TO: BUSINESS ADMINISTRATION OFFICE, FINANCIAL AID OFFICES NEBRASKA COLLEGES The Independent Insurance Agents of Nebraska Foundation is now accepting applications for the Curtis M. Elliott Memorial Scholarship in the amount of $1000. The Foundation is a non-profit corporation organized under the laws of the State of Nebraska in 1989 for educational uses and purposes. Selection of the scholarship winner will be based on scholastic application, as well as participation and leadership in school and community activities, and educational goals. Applicants must be a Nebraska resident enrolled as a full-time student and at least a second semester college sophomore or a junior at an accredited postsecondary institution located in Nebraska. Scholarship guidelines and application are enclosed. Please announce the availability of the scholarship to your students. 3 INDEPENDENT INSURANCE AGENTS OF NEBRASKA FOUNDATION SCHOLARSHIP PROGRAM Dr. Curtis M. Elliott Memorial Scholarship Guidelines 1. Applicant must be a Nebraska resident enrolled as a full-time student and at least a current second-semester sophomore or a junior at an accredited post-secondary institution located in the state of Nebraska. 2. Applicant must have an overall cumulative grade average of B or its equivalent, or better, in order to qualify. The selection committee will also consider the applicant s participation and leadership in school and community activities, and educational goals. 3. There are no criteria relative to gender or financial need. 4. Applicant must be a business major with a preference for an insurance-related field. 5. The Independent Insurance Agents of Nebraska Foundation will determine the number and amount of scholarships to be awarded each year. These numbers and amounts may change annually. 6. Applicants must submit evidence of being enrolled as at least a second-semester college sophomore along with an official transcript to the selection committee. Scholarships are not awarded until the recipient is enrolled as a junior. The scholarships will be paid directly to the college or university where the recipient is enrolled and reports will be required from the institution regarding the recipients enrollment in the institution. 7. Selections are made by a committee consisting of Foundation directors and community business and professional leaders. 8. There is no limitation on persons who are eligible recipients of scholarships. Scholarships will be given without regard to race, creed, religion, national origin, sex, or employment status of prospective recipients or of any relative of any prospective recipient. 9. In the event the scholarship winner does not accept the scholarship by failing to enroll as a junior in a college or university, the gift will lapse. Scholarship winners are only eligible one time in their college career. 10. Applications must be postmarked by March 1, 2015.

If the applicant is chosen as a scholarship winner, the applicant agrees to provide the Selection Committee with evidence of being enrolled as a college junior and an official college transcript after his/her first sophomore year of college.
