2 Senior Academic/Merit Scholarship Application Instructions FOR 2016 GRADUATING HIGH SCHOOL SENIORS The Hurlburt Spouses Club is pleased to offer a Turnaround Scholarship to selected Air Force dependents that are graduating high school seniors and have had the dedication and resolution to improve their academic career through the four years of high school. ELIGIBILITY. Applicant must meet ALL of the requirements below: 1. The applicant must be a class of 2016 graduating high school senior. 2. The applicant must be a dependent of any Hurlburt Field military member. This includes: a. Active duty personnel stationed at Hurlburt Field b. Retired military personnel* c. Deceased military personnel* d. A military member currently on an unaccompanied tour from Hurlburt Field* e. A military member reassigned (out of area) from Hurlburt Field but the applicant remained behind to graduate from high school f. A military member stationed at Hurlburt Field but the applicant resides elsewhere Note: *If the military member is retired, deceased or remote/unaccompanied, the member s last duty station must have been Hurlburt Field unless the applicant is a dependent of a current Hurlburt Spouses Club member AND 3. The applicant must have a valid military identification card. 4. A principal, counselor, or teacher must recommend applicant. 5. The applicant must have had a GPA below 3.0 the first year of high school and made significant improvement throughout the remainder of his/her high school career. 6. The applicant may not be a family member of the HSC Scholarship Committee nor of any of the judges. RESTRICTIONS 1. This scholarship may be used only for payment of tuition, room, board, books or other fees paid through the finance/bursar s office of an accredited school of the student's choice. Accreditation must be recognized by CHEA, HLC or another authorized accrediting body recognized by the US Department of Education. 2. Students receiving Spouses' Club scholarships from any other base will be ineligible to receive this scholarship. 3. It is the applicant s responsibility to let the HSC know if they become ineligible for this scholarship. 4. The winner shall not be awarded the actual scholarship until he/she is accepted at an accredited school and provides the HSC Scholarship Chairperson a verification of enrollment from the school. The Scholarship Chairperson will mail the scholarship award directly to the school of choice. 5. Should a recipient transfer to another accredited school before using all available funds, remaining funds may be transferred by the original school to the new school of the student's choice. The HSC Scholarship Chairperson must be notified immediately. 6. Should the winner cease to attend classes at the school of choice and not immediately transfer to another school, any money remaining in this fund shall be returned to the HSC Scholarship & Charitable Account. 7. Any student planning delayed school enrollment should notify the HSC Scholarship Chairperson immediately. All scholarship funds must be dispersed by March 1, Money not deposited with the student's school of choice by that date would revert to the HSC Scholarship & Charitable Account. 8. Scholarship money reverting to the HSC for any reason will be returned to the HSC Scholarship & Charitable Account. 9. Applicants may apply for only one HSC scholarship. 3 DEADLINES/CONFIRMATION OF RECEIPT: The application and all appropriate paperwork must be received by March 21, Incomplete or late application materials will not be considered. An or letter verifying that your application has been received will be sent to you. If you have submitted your application but not received this verification letter by March 25, 2016, contact the Scholarship Chairperson immediately. Read, complete and submit ONE copy of PART 1 (including the Statement of Understanding) signed by the applicant and his/her sponsor. 2. Complete the application form for Senior Academic/Merit Scholarship PART II. Submit FOUR total copies of each of these forms. 3. Ask your high school counselor complete the Counselor s Recommendation Form (PART III). Ask your counselor to make FOUR copies of this form and place them together in a sealed envelope with your name on it. Submit these with your application. 4. Submit two (2) letters of recommendation from people other than relatives (PART IV). One (1) letter must be academically based from a school official: teacher, counselor, and/or principal. One (1) letter must be a recommendation based on your moral character. Ask your letter-writers to make FOUR copies of their letter and place the copies (together) in a sealed envelope with your name on it. Submit these letters with your application. 5. Submit ONE copy of your sponsor s DD form 214 OR military orders (thoroughly black out SSN) OR Memo for Record: Verification of Eligibility for HSC Scholarship (PART V) a. Dependents of retired members PLEASE HIGHLIGHT THE BLOCK THAT SHOWS MEMBER RETIRED FROM HURLBURT b. We need only ONE copy of either documentation 6. Write and submit FOUR total copies of a personal essay of no more than 500 words expressing your short and long-term educational and professional goals. You may include your experiences, values and/or interests. 7. Submit ONE official school transcript of last seven (7) semesters and THREE copies of that transcript 8. Ensure that your name is on all supporting documents. HIGH SCHOOL COUNSELOR INSTRUCTIONS: 1. Complete the Counselor's Recommendation Form and work with student to obtain supporting documents. 2. It is ultimately the student's responsibility to ensure that the scholarship package is complete. However, if your school prefers to aid the student in completing the package, please verify that FOUR total copies of the application and appropriate supporting documents are included. Applications without the requested copies and all supporting documents will not be accepted! JUDGING AND AWARD NOTIFICATION A panel of independent judges from the local community selects the scholarship recipients. They are not affiliated with the HSC and do not have access to applicant identification during the selection process. You will be contacted by April 11, 2016 if you have been selected for the award. An awards ceremony will be held in your honor in mid-april. Further details will be sent via US mail and/or . Applicant Information: Name: Address: Phone: GUIDELINES FOR LETTER WRITERS The student listed above is applying for a High School Senior Turnaround scholarship from the Hurlburt Spouses Club (HSC). Scholarship recipients are selected based on significant academic improvement during their four years in high school, awards/honors, school & community activities, leadership and community service. We use the letters of recommendation to gather additional information about an applicant for our scholarship program. Each applicant must submit one letter of recommendation from his/her school and one from another source. We re looking for an overall picture of the applicant s: Personal/moral characteristics Academic performance (if applicant is associated with you in an academic setting) Strengths/capabilities Growth/evaluation of character over the period of time that you ve known the applicant Please note that we do not accept letters of recommendation from the applicant s family members (immediate or extended). Make four copies of your letter of recommendation (one for each judge) and place all four copies in a sealed envelope.
