2 White Plains High School Local Community Scholarships Program Directions for Completing Your Application(s) Please read all of the attached information carefully, and follow all directions when applying for these scholarships. You can apply to no more than three of the scholarships listed in this booklet. Before applying for a specific scholarship, check the selection criteria and application requirements listed in the attached grid. o o Be sure that you fit the criteria for each scholarship for which you are applying Be sure to include all supporting documents required for each scholarship You must submit ONE copy of the first two pages of your parent s 1040 Income Tax Return for 2014 or 2015 in a sealed envelope. Please black out social security numbers. All other documents must be provided for each scholarship for which you are applying. If you are applying for 3 scholarships, you will need 3 copies of your resume, etc. If any of the scholarships you are applying for requires an interview at WPHS, you must attend an orientation meeting during lunch on Tuesday, April 14, 2015 to receive your interview time(s). If any of the scholarships you are applying for requires an interview, you must be available the evenings of April 20 and 21 for your interview(s), which will be sometime between 6 p.m. and 10 p.m. INTERVIEWS CANNOT BE RESCHEDULED. Use blue or black ink when completing any forms that must be hand-written. You should use the attached checklist to be sure that you have successfully completed your application(s) prior to submitting them and include all required materials. You must meet the application deadline in order to be considered for these scholarships. All local scholarship applications must be turned in to the College and Career Center, Room C141, by no later than Friday, March 6, 2015

7 WHITE PLAINS HIGH SCHOOL LOCAL COMMUNITY SCHOLARSHIPS PROGRAM Budget Sheet 2014/15 Student last name Student first name Instructions To estimate the amount of scholarship that you will need for the first year of your post-high school education or training, please do the following. 1. For each of your top three college choices*, Google the name of the school + net price calculator. For example: If SUNY Purchase was one of my choices, I would Google: SUNY Purchase net price calculator *If you have already been admitted to a school that you have committed to attend in the fall, you only need to provide financial data for that one school. 2. Fill in all the information requested in the form. You will need your parent/guardian s assistance in filling out the required information, as the calculator will ask you to enter information regarding your family income. 3. Once you have filled out the online form, a summary of expenses will be generated. Please print that result and attach it to this form. The result will be different for each school, so you must go onto the net price calculators for each of your top three schools and attach the results from each of those schools. 4. 8 White Plains High School Local Community Scholarships Program Important Dates Friday, March 6, Completed scholarship applications due Tuesday, April Mandatory lunch meeting for students who have applied for scholarships that require interviews Monday and Tuesday, April 20-21, 6-10 p.m. On or before Thursday, May Scholarship interviews students selected to receive scholarships will receive an invitation to Senior Awards Night Tuesday, June Senior Awards Night Application Checklist Be sure that when you turn you in your application for local scholarships you include: Registration Form (one copy) Tax return documents (one copy) For EACH scholarship for which you apply, you must staple together and include: Individual Scholarship Cover Sheet (one per scholarship) Personal/Financial Statement (one per scholarship) Budget Sheet and Net Price Calculator Summary Sheets (one per scholarship) Resume (one per scholarship) Essay (one individualized! per scholarship) Please place all application materials in a 9 by 12 inch envelope with your name and Local Scholarship Application printed on the outside of the envelope. Applications that are not complete or are not submitted by the March 6 th deadline cannot be considered. Please return all application materials to the College and Career Center, room C141.
