2 Terms & Conditions 1. The abovementioned event ( Event ) is organized by Sport Singapore ( SportSG ) and Basketball Association of Singapore ( BAS ) in accordance with the Terms and Conditions applicable to the players. 2. SportSG and BAS shall collect and use the personal information that players have provided so as to serve the players in a most efficient and effective manner, including usage for future marketing, planning, data processing, statistical or risk analysis and research purposes within SportSG, BAS and its agent(s), unless such sharing is prohibited by law. Personal Information refers to all and any information relating to the players obtained by SportSG and BAS in the course of and as a result of the provision of services to the players. Such personal information includes all the information listed in this Form. SportSG and BAS shall treat players personal information as confidential. Players understand that you can withdraw your consent at any time via the indicated , SportSG and BAS may not be able to continue providing its services to you as a result. 3. Players declare that all the information provided in this Form are true, accurate and complete. 4. Registration is based on first-come-first-served basis and will be closed once maximum number is reached. 5. Registration is only complete upon receiving the Registration Form, Under18 Parent s Consent Form. Payment to be made after receiving of invoice and is non-refundable. 6. Only names that are listed in the Registration Form shall be eligible to participate in the Event. Changes to the line-up will not be accepted, once it has been submitted to the presiding competition officials on the Players/Team Captains Briefing Day. Any changes / amendments / decisions would be made at the sole discretion of the Organizing Committee. 7. Any protest must be made on an Official Protest Form on the actual event day. A nonrefundable protest fee of S$100 must be paid along with the Protest Form by the players. 8. Upon Closing, if a minimum no. of participating players / teams is not achieved in the respective categories, the sole discretion of the Organizing Committee to scrap the category will be undertaken. 9. Whilst reasonable precaution will be taken by SportSG, BAS and its agent(s) to ensure players safety, players are to understand that you are taking part in the Event at your own risk. SportSG, BAS and its agent(s) will not be held liable for any injury or death arising from participating in the Event or for any loss of or damage to your property occurring in the course of the Event, except for such injury or death that is caused directly by SportSG, BAS or its agent(s) breach of legal duty of care. 10. In consideration of SportSG and BAS allowing the participants to take part in the Event, the participants undertake that if, in the course of the Event, the participants deliberately or negligently cause any injury (whether fatal or otherwise) to any person or any damage to or loss of any property of any person, the participants shall indemnify SportSG and BAS in instance where the suffering party makes claims or takes actions against SportSG and BAS or its agent(s) for costs or expenses. 11. The participants understand that photographs and/or videos of you may be taken during the Event, for use by SportSG and BAS on its websites and in other materials for event publicity, internal reporting and other related purposes.
