1 Chapter Alpha Epsilon Pi Alpha Epsilon Pi Violation University of Arizona Hazing Policy Sanction Completion of Sanction 1) Warning for Case 21F-11 1) Your Chapter President, Vice President, New Member Education Chairman and Scholarship Chairman are to create a new member manual to hand out to your Spring 2012 pledge class members that lists a detailed new member calendar including new member meetings, study tables, initiation, committees, expectations, important dates, Alpha Epsilon Pi roster, fraternity history, etc. This new member manual must be approved by Johanne Ives of the FSP office by January 23rd 2012 before it is distributed to new members. 2) 100% of your Spring 2012 pledge class needs to meet with Jenny Nirh to talk about the new member process. 25% of the Spring 2012 pledge class needs to meet with Jenny by February 15th % needs to meet with Jenny by March 19th % needs to meet with Jenny by April 13th And the final 25% needs to meet with Jenny by April 30th ) Your new executive council needs to create a hazing presentation to present to your chapter(both active and new members) by February 20th Jenny Nirh must approve the presentation before it is given. 4) We recommend that Alpha Epsilon Pi dismisses of the designated driving program or makes it a requirement of all

3 Alpha Delta Pi Events with Procedures/GAMMA Paperwork Alpha Delta Pi must have their Social Chair and Assistant Social Chair attend the April 2012 GAMMA meeting Alpha Phi Code of Coduct #11: Disrupting University Environment Alpha Phi must write a letter of apology to Pima Dormitory by May 2nd, Upon completion, this letter should be given to Johanne Ives who will deliver the letter to the Community Director of the Residence Hall. Alpha Phi Gamma Philanthropy Advisory Policy The future President and Philanthropy Chair must meet with the PAB Director at least 4 weeks before your next philanthropy event to ensure that all forms are turned in on a timely basis. Alpha Sigma Phi Events with Procedures 1) The GAMMA Chair s will make a presentation about the FSP Events w/ Policy procedures and rules to the Alpha Sigma Phi chapter before March 31st Alpha Sigma Phi will need quorum at their meeting and for the GAMMA Chair s to acknowledge there is quorum at the meeting. 6 Delta Lambda Phi Failure to Pay FSP Dues 1) Make a partial payment of your FSP owed dues by December 12th ) Work with Jenny Nirh to set up a Budget & Fundraising Workshop by March 1st 2012 Dean of Delta Sigma Phi Violation of the University of Arizona Hazing Policy, Offcampus conduct that a reasonable person would believe may present a risk or danger to the health 1) Complete a comprehensive overhaul of the new member program to remove all activities that involve of hazing and submit the new program for review. Delta Tau Delta Procedures/Walk Through 1) Your Executive Council and Social Chair must have 100% attendance at the October 2011 and November 2011 Gamma

9 Lambda Theta Phi Philanthropy Advisory Policy 1) Turn in the PAB Recap form to the PAB Director,by May 2nd, ) The Chapter President and Chapter Philanthropy Chair must set up a meeting with the PAB Director, to go over the PAB checklist they created to ensure it is inclusive of all policies. This meeting must take place during the Fall 2012 semester, prior to September 1st, ) The current Chapter President must create a transition binder to pass down to the next President which will include all policies for GAMMA, PAB, and other frequently used FSP procedures. Omega Delta Phi 1) An apology letter to the Gamma Co-Chairs by October 1st ) Obtain the notes/powerpoint from the Gamma meeting you missed. 10 Phi Beta Sigma NPHC and Student Organization Policy: Failure to Pay Council and Office Dues 1) The chapter must pay the debt of their dues balance before they are elligable to become a recognized orgnization Phi Delta Theta Events with Procedures 1) Warning for Case 11S-12 Phi Gamma Delta 1) Warning for Case 18F-11 Dean of Phi Gamma Delta Off-Campus Events with Procedures, Code of Conduct Violations for University Rules and laws governing alcohol use, distribution and sale, Off-Campus conduct that a reasonable person would believe presents a danger or risk. 1) University Probation until May ) Host a retreat with National Fraternity Staff to discuss chapter risk management around alcohol, 3) TIPS training for all live in members 4)No events with alcohol until Oct. 31st, 2011
11 IFC Phi Gamma Delta IFC Fall 2011 Recruitment Policy GSB upheld the IFC policy and decsion to fine Phi Gamma Delta and to prohibit Phi Gamma Delta from participating with IFC in the recruitment process for one full year ending after the fall 2012 recruitment Phi Gamma Delta Off-Campus Events with Procedures The President and Social Chair must meet with the GAMMA Chairs at least 4 weeks prior to the next scheduled weekender event to ensure that all contracts have been planned and forms are received in a timely manner. Phi Gamma Delta Greek Community Policy/Student Code of Conduct #11 Disrupting University Environment The GSB supports the chapter's decsion to cancel this event in the future.
12 Phi Kappa Psi Procedures, Unregistered Event 1) Any violation of the IFC Rush Policy in Spring 2012 or Fall 2012 will result in no participation in formal rush for the next 2 following semesters. 2) The IFC executive board will review the Spring 2012 rush policy with 100% Phi Kappa Phi chapter membership attendance by December 9th ) Phi Kappa Psi will be required to present a presentation at the Spring 2012 IFC Rush Informational called The Harms of Abuse. This presentation must be 10 minutes long. Phi Kappa Psi must work with the IFC VP of Membership and IFC Advisor to create it and Phi Kappa Psi must be unaffiliated when presenting. 13 Phi Kappa Psi Procedures, Unregistered Event 1) Phi Kappa Psi will be on Activities Suspension during February Phi Kappa Psi however may have any event except a social event. This includes any registered alcohol or non-alcohol social event. For example, you may not have a house party this weekend or date dash the month of February. You may host a dinner exchange, philanthropy, fundraiser, community service, athletics, chapter, etc Not Applicable due to permanent loss of recognition Phi Kappa Psi 1) Warning for Case 23F-11 Phi Kappa Psi Student Code of Conduct Hazing Policy 1) Have a meeting/workshop with Jenny Nirh by March 1st 2012 to go over FSP Policies and Educational Programs such as; PAB, On and Off Campus Events with, Greek Community, Communication, Step Up! and more. Phi Kappa Psi s executive council and Spring 2012 New Member executive council will be required to be there. 2) The Greek suggests not conducting any active brother or new member auction in the future. Not Applicable due to permanent loss of recognition
14 Dean of Phi Kappa Psi Violations of the University of Arizona Hazing Policy Removal of Greek Letter Recognition until May 2016 Pi Kappa Alpha On-Campus Event with Procedures/Registration 1) Warning for Case 25F-11 but if a Gamma rule is found broken in Fall 2011, Pi Kappa Alpha will be sent straight back to standards for a tougher sanction. Pi Kappa Alpha Philanthropy Advisory Policy 1) Pi Kappa Alpha s executive board will sit down with the Philanthropy Advisory Director before the end of the Spring 2012 semester to thoroughly go over all of the Philanthropy Advisory rules to ensure this does not happen again. Pi Kappa Alpha Off-Campus Events with Procedures/GAMMA Paperwork 1) Pi Kappa Alpha must register all Fraternity Formals in the future 2) Pi Kappa Alpha must register a Fraternity Formal at least 4 weeks ahead of time and have the President, Vice President and Social Chair sit down with the Gamma Co Chair to ensure all of the details and information is presented 4 weeks ahead of time.
15 Pi Kappa Phi Procedures/Invitations 1) Warning for Case 24F-11 but if a Gamma rule is found broken in Fall 2011, Pi Kappa Phi will be sent straight back to standards for a tougher sanction. Dean of Pi Kappa Phi Violations of the University of Arizona Hazing Policy 1)Monetary Sanction 2) New Member Education Program Review and Revisions 3) Wrap up to discuss New Member program No Pi Kappa Phi Events with Procedures 1) Turn in the GAMMA Educational Policies form by March 30th You can find this form on the FSP website. 2) Complete the GAMMA Education Quiz. 80% of your active membership and 100% of your Spring 2012 new members must complete the survey by April 6th 2012 No
16 Sigma Alpha Epsilon On-Campus Events w/ Policy/Student Code of Conduct 1) Create a calendar for the Fall 2012 semester that will outline which members will act as responsible sober officers each weekend night. 2) The President and Executive committee must create an action plan/risk management manual of what proper procedure will be when any person s safety is in question, regardless of their gender or affiliation. The plan should specifically include the protocol for assisting when someone is too intoxicated and needs medical assistance. 3)Every member living in the chapter house during the Fall 2012 semester must complete the Step Up Bystander Intervention Program before Sigma Alpha Epsilon is allowed to GAMMA register any event for the school year.
