1 SCHOLARSHIP OFFERINGS PROVIDED THROUGH NOTRE DAME HIGH SCHOOL DEVELOPMENT OFFICE Internships/Shadow Programs with Scholarships Buessing Business Scholarship/Internship Juniors involved with FBLA are eligible to apply (application provided by FBLA moderator) Minimum GPA of 2.5 Summer internship required at specific business/entity (paid) Enrolled in applicable business courses at NDHS Knowledge in Microsoft Window Suite Each student is required to develop and present a business plan For award: Two (s) scholarships in the amount of $1,500 each along with 2 paid summer internships DEADLINE: March 15, 2018 SEE DR. GENTILE FOR APPLICATION INFORMATION. St. Luke s Shadowing Program with Scholarships Rising seniors eligible to apply Personal Data Form to be completed Student should have an interest to pursue a career in healthcare, administration or business fields Student must complete a summer shadowing program (unpaid) of 50 hours Internship hours are arranged through hospital contact and scholarship is not applied to tuition account until hours are completed and evaluation is returned Applicant must submit two recommendations from NDHS faculty or staff Applicant must submit a typed, one page essay on the following subject: How quality healthcare affects all aspects of the community it serves. For award: Four (4) scholarships of $1,000 each Deadline: March 15, 2018

2 Memorial Scholarships Christopher Meddaugh Memorial Scholarship Underclassman Award One page handwritten essay on the following subject: Describe how your community activities have made you a better person. For award: Three (3) awards in the amount of $1,000 each John Reichl Memorial Scholarship Students entering grades 9, 10, 11 and 12 are eligible List of co-curricular activities, employment (if applicable), leadership roles, honors/awards/recognitions Consideration will be given to character of the student, student role models, variety of activities, school pride and teamwork FACTS application necessary as financial need will be considered One page, typed essay on the following theme: My time at ND will help guide my life, both personally and professionally because.. For award: One (1) award of $1,500 O Brien Family Memorial Scholarship Minimum GPA of 2.50 List of all community activities involved with as well as any employment One page essay on the following subject on one of the following two subjects: o Choice 1: Living in the moment is important because. o Choice 2: Describe why the arts are important to students today. For award: Two (2) $250 scholarships will be awarded and applied to current year tuition account. One of the two scholarships will be awarded to a student within the Acquinas program at Notre Dame. The remaining award will be open to any student meeting the criteria and has an interest in the area of art and/or communications.

For award: One (1) $500 scholarship will be awarded and applied to current year tuition account DEADLINE: May 1, 2018 Scholarship to be awarded at the 2018 Veteran s Day celebration at Notre Dame High School Jeff Dailey Memorial Scholarship Students entering grades 9, 10, 11 and 12 are eligible Student must have a GPA of 3.0 or higher 1-2 page essay that demonstrates how you embody or emulate the spirit of Jeff Dailey with the theme: I am my brother s keeper. For award: Eight (8) $1,000 awards to students entering grades 9-12 DEADLINE: May 1, 2018 (award presented in December 2018) Scholarships to be awarded at the 2018 Jeff Dailey Memorial Tip-off Basketball Tournament. Endowment & Other Scholarships Bishop Thompson Writer s Scholarship Incoming freshman only are eligible for this award An essay of no more than 400 words explaining why you chose Notre Dame for your high school education For award: Four (4) $250 scholarships

4 Tuskes Family Scholarship Fund Incoming Freshman from St. Jane de Chantal School Award Students entering grades 9 from St. Jane s School Satisfactory academic and discipline record while at St. Jane s as evidenced by a letter from the Principal Proof of registration at Notre Dame High School Student must be approved by the NDHS Financial Aid committee to receive financial aid based on FACTS application and NDHS criteria For award: One (1) $500 scholarship credited to student s tuition account **This scholarship is a four-year renewable award. The following criteria must be met to receive subsequent year awards: Maintain satisfactory academic record (GPA of 2. 0 or higher) Maintain satisfactory discipline record Remain a student at Notre Dame HS Continue to apply for and receive financial assistance through a FACTS application and finance committee award at Notre Dame High School Tuskes Family Scholarship Award Returning Notre Dame Student Students in grades 9, 10 and 11 are eligible to apply for this one year award Personal Data Form completed Proof of re-enrollment at Notre Dame HS for upcoming fall semester Student GPA of 2.0 or higher while at NDHS Student has a satisfactory discipline record List of co-curricular activities while at Notre Dame, including activities inside and outside of school and employment Student must provide two letters of recommendation Student must have applied for and been awarded financial assistance through a FACTS application at Notre Dame High School Student submits one essay (typed, double spaced, one page) titled: Receiving an ND education is important to me and my family because.? Award: Minimum of one (1) $1,000 scholarship payable to the student s NDHS tuition account Tony Koury/Our Lady of Lebanon Church Award Students in grades 9, 10, 11 and 12 are eligible Student must be an active member of Our Lady of Lebanon Church Personal Data Form completed Award is dependent on number of applicants and available fund from the church donation each year DEADLINE: May 1, 2018

5 GRADUATING SENIOR SCHOLARSHIP AWARDS Red White & Blue Club s Military Scholarship for High School Seniors Current ND Senior Students eligible only Applicant must have a parent, step-parent, grandparent or sibling who is serving or has served in the US Military Applicant must have a 2.5 GPA or better Applicant must be enrolling in a 2- or 4-year college, university, or trade school 2018 Award: One (1) $1,000 scholarship awarded DEADLINE: March 12, 2018 Application available on school website under the Student Life Scholarships & Internships drop down tab or in the guidance office. For award: Two (2) awards in the amount of $1,000 each Tuskes Family Scholarship Fund Graduating Senior Award Current Notre Dame senior students, who have been accepted into a two- or four-year post secondary institution are eligible to apply Personal Data form required Student has achieved a 2.0 GPA while at NDHS Student has a satisfactory disciplinary record while at NDHS List of all co-curricular activities while at NDHS, including activities inside and outside of school and employment Student must provide two letters of recommendation Student has received financial assistance through the Notre Dame HS financial aid committee while at NDHS For award: One (1) $1,500 award credited to the higher education institution on behalf of the student This scholarship is a four-year renewable award. For subsequent years, Student will provide the following: Maintain a satisfactory academic record (GPA of 2.0 or higher) Proof of re-enrollment
