the field of pharmaceutical supply chain management. It is therefore crucial to provide hands- on SCM skills as part of the compulsory one- year internship. Objective This 3 month work attachment program, as part of the compulsory one- year internship, is targeted at building the capacity of intern pharmacists in managing pharmaceutical supply chain systems for improved medicine and health product availability in Tanzania. i+solutions will provide a course for each intern. Beneficiaries: Government of Tanzania, private sector, non- governmental organisations. Subsidize 1 online couse per intern for (390 euros) for 13 interns 5,000 Total Euro 5, Contribute to i+ solutions Training Stratey The i+academy has grown to 2,000 users in 2017, from 1,200 in January 2016, and currently provides 18 courses in 4 languages. In order to sustain, expand and fully utilize i+academy potential as a training tool and communication medium, there is currently a need for investment, which will decrease over the years. As was presented in the multi- annual strategy, we recommend a 30% investment from IplusANBI, with the remaining 70% of the 2017 budget being covered by customers, participants and overhead. This represents 50,000 euros. In 2016, a multi- year strategy was developed for the training department, focusing on the future of i+academy which delivers around 25 courses a year, both within projects and also for the general public. More than a training tool, i+academy contributes to the visibility of i+solutions, and is an asset for business development. i+academy demonstrates our core competencies in providing quality procurement and supply services in alignment with our skill development focus in LMICs i+academy serves as a knowledge repository for i+solutions i+academy serves as a tool for participant administration i+academy serves as a networking tool, reaching out to a 2000 strong user base The platform creates sustained communities of learning and practice in SCM Page 5 of 11
6 The platform provides: Courses in English, French, Spanish and Portugese. Courses in specific areas such as good manufacturing practice and quality assurance offer great potential in certain targeted professional circles (manufacturing, regulatory authorities e.t.c). i+academy also provides a platform for sustainable coaching and mentoring for health workers without the need for physical presence. It offers an interface to partner with training providers using different training methods, or different technical expertise. It brings in content based on hands- on experience in a variety of procurement and supply chain projects and consultancies. To allow for the continuous delivery and update of courses, part of the i+academy operating budget needs to be secured independently of the income coming from other training projects. This will allow for a consistent output of quality content to health care professionals around the world. This amount will serve to fulfill the following deliverables: Update all curricula Deliver ca. 25 course sessions through 2017 Enhance i+academy graphic design Revamp the quantification course in English and French Revamp the procurement course into a stand alone unit Pursue partnerships with new institutions (academic, private entities ) Seek further accreditaions Communicate about the platform ( quarterly newsletter, list serves and social media) Update curriculum and hold 25 course sessions (10,000 consultancy + 15 days) 20,500 Revamp procurement course and F&Q course in 2 languages (10 days) 7,000 Pursue partnerships with new institutions (academic, private entities ) and advertise the platform (20 days + 3,500 consultancy) 17,500 Enhance i+academy graphic design ( consultancy contract) 5,000 Euro 50, Rebuilding of the PSM toolbox website It has been 10 years since the PSM Toolbox website was created. The website aims to disseminate information on the available tools in Procurement & Supply Chain management, and facilitates easy access to updated information for health professionals. At the moment the website accounts for approximately 600 tools in 7 different languages. During a Technical Working Group meeting that was held in Amsterdam in December last year, it was highlighted that in order to improve the usability and visibility of the website on searching engines, it is necessary to rebuild the entire website making it future proof. At the moment Page 6 of 11
7 the website does not score high in the search results such as Google. The reason for this is the evolvement and changes of the online techniques and much bigger competition. In order to make PSM Toolbox future proof it is really necessary to build a new SEO (Search Engine Optimization) friendly website. In several reports made by internationally recognized organizations, Kenya is mentioned as business opportunity country number one for the coming 3-5 years. i+solutions currently has limited activities in East Africa due to its past focus in other areas. In 2017, i+solutions will use funds from the Stichting IplusANBI to expand our business relations with in- country partners and stakeholders. We are aware that reform of the health supply chain in Kenya is creating procurement and supply chain challenges, and believe this is the moment to start the dialogue with the Ministries of Health and other countries to offer i+solutions services and programs. This investment is expected to result in training and consultancy assignments starting in Our ontinued presence in Rwanda will guarantee the identification of new leads and/or projects in co- operation with the Ministry of Health and UNFPA. Objective: Explore business opportunities in East African countries and develop a business case for opening an office in Kenya or other East African country. For every woman who dies, there are approximately thirty (30) others who suffer infirmity. These morbidity and mortality rates cause around 5 billion dollars of productivity losses in the region. The sub- region is also characterized by the highest fertility rates in the world, with a very low contraceptive prevalence and an extremely high total fertility rate. About 25% of married women age would like to space or limit births, but are not using modern contraceptive methods, mainly due to the inaccessibility of family planning services. This is mainly due to a weak health system, socio- cultural barriers, timid political commitment and limited funding. 1 Regional and global initiatives are underway to address the above through the following: The Ouagadougou partnership 2 The SWEDD project 3 FP Increased bi- and multilateral support. (WB, UNFPA and governments of more developed countries) Increased participation in the SRHSC System Strengthening Working Group During an FP2020 Focal Point Workshop in June in Abidjan, to which i+solutions was requested to dedicate someone to act as a resource person for supply chain management, it became clear that a central problem is last mile delivery and optimal availability. There is lack of timely and accurate visibility of consumption data and stock movements at the level of health facilities, resulting in uninformed distribution of product which results in all the negative consequences of unavailability, excess stock and expired goods. An 1 Ouagadougou partnership website accessed The Ouagadougou Partnership was launched in Ouagadougou, Burkina Faso in February 2011 at the Regional Conference on Population, Development and Family Planning held by the nine governments of Francophone West African countries and their technical partners and financial resources to accelerate progress in the use of family planning services in Benin, Burkina Faso, Côte d Ivoire, Guinea, Mali, Mauritania, Niger, Senegal et Togo. The main objective of the Partnership is to reach at least 2.2 million additional family planning methods users in the nine countries by Additional financing made available to Sahel countries with the objective to improve Sahel Women s Empowerment and Demographic Dividend. The project development objective is to increase women and adolescent girls empowerment and their access to quality reproductive, child and maternal health services in selected areas of the participating countries, including the recipients' territory, and to improve regional knowledge generation and sharing as well as regional capacity and coordination. 4 FP2020 works with governments, civil society, multilateral organizations, donors, the private sector, and the research and development community to enable 120 million more women and girls to use contraceptives by Page 8 of 11
9 affordable solution to register logistics data, report consolidated stock status and inform supply chain management decisions that lead to optimal availability of stocks is required. Activity Plan 1. Together with a selected Francophone country (most likely Benin or Cameroon), develop and implement an affordable concept to ensure timely and accurate visibility of consumption data and stock movement data at all levels of the health system. This will support forecasting and quantification, procurement, importation, storage and distribution along the in- country supply chain up to the service delivery points. 2. Identify appropriate reporting tools and software. 3. Identify in close co- operation with the Ouagadougou partnership, UNFPA WCARO region and FP2020, which countries could benefit. 4. Engage with selected country governments and the relevant development community to confirm SCM needs and related interventions. Senior consultant 20 days Euro 25,800 Euro 17,200 Two regional visits of which 1 accompanied by the technical expert Euro 12,000 Total Euro 55, Improve access to male and female condoms / Condom Caucus Male and female condoms are the only commodities currently available that offer triple protection; against HIV, other sexually transmitted infections (STIs), and unintended pregnancies. Despite considerable support from donors, the quantity of condoms procured and supplied by the public sector over the past 10 years has fluctuated between 2.2 billion and 3.4 billion per year. Of these, 60% to 80% go to Africa. In 2013, the total number of condoms procured showed a deficit of roughly 50% for male condoms and 66% for female condoms. The condom gap is widest in Sub- Saharan Africa, where most people rely on free condoms provided by governments and social marketing organizations (SMOs), who in turn are heavily dependent on donors for funding. Globally however, donor funding for male and female condom procurement is being reduced, and as a result gaps in availability of free condoms in Africa are expected to be further on the rise. Building and developing a private market for condoms will be crucial to limit this gap. The role of private manufacturers and distributors in reshaping the condom market through their active engagement in building a private market will be crucial in reaching these objectives. The United Nations Population Fund (UNFPA), in collaboration with the World Bank, the Reproductive Health Supplies Coalition (RHSC), USAID and ILO launched a new initiative to increase the availability, access, and use of 20 billion condoms in low and middle income countries by the year 2020: the 20 by 20 initiative. 10 As part of the 20 by 20 Initiative, the private sector participants have formed a Private Sector Condom Group (PSCG); a coalition of trade organizations, consisting of condom manufacturers and distributors who have a vested interest in creating a sustainable supply model for condoms in Africa. Private sector condom manufacturers find it difficult to gain a commercial foothold in markets where most people get their condoms for free. Barriers to market entry need to be addressed and total market approaches need to be developed.these market approaches should lead to enabling manufacturers to develop a private market segment that provides condoms to those wealth quintiles that have sufficient ability to pay and that offers a sustainable business to condom manufacturers and distributors. This will enable governments and donors to reserve scarce public and donor funding to the indigent population that cannot afford to buy condoms themselves. Six markets with the most potential were selected to become the first target countries for the 20 by 20 private sector initiative: South Africa, Zambia, Zimbabwe, Kenya, Botswana, and Namibia. Because of the unique experience which i+solutions acquired through working with the Universal Access to Female Condoms, its technical capabilities in the field of access to medicines in low and middle income countries and in depth knowledge of and networks amongst manufacturers, governments of LMICs and the development community at large - i+solutions is considered to be a neutral and knowledgeable organization and was invited to lead the PSCG. Activities 1. Assist governments and donors to allocate an increased portion of scarce public and donor funding towards the indigent population that cannot afford to buy condoms themselves. 2. Assist manufacturers and distributors for male and female condoms to reduce market access barriers and initiate total market approaches that lead to sustainable private market segments in targeted LMICs. 3. Lead the PSCG to become a meaningful collaborating partner to the Reproductive Health Supplies community that pursues objectives, strategies and actions that are contributing to objectives 1 and 2 above. Efforts will be undertaken for PSCG to become a caucus to the Reproductive Health Supplies Coalition in the course of This Coalition includes over 360 institutions from across the "total market" for reproductive health supplies.
