1 The Bishop Joe Sartoris Excellence Scholarship For Bishop Montgomery High School This scholarship will be awarded to students who have displayed an outstanding track record and shows great potential in the areas of arts & athletics, academics and spirituality. The scholarship will provide at least one-half of a year s tuition depending on merit and need is renewable every year there after up to a total of four years; and Bishop Joe Sartoris Excellence Scholarship 1

2 This scholarship is inspired by Bishop Joe Sartoris, whose leadership has guided countless people to live fuller and happier lives. Bishop Joe was born on July 1, 1927 in Los Angeles, CA. He was ordained to priesthood on May 30 th, 1953 after attending St. John s Seminary. He was Pastor for St. Margaret Mary in Lomita for 16 years and was ordained Auxiliary Bishop of L.A. by Cardinal Roger Mahoney on March 19 th, He served as Regional Bishop of San Pedro until retiring in This scholarship is sponsored by Jason Vivas BMHS Class of 96 MIT, Bachelors & Masters, 01 And other charitable contributors Bishop Joe Sartoris Excellence Scholarship 2

6 APPLICANT AGREEMENT I certify that the information reported on this application is true, complete, and accurate to the best of my knowledge. I will notify Jason Vivas of any changes in my financial status or my future plans, such as the post-secondary school that I attend. I authorize Jason Vivas to secure information including transcripts or other school records, including financial aid and business account information. I understand that the Scholarship will be paid directly to Bishop Montgomery High School. I understand that a follow-up questionnaire will be required to renew the Scholarship. Failure to meet renewal conditions will mean forfeiting funds. Renewal conditions will be sent to the winner of the Scholarship and are subject to change. The Scholarship will adjust for any raise in tuition over the course of 4 years. This Scholarship is not pre-funded. Jason Vivas or next of kin will have the right to cancel the Scholarship in the case of any extraordinary event.
