These clinic services are primarily provided by local health departments, hospitals and Federally Qualified Health Centers (FQHCs). The information contained in the document is from organizations that responded to a "Dental Health Services Survey" conducted by the New Jersey Department of Health. Program participation requires a referral from a dentist and application. Other program requirements include: a complete application with all required documents and a $25 non-refundable application fee.
