Upon arrival I assisted with perimeter around the Nightclub until S.W.A.T. units could arrive on scene. Once S.W.A.T. units took over perimeter I assisted with removing victims from the night club, so they could be transported to the hospital. S.W.A.T. took over the scene and I assisted with security at the Orlando Regional Medical Center. This concludes my involvement in this incident. I responded with my trainee, Deputy Darren Shields. On arrival, I made contact with several law enforcement units that advised the suspect was barricaded inside the establishment with multiple hostages. I was directed to make contact with Seminole County units at Einstein Bagels, which is directly across the street from Pulse Nightclub. Once at the Einstein Bagels parking lot, I saw multiple victims on the ground deceased, with apparent gun shot wounds. These victims were in the custody and care of multiple Orlando Police Officers and medical personnel. Deputy Shields and I maintained our perimeter position with several other units at Einstein Bagels until I made contact with Sgt. Luge and Sgt. Risher, who directed us to move to the new staging area at the Ragin Cajun Bike Shop, just north of Kaley Avenue. Deputy Shields and I maintained a ready status at that location until we were summoned by radio for victim extraction from the club. Incident Command requested any law enforcement units with pickup trucks respond to the rear of Pulse Nightclub to assist in transporting victims to the hospital. I subsequently responded in my Ford F-150 patrol vehicle to the rear of Pulse Nightclub with several additional officers with pickup trucks. Once there, I began assisting units by helping load victims in the beds of other pickup trucks. One unidentified female, who appeared to have been struck by multiple gun shot wounds, was being escorted out of the back of the club. As she was being escorted, she collapsed on the ground before making it to our trucks. At that time, with the assistance of two other officers, I grabbed the injured female around her legs and we picked her up off the ground. We then carried the female several yards across the parking lot and loaded her into the back of a pickup truck where she, along with many other victims, were quickly transported to ORMC. Deputy Shields and I maintained a ready status behind the club awaiting any more victims for transport to the hospital. I was advised shortly thereafter that my pickup truck would not be needed for transport as all remaining victims inside the establishment were deceased. Deputy Shields and I maintained our position behind the nightclub until I was told to secure by Sgt. Risher. I never made entry inside the establishment at any time. This concludes my involvement. 3 On June 12, 2016 at approximately 0230 hours I, FTO Joseph Meyer with recruit Officer Richard Scovel, responded to assist the Orlando Police Department during an active shooting incident at the Pulse Nightclub located at 1912 S Orange Avenue, Orlando, Orange County. Upon our arrival, we staged at the 7-11 at the corner of Kaley Street and Orange Avenue. We later met with Sgt. Pina with Orlando Police Department, who instructed us to respond to Florida Hospital South for scene security at the Emergency Room. We arrived and remained at the ER for approximately one hour. Orlando Police on scene then advised us the ER had enough law enforcement on scene, and to respond back to the staging area for further instruction. We returned to the 7-11 and were later instructed to assist in evacuating the area 00 feet from the Pulse Nightclub due to a possible bomb on scene. We responded to the Subway at 1742 S. Orange Avenue and met with three store employees and two citizens that were inside Pulse when the shooting occurred. We instructed everyone to evacuate and walk north. We instructed the two possible witnesses to travel to the witness staging area on Hollenbeck Drive. We then remained in the area until the scene was secure and the shooter was dead. We then returned to the City of Oviedo and went off duty at approximately 0720 hours. No further action was taken. Upon arrival, I linked up with other SCSO deputies and our supervisors in charge, Sgt Luge and Sgt Risher. I was advised to check in at Command Post and standby for instructions. My first assignment was to block traffic at Kaley and Orange Ave, and only allow emergency vehicles through. At this time, the scene was still very active and the negotiation team was in contact with the suspect inside. At the time, all I was aware of was hostages located inside the night club and that the suspect was armed and potentially had explosives. I tried to keep contact with an Orange County unit to maintain situational awareness on the scene as my radio had poor connection even with a patch. I was conducting my traffic assignment with an Orange County Deputy when a report came out of shots fired at Orlando Regional Medical Center. The report was from both law enforcement and witnesses. I did not hear any shooting, but the Orange County Deputy and I were very close to the hospital so we traveled on foot to the loading dock and linked up with another two man element and began sweeping the area. The hospital went into immediate lockdown procedures. I observed approximately 2 or 3 other elements sweeping as well and we made sure to keep constant contact with each other. There were no shots fired during this evolution and we could not confirm the reported shootings at the hospital. Orlando PD did make an arrest while we were conducting our sweep of the area, however it is unknown if it was related to the reported shootings. I then checked back in to command post and received another assignment to report to Winter Garden Hospital and advise if any victims had been admitted. Myself and D/S Lopez made contact with hospital staff and they advised no one from the Orlando incident was there. While at the hospital SWAT reported shots fired at the Orlando night club again. I advised hospital staff to immediately contact Winter Garden PD if they had any victims and then ran emergency back to command post in reference to the second wave of shooting. Upon arrival at command post, the perimeter was expanded due to the threat of explosives, however, it was confirmed that the suspect was down. I maintained security of the scene and assisted in any other ways requested until we were finally instructed to report to a new command post at Orange Ave and Harding. At that location I was relieved and went out of service. 4 On June 12, 2016 at approximately 0230 hours, I responded to assist the Orlando Police Department with an active shooter incident at the Pulse Nightclub located at 1912 South Orange Avenue, Orlando, Florida, Orange County. I responded with my Field Training Deputy Z. Seal to the scene. On arrival, Deputy Seal and I made contact with several law enforcement units that advised the suspect was barricaded inside the establishment with multiple hostages. My field trainer and I were directed to make contact with the Seminole County units at Einstein Bagels, which is directly across the street from the Pulse Nightclub. I observed multiple victims on the ground deceased, with apparent gun shot wounds. Deputy Seal and I maintained our perimeter position with several other units at Einstein Bagels until we made contact with Sgt. Luge and Sgt. Risher, who directed us to move to the new staging area at the Ragin Cajun Bike shop, just north of Kaley Avenue. Deputy Seal and I waited at that location until Incident Command requested any law enforcement units with pickup trucks to respond to the rear of Pulse Nightclub to assist in transporting victims to the hospital. Deputy Seal drove his F-150 patrol vehicle to the rear of Pulse Nightclub alongside other officers with pickup trucks. I was in the front passenger seat with Deputy Seal. I began assisting units by clearing the back of the truck of items and handing out latex gloves to other law enforcement officers. I provided hand disinfectant to several law enforcement officers who had been in contact with blood. Once there, I was ready to assist units by loading victims in the beds of other pickup trucks. I observed two unidentified females, who appeared to have been stuck by multiple gun shots wounds. The females were escorted to the back of other pickup trucks as I stood nearby waiting to assist. The females were loaded into the back of pickup trucks and were quickly transported to ORMC. Deputy Seal and I maintained a ready status behind the club awaiting any more victims for transport to the hospital. Deputy Seal and I were advised shortly thereafter that our pickup truck would not be needed for transport as all remaining victims inside the establishment were deceased. Deputy Seal and I maintained our position behind the nightclub as scene security until Deputy Seal advised we were cleared to leave. This concludes my involvement. 5 On 06/12/2016, at approximately 0208 hours I, K-9 Officer Bologna of the Oviedo Police Department, was notified by Seminole County Communications of an active shooter in Orange County. Communications advised Orange County Sheriff s Office and the Orlando Police Department were requesting mutual aid between all agencies in Central Florida. Communications then advised Orlando Police requested all K9 s available to be en route to the Pulse night club located at 1912 S. Orange Avenue, Orlando, FL. Sgt. Walton then gave me permission to respond to Pulse night club after the request. While en route, Communications advised the Orange County Sheriff and Orlando Police Department were still engaged in a gun battle with a suspect and they had multiple victims on the ground. Upon arrival I came into contact with an Apopka Police K9 officer and we ran down Orange Avenue towards Kaley Street. We then came into contact with a Sergeant from the Orlando Police Department. The Sergeant then requested we grab a back board and gloves from the Orlando Fire Department across the street from our location. I then grabbed gloves and other medical supplies from a fire truck while the Apopka K9 grabbed a backboard. We then ran to the triage area located at the Einstein s Bagels located at the corner of Orange Avenue and Kaley Street. While I was at the triage area I noticed multiple victims on the ground and other victims with apparent gunshot wounds to multiple parts of their body. I then started to apply direct pressure to some of the victim s wounds during this time. I was then requested by officers from Orlando Police Department to run across the street to Pulse night club to help with extracting other victims. While I was standing at the front door area of Pulse, I was advised the suspect was barricaded in a bathroom towards the rear of the night club. During this time I noticed multiple victims on the ground inside of the night club. I did not see any signs of life or movement coming from any of the bodies on the floor in the night club. I also noticed approximately officers and deputies inside the night club with long guns. Approximately two (2) minutes later I heard numerous officers inside the building they were in contact with subjects coming from the second floor of the night club. I then noticed two subjects exit the second floor and walk towards the entry way of Pulse night club. I then noticed both subjects were actually victims and had multiple gunshot wounds. I then grabbed one victim, conducted a search of his person, and transported him to the triage center behind the Einstein s Bagels. I then noticed his right arm had multiple gunshot wounds, so I applied direct pressure onto his arm until we arrived at the triage center. I then advised the EMS personnel regarding this victim where they made me sit him down on a curb. I then ran back towards Pulse night club to a cover/concealment area until they needed anymore extractions from the inside. During this time I noticed an unmarked Orlando Police pickup truck arrive on scene and requested officer to load up two victims who were on the ground next to the building. It was at this time I helped two Orlando Police Officers and a Seminole County Deputy load one of the two victims into the truck. During this time I did not see any life or movement coming from these victims while loading them into the back of the pickup truck. I then heard over my radio all K9 s on scene needed to stage at the 7/11 convenience store located at the corner of Kaley Street and Orange Avenue. I was then requested to meet with a Sergeant from the Orange County Sheriff s Office and check in. After approximately two hours stationary at this location, we were advised to move down Orange Avenue towards Arnold Palmer Hospital. After approximately 15 minutes of staging at Arnold Palmer Hospital, I heard numerous gunshots coming from the south of our location. We were then advised the subject was down but had some type of device strapped to his chest. Approximately one (1) hour after the gunshots I was advised to meet at a location off of Orange Avenue for a possible debrief. I then approached a Lieutenant from the Orlando Police Department who was at the mobile command center. I then gave my name, agency information, and phone number to the Lieutenant where he released me from the scene. This is my only involvement in this case. 6 On June 12, 2016 at approximately 0230 hours, I responded to assist the Orlando Police Department with an active shooter event at the Pulse Nightclub located at 1912 South Orange Avenue, Orlando, Florida, Orange County. On arrival, I made my way to the staging area and noticed a that there were no command personnel present to assign me to a position. So, I geared up and made my way to the east side entrance of the nightclub, where I incorporated myself into a team consisting of Orange County Deputies and Orlando Police Officers assisting with the evacuation of casualties as well as providing security to other elements conducting operation inside. As we were in position, an unknown Orlando Police Lieutenant, stated that they had found a van parked on the side of the club with ammo and a possible bomb inside and that they needed bomb dogs to search the areas. I called and woke up Deputy Welch and advised him of the situation. Welch stated that he would be en-route, and I relayed that information to the Lieutenant. Approximately 15 minutes later we were again told to push up to the east entrance and secure all civilians that exit the club. We moved and I placed myself at the door of the club so that I had an unobstructed view of the front dance floor and held security as some members prepared to receive the wounded. As the S.W.A.T Team pushed into the building and secured the front we maintained the east side perimeter. After the front was secured, we were directed to pull back to Orange Ave. as the parking lot had not been swept by E.O.D. for secondary devices. A short time later we were directed to return to the staging area. As we were making are way there, E.O.D. neutralized the van. I made my back to the staging area behind the Einsteins bagels and then to the 7-11 were I waited for the next event or assignment. No other assignments were made and I was relieved from the scene and approximately 0745 hours. On my way to the call, I was informed by dispatch to meet units outside of the Einstein Bagels just across the street from the nightclub. When I arrived on scene, I saw Orlando Police Officer s carrying victims from the club to the back of Einstein Bagels. I then began helping carry victims to the back of the business. Once there, I noticed that the location was a triage area with many gunshot wound victims lying on the ground waiting to be treated by medical personnel on scene. Many were already deceased. While I was there, a few more dozen people walked up to the area, all with gunshot wounds crying out for help. All of the medical personal on scene advised that they had no other way of transported any more victims to the hospital. I then started helping OPD Officers load victims in the back of an Officer s pickup truck with only the victims that were still living. While this was going on, the active shooter was still inside of the building, and was in a women s restroom in the back of the business with 3 people with him. I then ran over to the entrance of the club to help get more wounded out with other Officer s. While inside of the club, I saw many wounded and deceased victims lying all over the floor of the club. After helping more victims to the triage area, I made contact with Sgt. Risher there who instructed all Seminole County Deputies on scene, to meet him and standby in front of the Ragin Cajun Bike shop at 1809 S. Orange Avenue before doing anything else. I was instructed to get all of the information and phone numbers from all Seminole County Deputies on scene, and verified with CAD that all of our deputies were accounted for. After waiting about an hour on standby, I was assigned a post at Orlando Regional Medical Center. (ORMC) in front of the north loading dock after the hospital was put on lock down. I was there until I was told it was ok to leave at 0800 hours. 7 On 12 June, 2016, at approximately 0234 hours, I arrived at 1912 South Orange Avenue, Orange County, FL, in reference to an active shooter inside the Pulse night club. Upon my arrival I made contact with first responders at the causality collection point located behind the Einstein Brothers Bagels on the corner of East Kaley Street and South Orange Avenue. During this time I observed multiple people lying on the ground who were wounded. I was advised by a first responder that wounded victims were still being brought out of the night club. I responded to the eastside entrance to the night club as they were bringing out two more wounded victims. I observed a deceased person lying on the floor at the outside bar. I made entry into the night club from the eastside door to help provide security while other officers brought wounded victims out. I observed multiple victims lying on the ground who were deceased from the end of the bar closest to the eastside door. Orange County EOD Unit had arrived on scene and I made contact with EOD Team Leader Brooks to provide any assistance I could give. Brooks advised he could use my help and directed me over to their EOD vehicles where I helped make a breaching charge with the other Orange County EOD members. I stayed with the Orange County EOD Unit and helped with any tasks that presented itself until I relieved at 0800 hours. Upon arrival, I made contact with other Seminole County Deputies already at the scene, asked Orlando Police Department if there was anything I could assist them with and maintained ready status in the staging area pending instructions. I then responded to Winter Garden Hospital and confirmed there were no shooting victims transported to their location. Upon confirming that, I returned to the staging scene until dismissed. Orange Avenue, Orlando; to assist with an active shooter at the Pulse Nightclub across the street. The Einstein Bagel had been set up as a staging area for Law enforcement and a casualty collection point. Upon arrival, I made contact with units from the Orlando Police Department and the Orange County Sheriff s Office and obtained pertinent information as to shooter location, casualties, resources needed, and location of the command post. At the time I was advised there was no command post established. I assisted with directing the walking wounded to Fire Department units and coordinated with the responding SCSO units until the first command post was established. Once the initial command post was established I made contact with the OPD/OCSO command units and advised them of SCSO strength and presence. The staging area was later relocated to the parking lot of the Ragin Cajun Bike shop at 1809 S. Orange Avenue. I responded to that location and made contact with Sgt. Becker of the Orange County Sheriff s Office. I assisted Sgt. Becker with the listing and deployment of personnel resources and maintained the locations and assignment of the SCSO units on scene. SCSO units were deployed to several area hospitals to check for wounded and security posts at the Orlando Regional Medical Center. I continued to assist with monitoring SCSO units and deployment until clearing at approximately 0800 hours. 9 On Sunday, June 12, 2016 I responded to the area of 1912 S Orange Avenue, Orlando, FL to assist Orlando Police with an active shooter situation. Upon my arrival I first made contact with a Orlando Police Sargent behind the Einstein Bagels. This Sargent directed me to help with evacuating victims from Pulse Night Club. I then went across the street and entered the night club with several other Orange County and Orlando Police Officers. Once inside I assisted in picking up a wounded male, whom had been shot several times. While inside I saw several bodies laying around the ground with gun shot wounds. Once I assisted in removing this subject I made contact with Sargent Luge who advised me to continue helping where I was directed. I then went back to the front entrance of Pulse Night Club where I was asked to provide security at the front. I was told that there was potentially roof access, so I maintained a visual of that area. Shortly after this I was advised that Officer's had the front area locked down and asked that we start sweeping the parking lot for any explosive or secondary devices. At that point I began walking the parking lot but did not find anything suspicious. As I did this I made it around the the side entrance where I was asked to help remove several bodies from the entrance area. There I assisted in caring a male's body whom was obviously deceased. He was carried to a waiting police Pickup truck. I then took a security position behind a silver vehicle that was parked near the side entrance. While in this position I heard that the S.W.A.T Operators had a wounded subject coming towards the side door. Myself and another Orlando Police Officer went forward and again entered into the nightclub. There we stood near the stage area waiting to be directed to go assist the subject. However due to the threat being so close to this wounded subject we were asked to back out. Once the S.W.A.T operators went inside and assisted this subject out, myself and the Orlando Police Officer grabbed each of this subjects arms and dragged him behind cover where he was turned over to other Officers. I then again took a position behind the silver vehicle. A short time later we were directed to move back and to cover the side street. I stayed here for several minuets until the Orlando S.W.A.T armored vehicle's arrived along with several other S.W.A.T officers. Once relieved I again made contact with Sgt Luge and stood by with him and several other Seminole County Deputies at the A short time I was directed to go to a Hospital in Ocoee to check to see if any wounded had arrived. There I found no wounded and was asked to stick around for a little longer. Once I was asked to leave I returned to the area of Pulse Night Club where I again met up with Sgt Luge. While there I observed a subject that I recognized as someone who had ran from the Night Club. I escorted this subject to the witness collection point and returned back to our staging area. Soon after that I was told that I could secure for the day. 10 On Sunday morning, June 12, 2016, at 0212 hours, the Orlando Police Department requested the assistance of the Seminole County Sheriff s Office regarding an active shooter at the Pulse Nightclub establishment located at 1912 South Orange Avenue, Orlando, Orange County, Florida. Information provided from the communications center revealed several persons down of at least twenty-six victims with injuries sustain during gunfire. Watch Commander Kvalheim ordered me to provide several units to respond to the scene to provided assistance to the Orlando Police Department and the Orange County Sheriff s Office. I immediately requested a radio channel for Seminole Units. At 0248 hours, I was directed by Lieutenant Kvalheim to respond to the scene to assist Sgt. Luge. At 0300 I arrived on scene and shortly after arrival, I made contact with the Command Post at 03 hours, located at the intersection of Orange Avenue and Kaley Street. I made contact with two Orlando Police Department Lieutenants and one Orange County Sheriff s Office Lieutenant and we established abbreviated unity at 0315 hours. I provided OPD with a verbal list of Seminole County resources that were on scene and advised that we were prepared to accept marching orders. While unified, Lieutenants requested that I take a look from the outside and provide thoughts and suggestions regarding the scene. I provided the Commanders with suggestion based on Seminole County General Order #64; Response to Active Threats/Casualty Events. I asked Sgt. Luge to maintain a list of all Seminole County resources at all times including assignments. Additionally, I requested Sgt. Luge to continue working with the Seminole County Communications Center to establish communications (patching channels) for Seminole Units. The following is a list of events/times that I recorded in my notes: 0212, received request for assistance. 0213, requested a radio channel. 0248, I responded to the scene. 0300, I arrived on scene. 03, I arrived at the Command Post. 0315, Unified with OPD/OCSO. 0330, received information regarding possible shooter. 0330, 19E patched with OPD 0339, requested communications to units 0340, all SCSO units. 0407, Seminole County EOD on stand-by 0513, evacuating victims. 0531, suspect down. 0556, all victims extracted 0736, requested patch terminated. 0740, patch terminated. 0745, all SCSO units accounted for 8314 still 97 at ORMC for security. The following is a list of Seminole County Units and their actions while on scene: Deputy S. Meadows remained at staging waiting for assignment. Deputy T. Madewell was on perimeter and helped extract victims from Pulse then provided security at ORMC. Deputy Z. Seal provided perimeter security at casualty collection point (CCP), responded with truck to transport victims, loaded victims, and maintained responsibility of trainee Shields. (note- did not transport victims in SCSO vehicle) Deputy K. Gonzalez conducted traffic control at Orange Ave. and Kaley Street, responded to ORMC regarding shots fired, swept the load dock, responded to Winter Garden Hospital to check for victims. Deputy D. Shields (trainee with Deputy Zeal) Deputy T. Smith maintained security at the East entrance of Pulse. Deputy B. Rush maintained security at CCP, help load victims into trucks, entered Pulse and extracted victims to the CCP, logged contact information of all SCSO units, responded to ORMC and maintained security. Deputy S. Sullivan provided security within the East entrance of Pulse, swept the area for explosives, assumed assignment with EOD making breaching charge. Deputy L. Lopez responded to Winter Garden Hospital to check for victims, returned to staging. Sgt. Luge was the staging supervisor/liaison with Sgt. Risher Deputy P. Rehder extracted victims from Pulse, security at entrance, responded to Health Central to check for victims, provided security at the hospital I remained in constant contact with operational commanders and incident command at the command post until released by Major Tomeo at 0834 hours. Upon my arrival, at approximately 0400 hours, I met with members of K-9 units from multiple agencies to maintain a ready status in the event other EOD K-9's were needed to conduct additional EOD sweeps.
