8 NETRC OFFICE Our Client The Northeast Telehealth Resource Center (NETRC), on behalf of the National Consortium of Telehealth Resource Centers Project Goals Increase physician awareness, interest, and willingness to participate in telehealth programs (primarily live videoconferencing) Focusing on increasing access to health care in rural and other underserved communities 8
9 Needs Assessment Who is the target group? Health Care Access in Rural Areas Chronic health conditions are more prevalent among rural individuals Socio-economic challenges disproportionately affect them Transportation difficulties, limited healthcare supply (density of health providers), lack of quality health care, geographic isolation, and financial constraints Discrepancy in health insurance coverage (10.4% less) Bennett et al., 2008; National Center for Health Workforce Analysis, 2010; Larson and Hill

11 Literature Results Tip: Before designing an intervention review the literature on the topic. This will allow you to identify potential strengths and weakness of other programs you may want to consider 11
12 Literature Results Social Ecological Model Tip: When planning an intervention, use the Social Ecological Model to ensure that all factors are being considered. This will help determine the target for the intervention. 13 Literature Results Providers are the main gatekeepers of telehealth patient use. Key factors to engage providers in using telehealth: Interpersonal level components are any formal and informal social networks and social supports systems, including family, work groups, and friendship networks, among others. (McLeroy, Steckler, and Bibeau, 1988) Positive acceptance Trainings that facilitate proper use of this technology Patient-centered communication Brooks, Turvey, & Augusterfer, 2013; Kingue, Angandji, et al., 2013; Agha, Schapira, et al., 2009; Whitten & Mackert,

15 Aim of Tele-T Intervention Health and Quality of Life Outcomes Target Population: Adult Underserved Residents of Rural Massachusetts with Chronic Health Conditions Tip: Intervention planning: research conducted informs about existing gaps that need to be addressed in the intervention. Tip: Consider some practical tools that facilitate the intervention planning, and evaluation processes such as the logic model. Increase telehealth usage by both providers and patients Improve access to healthcare for chronic disease Improved chronic disease outcomes in patients due to access of care 15
16 Tele-T Intervention Goals of The Training: Providers will... Understand how and when to advise patients to use videoconferencing services Know that telehealth services can increase access to appropriate care Tip: In intervention planning, it is essential to personalize your intervention to the target group by conducting a needs assessment review, observations, and/or interviews. Be able to explain how, when, and where to access telehealth services Believe that telehealth services are an appropriate alternative to in-person care Be able to explain the personal benefits and advantages of using telehealth services 16
17 Tele-T Intervention Tip: Have a paper/ electronic prototype, or mock-up to test the intervention idea with your target group and stakeholders to get feedback (wireframes). Online Training Modules Modules will consist of brief 5- minute training videos that present information relevant to providers in an engaging and interesting way. In-Person Extended Trainings In-Person Trainings will be 30- minutes in length and will be tailored to specific organizations and hospitals. These trainings will present telehealth information in a way that is relevant to the providers and administrators in their unique environment. 17
18 Tele-T Intervention: Online Training Tips: 1. Create modules using physician s core values and framing theory. 2. Include only direct and essential information that is easy to learn. 3.
