1 2017 HIGH SCHOOL SENIOR SCHOLARSHIP AWARD REGULATIONS ELIGIBILITY/LIMITATIONS FOR SCHOLARSHIP AWARD Applicant and sponsor must meet the following eligibility requirements: 1. Applicant must be a senior graduating from a high school or home school program during the school year. 2. Applicant must be a dependent with a current identification and privilege card. 3. The sponsor of the applicant must meet ONE of the following eligibility requirements: Active duty U.S. Military member currently assigned in Hawaii Active duty U.S. Military member on a remote assignment from Hawaii Retired U.S. Military member residing in Hawaii Full time Hawaii National Guard member** Full time U.S. Military Reserve member residing in Hawaii** Deceased U.S. Military member with applicant residing in Hawaii **Please contact hoscscholarship@gmail.com to verify eligibility** The following applicants are NOT eligible for an award: 1. Anyone who has previously been selected for an HOSC or First Command scholarship award 2. Anyone on a full scholarship 3. Selection committee members and family members of selection committee members 4. Scholarship coordinator/co-coordinator and family members of scholarship coordinator/co-coordinator 5. HOSC executive board members and family members of HOSC executive board members CRITERIA FOR AWARD 1.Educational information 2.Employment Information 3.Volunteer service hours 4.Non-school related clubs/activities and self-improvement activities 5.School activities and leadership roles 6.Awards/honors 7.Personal essay 8.Letters of recommendation APPLICATION INSTRUCTIONS The HOSC and the Scholarship Committee are not responsible for lost or misdirected applications. Please make sure your application is complete. Incomplete applications, or those received after the deadline will not be considered. Application must be RECEIVED by March 1, Applications must include ALL of the following: 1. Copy of sponsor s military orders showing assignment in Hawaii or retirement paperwork (DD 214) 2. Official military document proving applicant s dependent status if not shown on sponsor s military orders or DD Proof of residency of sponsor (example: copy of utility bill with home address) 4. Officially sealed high school transcript 5. Officially sealed ACT or SAT report unless already reported on high school transcript 6. 2 COPIES OF EACH - 2 Letters of recommendation from teacher, counselor, employer, community member or service professional written within the past year (family/friend references not accepted) 7. 1 COPY Applicant Information Sheet (Attached) 8. 2 COPIES Activity Information Sheet (Attached) 9. 2 COPIES Applicant Essay. Home school students please note - In addition to the previous instructions, the following are required: Copy of completed form 4140 or acknowledged letter of intent to homeschool signed by the principal of the neighborhood school and complex area superintendent. A high school transcript is required, but does not have to be sealed Copy of OFFICIAL ACT or SAT report (regardless of whether it is reported on high school transcript) 1 of 7

2 ESSAY QUESTION Answer the following with a one page typed essay on an 8 ½ x 11 single-sided, single spaced page with one inch margins using Times New Roman 12 point font. Essays exceeding more than one page will not be accepted. How has your family s military background affected the way you see the world? How does it impact your life now and how will it impact your life as you move forward with your future goals? MAILING APPLICATION Please submit completed application by mail. BOX Honolulu, HI NUMBER AND AMOUNT OF AWARDS The HOSC Governing Board will approve the total number of scholarships and total amount awarded. SELECTION COMMITTEE Scholarship applications shall be reviewed and judged by an outside source other than the HOSC governing board, advisors, and any active duty military members and dependents. Decisions of the selection committee are final. AWARD RECIPIENT RESPONSIBILITIES 1. The scholarship must be used by the recipient at an accredited trade school, 2-4 year college, or university, during the fall semester of 2017 and/or spring semester of In addition, scholarships may be used at a school pending accreditation by an organization approved by the US Department of Education. 2. The scholarship may be used for the payment of tuition, fees, and/or room and board charged by the Finance or Business Office of the school. 3. Recipients must submit to the Scholarship Coordinator a Use of Awards Agreement and Statement of Intent form no later than September 30, Use of Awards Agreements and Statement of Intent forms will be given to each recipient following the announcement of winners. 4. An official Verification of Enrollment letter from the educational institution s Registrar must be sent to the Scholarship Coordinator and received by September 30, Electronic verification through National Student Clearinghouse will also be accepted. Any fee associated with this service is the responsibility of the recipient. 5. The HOSC Welfare Treasurer shall issue a check in the full amount of the scholarship payable to the educational institution that has verified the recipient s enrollment. The Scholarship Coordinator shall forward this check within one month of receiving all necessary information from the recipient and the educational institution. 6. Any scholarships not claimed by September 30, 2017 will be forfeited. 7. Recipients who plan to attend only the spring 2018 semester must provide written intent of this to the Scholarship Coordinator by September 30, 2017 and verification of enrollment by January 30, QUESTIONS Contact the scholarship coordinator at hoscscholarship@gmail.com PLEASE NOTE Correspondence between the scholarship coordinator and applicant will be via .
