1 2018 Ohio Troopers Coalition Memorial Scholarship Application Ohio Troopers Coalition Memorial Scholarships will be awarded to graduating High School Seniors who have successfully completed the application requirements and whose parent(s) are member(s) that meet the eligibility requirements. The parent must meet the eligibility requirements for the duration of the scholarship. Eligibility Requirements: The member MUST ATTEND a minimum of three (3) Lodge Meetings a year or qualifying Lodge Event (e.g. Shop with a Cop/Buying with Bears, Special Olympics, Dayton Air Show, etc.). Any member who has died in the line of duty is considered to be eligible under this requirement. Scholarships will be awarded on the basis of scholastic merit, standardized test scores, participation in extracurricular activities, and involvement in school government, community service work, and goals in life. Total amount of the scholarship will be up to $4,000 payable over a four (4) year period ($1,000 per year) to each person. The recipient of the scholarship will be required to provide proof of college enrollment. He/She must pledge that it s their intention to complete college, and to maintain at least a "C' average. He/She also commits to sending copies of their grades to the Ohio Troopers Coalition Scholarship Committee along with a resume of their achievements during each year. Grades not received will result in the suspension of the award until they are obtained. Students that are awarded an Ohio Troopers Coalition scholarship shall be required to complete a Scholarship Continuance Form each year. The form will be reviewed by the OTC Scholarship Committee and answers may impact the continued receipt of an OTC scholarship. If due to unforeseen circumstances, the recipient elects to take a leave from college for up to one year only, the payment will be deferred to the start of the next year of school. No more than one leave will be granted. After the second leave, the award will be forfeited. This decision is subject to review and can be appealed at the Combined Board Meeting. Application Requirements 1. The original application (see attached) should be submitted with one (1) current, original photo and four copies of the application and photo. Each copy must have a transcript of grades with current cumulative GPA, a copy of the SAT or ACT scores, and two (2) letters of recommendation from a High School faculty member on School District Letterhead. A character reference written by someone other than a member of the applicant's High School faculty or family must also be enclosed with the application. 2. The applicant s parent(s) must submit a Parent Involvement Form (see attached) detailing their recent involvement and how they have met the eligibility requirements for the previous year. The appropriate lodge representative must certify in writing, to the scholarship committee, verification that the parent has met the eligibility requirements. This form is an important part of the application process. This must be done each of the four years that the applicant is receiving an OTC scholarship. If the recipient and/or parent does not fulfill the scholarship requirements, the Scholarship committee will recommend to the Ohio Troopers Coalition Combined Board that the scholarship be terminated. A Committee composed of members appointed by the Chairman of the Ohio Troopers Coalition will meet prior to the Annual Conference for final selection. These regulations are subject to revision by the Ohio Troopers Coalition Combined Board at a regular or special meeting.
