3 Available to High School Seniors COLLEGE SCHOLARSHIP PROGRAM Wilkes Communications is a non-profit organization that is governed by a Board of Directors. Wilkes provides local and long distance phone service, high speed internet service, digital television, security and health monitoring service. In 2016, Wilkes will award scholarships to high seniors. This is for the year 2016 only. Eligibility Applicant must: Be a graduating high school senior. Their primary residence or business must receive their local telephone service from Wilkes Communications. The service must have one of the prefixes listed: Clingman 984; Lomax 957, 928; Champion 973 or Boomer 921 exchanges or be a customer in good standing in our 990 exchange. (cellular telephone service is not included) How to Apply Complete the scholarship application in full, along with a photograph. Have a high school teacher or counselor complete the Educator's Recommendation. In a one-page-or less essay, describe why this scholarship is important to you; why you wish to attend college; and what you would like to be doing in 10 years. Submit two letters of recommendation from local community leaders (telephone company personnel, elected official, or local business representatives) with your application. Letters cannot be from teachers, guidance counselors or other school personnel. Include title and organization for each individual. Include your most recent grade transcript with your application Award Procedure Scholarships will be awarded in April 1, Winners will be selected by a qualified committee. All entries will be acknowledged by April 1, Winners will be notified by mail by April 1, Scholarships will be awarded on a one-time basis and will not be renewable. Award will be paid on proof of registration at an institute of higher education.

4 Scholarship will be awarded without regard to race, ethnicity, national origin, religion, sex, or disability. Incomplete applications will not be considered. Make sure to: Include your application form, along with photograph. Attach your transcript. Attach your one-page essay. Attach your Educator's Recommendation Form. Attach your two letters of recommendation. No faxed copies will be accepted.
