Declaration Information: Incident Period: February 14- March 4, 2018 Date Issued Declaration Remarks 04/02/2018 FEMA-4363-DR-IN The State of Indiana Governor requested an extension for a Major Declaration beyond the originally specified 90 days ending on April 11, /11/2018 FEMA-4363-DR-IN The State s request for time extension was approved and extended to May 3, /27/2018 FEMA-4363-DR-IN Governor requested a Major Declaration for the counties of Benton, Clark, Crawford, Dearborn, Elkhart, Floyd, Fulton, Gibson, Harrison, Jasper, Jefferson, LaPorte, Marshall, Newton, Ohio, Perry, Porter, Spencer, St. Joseph, Starke, Switzerland, Vanderburgh, Vermillion, Wabash, Warren, Warrick, and White for Public Assistance (Categories A-G). Individual Assistance to individuals and households has been approved for Carroll, Clark, Elkhart, Floyd, Harrison, Jefferson, Lake, Marshall, and St. Joseph. Hazard Mitigation Grant Program statewide. 05/04/2018 FEMA-4363-DR-IN The President approved a presidential disaster declaration on May 4/2018 for the counties of Benton, Clark, Crawford, Dearborn, Elkhart, Floyd, Fulton, Gibson, Harrison, Jasper, Jefferson, LaPorte, Marshall, Newton, Ohio, Perry, Porter, Spencer, St. Joseph, Starke, Switzerland, Vanderburgh, Vermillion, Wabash, Warren, Warrick, and White for Public Assistance (Categories A-G). Individual Assistance to individuals and households has been approved for Carroll, Clark, Elkhart, Floyd, Harrison, Jefferson, Lake, Marshall, and St. Joseph. All areas of the State are eligible for the Hazard Mitigation Grant Program. 05/04/2018 FEMA-4363-DR-IN David G. Samaniego appointed as the Federal Coordinating Officer (FCO) for this disaster. 05/04/2018 FEMA-4363-DR-IN Closes the incident period for FEMA-4363-DR-IN as of 05/04/ Current Situation: An Initial Operating Facility is establish in the State Emergency Operations Center to support the State of Indiana for FEMA-4363-DR- IN. The Region V IMAT arrived to support on 05/05/2018. The first DRC had a soft opening in Elkhart County 05/10/2018. DSA Crews are dispersed to all IA designated counties to be deployed at the request of the county emergency manager for the completion of registration and canvassing. Total DSA staff Fatalities/Casualties: There were six (6) confirmed fatalities per the State of Indiana. 9a. Fatalities: See box 9 9b. Hospitalizations: The State reported three (3) Hospitalizations. 9c: Injuries (Major and Minor): See box 9b 10. Extent of Damage: Severe flooding impacts to homes and infrastructure. 10a. Specific Metrics (Area flooded, flood depth, total acres burned, number of aftershocks, etc.): Nothing Substantial To Report (NSTR). 10b. Structures Damaged: State reported damage to 1,865 homes. 10c. Structures Destroyed: State reported 11 homes were destroyed. 10d. Power Outages: All power has been restored 11.
