COMPLETED DA FORM 4187 (PERSONNEL ACTION) Must be signed by Soldier and Commander. If Soldier is unavailable for signature (i.e, inpatient) this must be annotated on the form). DA FORM 3349 (PHYSICAL PROFILE) Completed by the military medical authority. COMPLETED DA FORM 7652 (COMMANDER S PERFORMANCE AND FUNCTIONAL STATEMENT). STATEMENT FROM MEDICAL PROVIDER Will include, at a minimum, diagnosis, anticipated length of care, Primary Care Manager contact information, treatment plan, prognosis for recovery, and other medical documentation to substantiate the medical condition. STATEMENT VERIFYING SOLDIER IS NOT PENDING UCMJ PROCEEDINGS OR ADVERSE ADMINISTRATIVE ACTIONS AND THAT ETS, MRD, OR RCP WILL NOT OCCUR DURING THIS ACTIVE DUTY PERIOD.
