4 Westwood Community School Foundation Scholarship Submittal Requirements 1. Print and/or complete the application form and attach additional sheets if needed. 2. Enclose a copy of your most recent high school transcript. 3. Enclose a short essay, no more than one page, completed by you, the applicant, outlining your educational plans and career goals. Please include motivating factors or experiences that have helped shape your personal philosophy and moral character. 4. Enclose one letter of reference from a teacher, coach, or counselor and one letter of reference from someone in the community (clergy, friend, employer, etc.) using the attached form and have them returned to the school. 5. Applications must be returned to the Westwood High School Office C/O Foundation Scholarships at 1000 Rebel Way Sloan, IA by Friday, April 15 th Applications will not be considered unless all the requirements above are completed. All entries are confidential and will only be viewed by the Westwood Community School Foundation Scholarship Committee. The Westwood Community School Foundation will be awarding six $500 scholarships this year to high school seniors in the Westwood Community School District. To be eligible a student must have a minimum 2.5 Cumulative GPA and meet the specified enrollment guidelines below. The applicant must be a student who is planning to enroll in a full-time undergraduate course of study at an accredited, two year or four year university, or vocational/technical school. The scholarships must be used for education costs billed by the school, and the student will receive the scholarship for the spring semester once enrollment verification is received. The scholarship will be paid directly to the school of enrollment. *Westwood Community School Foundation Board Members with a conflict of interest (any relative) with any applicant will not be a part of the Foundation Scholarship Selection Committee.
