CONSENT AGENDA A. Approval Of Minutes Of The Village Board Meeting, April 20, 2015 Attachment 1: Unapproved Minutes B. Special Event Request For The Chapel Family Picnic The Chapel in Lake Zurich has requested to use Paulus Park s Shelter B on Saturday, August 1 from 8:00 am - 5:00 pm to host a picnic. The anticipated attendance for this event is between people. Attachment 1: Staff Memo C. Special Event Request For The LOVEolution Project Yoga Relay The LOVEolution Project, a non-profit organization that supports families in the aftermath of a crisis, has requested to use Paulus Park on Saturday, August 29 to Sunday, August 30 for a 24 hour yoga fundraising event. The anticipated attendance for this event is between people. Attachment 1: Staff Memo D. Ordinance Amending Bradford Town Crossing Planned Unit Development And Approving An Amended Site Plan (NW Corner Of Quentin And Route 22) (Ord. # ) In 2013 the Village Board approved Ordinance amending the Bradford Town Crossing Planned Unit Development to add an outlot for the McDonald s restaurant and the relocation of the Marino s building. The amended PUD caused certain public utilities and easements to be relocated. The plat of easement vacation and easement grant has been reviewed by staff and is recommended for approval. Attachment 1: Staff Memo & Ordinance E. Ordinance Authorizing Disposal Of Property Owned By The Village Of Lake Zurich (Assign Ord. # )
4 Village Board of Trustees Regular Meeting Staff has determined the items listed in this proposed Ordinance are no longer necessary, useful, or in the best interests of the Village to retain ownership. The proposed Ordinance declares the property as surplus and authorizes the sale or disposal of the items. Attachment 1: Staff Memo & Ordinance F. Letter Of Credit Reduction To $73, For The Shops Of Lake Zurich Corners At 440 S. Rand Road 440 Rand Road, LLC has requested a reduction of the letter of credit for public improvements at the Shops of Lake Zurich Corners. Staff recommends the letter of credit be reduced from $431, to a remaining escrow of $73, Attachment 1: Staff Memo G. Intergovernmental Agreement Between Ela Township And The Village Of Lake Zurich In partnership with Ela Township, a sidewalk installation project is proposed along Chancery Lane and Town Mall Drive to connect the Ela Township Community Center with the senior assisted living facility on Mohawk Drive. Attachment 1: Staff Memo & IGA H. Resolution For Illinois Department Of Transportation Motor Fuel Tax Expenditures In The Amount Of $615,000 This resolution confirms the expenditure of annual Motor Fuel Tax allotment for maintenance services that includes traffic signal maintenance, salt and deicing supplies, electricity, sign replacement, tree trimming, pavement maintenance and marking, and right-of-way mowing. Attachment 1: Staff Memo & Resolution 8. OLD BUSINESS A. None At This Time. 9. NEW BUSINESS A. Ordinance Authorizing A Transition To A Calendar Fiscal Year For The Village Of Lake Zurich And Amending The Municipal Code In Regard To The Village s Fiscal Year (Assign Ord. # ) (Trustee Beaudoin) Summary: The Village's 12-month fiscal year has traditionally operated from May 1 to April 30. As discussed at the April 11, 2015 Committee of the Whole budget workshop, changing the fiscal year to coincide with the calendar year will allow the
5 Village Board of Trustees Regular Meeting Village to better plan its financial operations and realize greater efficiencies in infrastructure investment. If approved, the current 2015/2016 fiscal year will continue until April 30, At that point, the eight month transition fiscal year will run from May 1, 2016 until December 31, Then beginning on January 1, 2017, the Village will begin a calendar fiscal year. Despite the difficulties that are inherent with changing fiscal years, staff is confident that the transition-related challenges over the next two years will be well worth the strategic long-term benefits of implementing a calendar fiscal year. Recommended Action: A motion to approve Ord. # authorizing a transition to a calendar fiscal year for the Village of Lake Zurich and amending the municipal code in regard to the Village s fiscal year. Attachment 1: Staff Memo & Ordinance B. Ordinace Approving Site Plan And Exterior Appearance And Granting Variations For The Hudson Townhomes On Terrace Lane (Assign Ord. # ) (Trustee Sprawka) Summary: JMLJ Construction has filed a zoning application seeking site plan approval, exterior appearance approval, and zoning variance approval for reductions in the rear yard setback, side yard setback, and minimum landscaped surface area for a proposed three-unit townhome development along Terrace Lane. The Plan Commission met on April 15, 2015 and voted 7-0 for a recommendation of approval of the site plan and exterior appearance, pending certain conditions which are summarized in the proposed Ordinance. The Zoning Board of Appeals met on April 16, 2015 and voted 7-0 for a recommendation of approval of the requested variances. Recommended Action: A motion to approve Ord. # approving site plan and exterior appearance and granting variations for the Hudson Townhomes on Terrace Lane Attachment 1: Staff Memo & Ordinance C. Purchase Of A 2016 Ford F-550 / Horton 623 Ambulance From Foster Coach, Inc. In The Amount Of $210,810 (Trustee Shaw) Summary: The Fire Rescue Department operates four front line ambulances within the Village and the Fire Protection District. The budget for 2015/2016 includes $240,000 for the replacement of a 15 year old ambulance with 105,000 miles and 8,980 engine hours (equal to 295,350 miles). The Village will be utilizing the Suburban Purchasing Cooperative for this purchase, an intergovernmental joint purchasing program that represents 140 municipalities. Once ordered, expected delivery time for the 2016 ambulance is 10 to 12 months. Recommended Action: A motion to approve the purchase of a 2016 Ford F-550 / Horton 623 Ambulance from Foster Coach, Inc. under the Suburban Purchasing
