Applications must be received by the deadline date. Application Checklist Complete answers to every question. If the question does not apply to you or your situation write not applicable or N/A as appropriate. Type or fill out the application in ink (PRINT). Do not write in pencil. Do not send applications in plastic folders or use staples. Detach this cover letter before submitting your application. Send application and all required attachments together. Paper clip each completed application and required attachments together. No staples please. Please put all applications in one large envelope, do not fold. We will not accept applications by fax, , items separately mailed or after the deadline date. Scholarship awards will only be paid to the schools, not to individuals or bookstores. Scholarships are for tuition only and accredited schools. You must be a full time student unless otherwise indicated in the scholarship requirements. Eligibility Criteria Every scholarship is different. Please check the list of available scholarships to make sure that you meet the criteria and different requirements of the scholarship(s) for which you are applying and the deadline dates. 1

2 Southern Illinois Community Foundation requires an application form for each scholarship you are applying for and each application must clearly identify the scholarship. Make sure that you read the required attachments for each scholarship; certain scholarships require additional information and have different deadline dates. Late and incomplete applications will not be considered Make a copy to keep for your records. Questions Please call: (618) or janice@sicf.org REQUIRED ATTACHMENTS In addition to a completed application form for each scholarship, each application must contain the following and in the order listed: Completed application form for each scholarship. Be sure to put name of scholarship in space provided at the top of page 3. Send only pages 4, 5, 6, 7, 8 and 9 of the application with your required attachments. Three and only three CURRENT letters of recommendation. o They can be from teachers, professors, guidance counselors, or non-academic references who are not relatives. o At least one letter should address your character and one should address your scholastic potential. o Please do not include award certificates. o If letters are not current you will be disqualified. An OFFICIAL TRANSCRIPT from your present or most recently attended school. High School Students enclose a copy of your ACT and/or SAT score report if not listed on your transcript. Please submit official cost of attendance figures (tuition only) for your school, either on letterhead or website screen print. A copy of page one of the print summary of your Student Aid Report (SAR) showing your EFC number (estimated family contribution) and students name. Do not include the FASFA application. (Not required for Merit Scholarships) 2

9 2016 SCHOLARSHIP FUNDS F=Financial Need M=Merit Please reference the Scholarship ID on your application (i.e. F01, M01, etc.) F01 HICKAM FAMILY SCHOLARSHIP Established 2007 Due Date: No Award in 2016 Traditional Jerry Hickam and his family established this scholarship in 2007 to further the education of qualifying individuals in the Southern Illinois area above the high school level. ELIGIBILITY This is a Traditional Scholarship and will benefit students in the Southern Illinois area pursuing education above the high school level. F02 WELLMAN & ADELINE KERR MEMORIAL SCHOLARSHIP Established 2014 Due Date: No Applications in 2016 Traditional The Kerr Trust was established in memory of Wellman and Adeline Kerr, former residents of Massac County, Illinois, who, although having moved away, wanted to remember the place they grew up. ELIGIBILITY Massac County High School graduate, continuing education at college, university or technical institute Award based on the need for financial assistance Academic performance of "C' average or better M01 MASSAC COUNTY ACADEMIC SCHOLARSHIP Established 1986 Due Date: See Joppa and Massac County High School guidance counselors Merit Every year since 1986 deserving seniors from Joppa High School and Massac County High School have been recognized for their hard work and academic accomplishments. With the collective support of many businesses, organizations, churches, professionals and private donors, over $285,000 has been awarded to over 360 students. This scholarship fund was originally established by the Kiwanis Club of Metropolis, IL. In 2012 the Metropolis Rotary joined the effort which has increased the amount of money that can be awarded. In 2015, 16 scholarships were awarded in amounts ranging from $1100 to $

M02 TOM MYGATT MEMORIAL SCHOLARSHIP Established 2013 Due Date: March 18, 2016 Merit Tom Mygatt, his wife Janet and all four of their children, Thomas, Carrie, Courtney and Cinnamon, graduated from Sesser-Valier High School. Tom and Janet attended SIU-Carbondale, but did not graduate as they went to work to support their growing family. Tom and Janet s dream was to ensure that all of their children obtained a college education and all four of the children obtained that college education by graduating from SIU-Carbondale. Tom believed in the value of education and the power it has to help increase knowledge, expand your horizons by educating about the world outside of your current universe and providing opportunities to fulfill your dreams. The purpose of the scholarship is to assist someone who wants to have those opportunities. ELIGIBILITY Graduate of Sesser-Valier High School 2 page essay, double spaced, font size 12 on how you will achieve your career goals. 11 M03 TIM HERRING MEMORIAL SCHOLARSHIP Established 2014 Due Date: March 18, 2016 Merit Timothy "Tim" J. Herring ( ), his wife Amy (Troue) Herring and their 2 kids Olivia Liv and Sam are avid fans of baseball and softball and huge Saluki and Cardinals fans. Tim carried a four-year 4.0 GPA and was a three-year varsity letterman in football & baseball for the Murphysboro Red Devils as well as the baseball MVP his senior year. Tim played college baseball at Three Rivers Community College and John A. Logan College. Tim enjoyed playing fast pitch softball for many years, travelling and playing for area teams all over the United States. He loved to hunt and fish and was a member of the Treece Hunting Club. Tim was a member of Immanuel Lutheran Church in Murphysboro and a member of the Plumber and Pipefitters Local # 160 of Murphysboro, and worked at SIU-Carbondale as a pipefitter. This scholarship program has been established as a lasting tribute to Tim s love of baseball & softball by the friends and family of Tim Herring. ELIGIBILITY Four year Murphysboro High School student, continuing education at college, university or technical institute Four year baseball/softball athlete Grade Point Average of 3.0 and above Recommendation letters from each of the following: Coach Teacher Community Member 500 words or less essay on the following topic: What are my goals and how I plan to achieve them Listing of Academic Awards/Honors Clubs/Societies/Organizations M04 JASON SETTLEMOIR KEEP THE MUSIC PLAYING MEMORIAL SCHOLARSHIP Established 2014 Due Date: March 18, 2016 Merit Jason Settlemoir ( ) graduated from Benton Consolidated High School in 1996 and was very active in the Fine Arts, playing trombone in the Band and Jazz Band, as well as participating in the Madrigals and Jazz Choir. He was an outstanding trombone player and continued with his talent and passion for music after graduation. He obtained his Bachelor's degree in Music Education from Illinois State University, and then attended the University of New Mexico where he received a Master's Degree in Music Performance. 11

12 Those who knew Jason appreciated his wit and sense of humor as much as his outstanding musicianship. The Jason Settlemoir Keep the Music Playing Memorial Scholarship is awarded to a BCHS senior who has decided to keep the music playing after high school by majoring or minoring in the Fine Arts. This scholarship program has been established as a lasting tribute by the friends and family of Jason Settlemoir.
