1 2014 MHS LOCAL SCHOLARSHIP APPLICATION Completed Applications due by 3:30pm March 4, 2015 To Mrs. Bunnell, MHS Main Bldg room 135 Only seniors enrolled in the Coos Bay School District who receive a diploma within the 2015 school year may apply. Applications must be typed. Some scholarships have specific requirements, be sure to complete all steps to be considered for these. Remember neatness counts! Your completed application will be copied and distributed to various selection committees who select the scholarship recipients. There are several scholarships with a wide range of criteria. The committee, a group of dedicated community members, will decide the scholarship(s) for which you best qualify. The list of scholarships can be found on the MHS College/Career Website. Scholarships will be awarded in May at the Scholarship Assembly and the Senior Awards Dessert. If you have any questions or need help please see Mrs. Bunnell, your counselor, or your ASPIRE/Talent Search/Upward Bound mentor. APPLICATION CHECKLIST 1. Download the application from the MHS College Information Webpage, change view to Edit, rename document and save. 2. Parent/Guardian and Student MUST sign application. (even if student is 18) 3. PROOF READ YOUR APPLICATION CAREFULLY! 4. Application should be put together in the following order, each should be limited to 1 page: a) Personal and Academic information b) Family and Financial Information c) Copy of FAFSA that includes your EFC (estimated family contribution) d) **Optional** Special Financial Consideration e) Activities Chart (will accept OSAC, CIS, or Activities Chart that is linked on webpage) f) Personal Statements 2 per page. 150 word max, include the question and number g) **Optional** Jason Forrester or Vocational/Technical Scholarships info page h) **Optional**Recommendation for Forrester or Voc/Tech Scholarships i) Staple the entire package (do not include this cover sheet) left hand corner j) Turn into MHS room 135 by the due date. Incomplete applications will not be considered.

7 THE FOLLOWING MHS LOCAL SCHOLARSHIPS HAVE ADDITIONAL REQUIREMENTS THAT MUST BE MET IN ORDER TO BE CONSIDERED FOR THESE SCHOLARSHIPS Include this page and provide the additional information requested only if you are applying for these scholarships. o Marshfield Vocational/Technical Scholarship. These scholarships are awarded to students planning to advance his/her education and training in skills needed for a career in a vocational or technical trade that requires a certificate or an associate s degree. For example: apprenticeships, auto technician, truck driving school, EMT, welding, cosmetology, medical assistant, culinary arts, etc. Students pursuing a bachelor s degree do not qualify for these scholarships. Selection criteria include: Motivation Leadership Involvement in school/community activities and service Financial need is a consideration The committee will pay particular attention to the applicant s goals and plans for achieving them in the personal statement, question #1. Please be specific as possible. One reference from a teacher or an adult mentor is required. The recommendation forms are available online. Applicant must complete an interview with members of the voc/tech scholarship committee, which will be scheduled in the spring by the committee.
