1 Becoming a Reserve Firefighter with the City of Waynesboro. In order to become a Reserve Firefighter you MUST follow the steps below. 1. Review the Reserve Firefighter Requirements and Policies (page 3) and agree to abide by the conditions set forth within. 2. Complete and return the Reserve Firefighter Application (page 5-6) along with copies of each of the following; (WFD can make copies) a. Current Georgia driver s license b. Birth certificate c. Social security card d. Proof of insurance on your vehicle e. High school diploma or general education development equivalency (GED). Must be accredited by an approved accrediting agency. f. Any firefighting related certificates or CPR cards. APPLICATIONS AND ALL PAPERWORK ABOVE MUST BE SUBMITTED TOGETHER APPLICATIONS WITH INCOMPLETE OR MISSING PAPERWORK WILL NOT BE ACCEPTED OR CONSIDERED. ONCE ALL PAPERWORK IS REVIEWED, YOU WILL BE CONTACTED FOR AN INTERVIEW 1. Complete required paperwork with city human resources department and successfully pass a drug screen, medical exam, and Hepatitis B vaccination check. 2. Be fingerprinted and a search made of local, state, and national fingerprint files to determine any criminal history. 3. 2 NON-CRIMINAL JUSTICE APPLICANT S PRIVACY RIGHTS As an applicant that is the subject of a Georgia only or a Georgia and Federal Bureau of Investigation (FBI) national fingerprint/biometric-based criminal history record check for a non-criminal justice purpose (such as an application for a job or license, immigration or naturalization, security clearance, or adoption), you have certain rights which are discussed below. You must be provided written notification that your fingerprints/biometrics will be used to check the criminal history records maintained by the Georgia Crime Information Center (GCIC) and the FBI, when a federal record check is so authorized. If your fingerprints/biometrics are used to conduct a FBI national criminal history check, you are provided a copy of the Privacy Act Statement that would normally appear on the FBI fingerprint card. If you have a criminal history record, the agency making a determination of your suitability for the job, license, or other benefit must provide you the opportunity to complete or challenge the accuracy of the information in the record. The agency must advise you of the procedures for changing, correcting, or updating your criminal history record as set forth in Title 28, Code of Federal Regulations (CFR), Section If you have a Georgia or FBI criminal history record, you should be afforded a reasonable amount of time to correct or complete the record (or decline to do so) before the agency denies you the job, license or other benefit based on information in the criminal history record. In the event an adverse employment or licensing decision is made, you must be informed of all information pertinent to that decision to include the contents of the record and the effect the record had upon the decision. Failure to provide all such information to the person subject to the adverse decision shall be a misdemeanor [O.C.G.A (b) and (b)]. You have the right to expect the agency receiving the results of the criminal history record check will use it only for authorized purposes and will not retain or disseminate it in violation of state and/or federal statute, regulation or executive order, or rule, procedure or standard established by the National Crime Prevention and Privacy Compact Council. If the employment/licensing agency policy permits, the agency may provide you with a copy of your Georgia or FBI criminal history record for review and possible challenge. If agency policy does not permit it to provide you a copy of the record, information regarding how to obtain a copy of your Georgia, FBI or other state criminal history may be obtained at the GBI website ( If you decide to challenge the accuracy or completeness of your Georgia or FBI criminal history record, you should send your challenge to the agency that contributed the questioned information. Alternatively, you may send your challenge directly to GCIC provided the disputed arrest occurred in Georgia. Instructions to dispute the accuracy of your criminal history can be obtained at the GBI website ( 2
3 PRIVACY ACT STATEMENT Authority: The FBI s acquisition, preservation, and exchange of fingerprints and associated information is generally authorized under 28 U.S.C Depending on the nature of your application, supplemental authorities include Federal statutes, State statutes pursuant to Pub. L , Presidential Executive Orders, and federal regulations. Providing your fingerprints and associated information is voluntary; however, failure to do so may affect completion or approval of your application. Principal Purpose: Certain determinations, such as employment, licensing, and security clearances, may be predicated on fingerprint-based background checks. Your fingerprints and associated information/biometrics may be provided to the employing, investigating, or otherwise responsible agency, and/or the FBI for the purpose of comparing your fingerprints to other fingerprints in the FBI s Next Generation Identification (NGI) system or its successor systems (including civil, criminal, and latent fingerprint repositories) or other available records of the employing, investigating, or otherwise responsible agency. The FBI may retain your fingerprints and associated information/biometrics in NGI after the completion of this application and, while retained, your fingerprints may continue to be compared against other fingerprints submitted to or retained by NGI. Routine Uses: During the processing of this application and for as long thereafter as your fingerprints and associated information/biometrics are retained in NGI, your information may be disclosed pursuant to your consent, and may be disclosed without your consent as permitted by the Privacy Act of 1974 and all applicable Routine Uses as may be published at any time in the Federal Register, including the Routine Uses for the NGI system and the FBI s Blanket Routine Uses. Routine uses include, but are not limited to, disclosures to: employing, governmental or authorized non-governmental agencies responsible for employment, contracting, licensing, security clearances, and other suitability determinations; local, state, tribal, or federal law enforcement agencies; criminal justice agencies; and agencies responsible for national security or public safety. 3

State of Georgia Support Firefighter Basic Training Program 2. IS-700 National Incident Management System 3. IS-100 Introduction to the Incident Command System 4. American Heart Association Heartsaver First Aid 5. American Heart Association Blood Borne Pathogens 6. American Heart Association BLS Healthcare Provider CPR/AED 7. Hazardous Materials Awareness Registered Firefighter (completed within six months of Support Firefighter completion) Firefighters with increased fire ground function who can perform in hazardous zones including interior fire suppression, climbing an extended aerial ladder and using SCBA under direct supervision of State Certified Firefighters. Training Requirements 1. Module 1 of the Georgia Basic Firefighter Training Course 2. Georgia Fire Academy Live Fire Course State of Georgia Certified Firefighter (completed within six months of Registered Volunteer Firefighter completion) Firefighters who can perform any function on the fire ground without direct supervision and can supervise Support and Registered Firefighters. 5 Reserve Firefighter Requirements and Policies Requirements 1. Be at least 18 years of age 2. Not have been convicted of a felony in any jurisdiction within ten years prior to employment/appointment. 3. Be fingerprinted and a search made of local, state, and national fingerprint files. 4. Have a good moral character as determined by an investigation. 5. Possess a high school diploma or a general education development equivalency. Must be accredited by an approved accrediting agency. 6. Be in good physical condition as determined by a medical examination and successfully pass the GFSTC minimum physical agility test. (see attached) 7. Valid Georgia Driver s License and dependable insured transportation. 8. No facial hair allowed except mustaches which shall not extend more than ½ past the corner of the mouth. 9. Negative drug screen. 10. Meet a minimum standard of 50% of the aggregate of creditable department activity and creditable calls for the calendar year using a point based system. One point shall be earned for each eligible call, and one point for each completed hour of department activity. Creditable department activity is defined as regular training classes to which the member shall attend a minimum of 25% which shall also be no less than 24 hours for the year. Members shall meet a minimum of 25% of creditable calls annually (structural fire calls). 11. Successfully achieve appropriate training levels in the time allotted. Training Training classes will be held on Tuesdays from 6-8 PM and Friday afternoon from 2-4 PM. You may attend either class or both. In order to receive credit, reserves must bring turnout gear and be clean shaven. Structural Fire Calls A structural fire call is defined as any call that is dispatched alluding to the possibility of a fire in a structure. This includes fire alarms, smoke smells, and stove fires. It does not include vehicle fires, grass fires, or carbon monoxide calls. If in doubt, respond. In order to receive credit for attending the call reserves must arrive at the scene with turnout gear and remain until released. If the units have returned to the station prior to the reserve s arrival at the scene, reserves are to report to the station. Reserves must report to the station within approximately 15 minutes of the unit s return to the station for credit. This will apply to calls that are approximate 15 minutes or less in duration. Failure to respond due to pager malfunction does not relieve the reserve from the fire call requirements. Pay There is a $25 benefit for each structural fire call attended. If the call is over 1 hour in duration then an hourly rate of $10 will be applied to the time over 1 hour. Training class attendance will be paid at a rate of $10 per class attended. In order to receive credit and pay, reserves must sign the training record and field report. Pay checks are issued on the Friday after the end of the 28 day pay cycle. Lack of Attendance Failure to show an effort to fulfill the attendance and training requirements will result in termination from the department. The Georgia Certified Firefighters Physical Agility Test INSTRUCTIONS Test Preparations 5
6 1. The candidate shall be provided with gloves, helmet, and a self-contained breathing apparatus (without the mask) to be worn during the entire test. The candidate shall be assisted with donning this equipment so that it is worn properly. 2. The following equipment will be needed: a rolled 50-foot section of 1½ or 1¾ inch diameter hose and a multi-story structure, a 24-foot aluminum extension ladder in a securely supported vertical position, a fire department axe and a target such as a wooden pallet, a charged (75 PSI nozzle pressure) 100 foot 1½ or 1¾ inch hoseline with a nozzle attached and secured in the off position, a 165-pound dummy with a strap or harness attached, and a 14-foot roof ladder placed in a horizontal position at a height of 5 feet. 3. Each exercise shall be properly prepared and ready for the candidate prior to beginning the test. 4. The overall distance from the starting point of the first exercise and the last exercise shall not exceed 400 feet and not be less than 300 feet. 5. The starting point (and ending point if applicable) for each exercise shall be pointed out to the candidate prior to beginning the test. 6. The candidate shall be given the overall instructions and the instructions for each exercise prior to beginning the test. (Instructions for all or any single exercise may be read again but time does not stop.) Overall Candidate Instructions 1. The candidate shall wear gloves, helmet, and a self-contained breathing apparatus (without the mask) during the entire test. 2. The candidate may not run during the test. 3. Time will begin at the starting of the first exercise and shall stop when the candidate completes the last exercise. (The candidate may pause during the test but time will continue to expire.) 4. The candidate may ask that instructions for all or any single exercise be read again but time will continue to expire if the test has already begun. 5. The candidate must complete the entire test in seven minutes or less. Exercise Instructions 1. Stair Climb - The candidate, given a rolled 50-foot section of 1½ or 1¾ inch diameter hose and a multi-story structure, shall carry the hose section up one flight of stairs to the second floor and then return to the starting point with the hose. The candidate must use each step while climbing or descending the stairway. 2. Ladder Extension - The candidate, given a 24-foot aluminum extension ladder in a securely supported vertical position, must completely extend the fly section (top section) of the ladder. The candidate must then lower the fly section in a controlled fashion to the starting position. 3. Ventilation Exercise - Given a fire department axe and standing on level ground with a target (such as a wooden pallet) placed on the ground in front of them, the candidate must strike the target with the axe 20 times. The axe must be brought completely over the shoulder to simulate a chopping motion as if cutting a ventilation hole. 4. Hose Advance The candidate, given a charged (75 PSI nozzle pressure) 100-foot 1½ or 1¾ inch hoseline, shall pick up the nozzle and advance the pressurized hoseline for a distance of 50 feet. After reaching the destination, the candidate shall lay the hose on the ground. 5. Rescue Drag The candidate, given a 165-pound dummy on a level surface, shall drag the dummy a distance of 50 feet. 6. Ladder Removal/Replacement The candidate, given a 14-foot roof ladder placed in a horizontal position at a height of 5 feet and with the ladder rungs in a vertical position, shall lift the entire ladder from its support and place it on the ground then pick the entire ladder up and return it to its original position.
