1 LEESBURG LIONS CLUB SCHOLARSHIP AWARD WCHS SCHOLARSHIP APPLICATION PROCEDURES CLASS OF 2019 ALL ITEMS REQUESTED SHOULD BE FILED BY THE DUE DATE OF APRIL 1, RETURN THE COMPLETED APPLICATION PACKET TO THE FOLLOWING ADDRESS: Mr. Robert W. Bishop 1865 W. 700 N. Leesburg, IN This award will be presented to the student who attended Leesburg Elementary School for at least two years and graduated from Warsaw Community High School or parent is a member of the Leesburg Lions Club. The Lions Club will be granting three (3) scholarships of $1, each this year. REQUIRED ITEMS ARE LISTED BELOW: 1. Application form (attached) 2. Transcript of Credits 3. Current Photograph 4. Two Letters of Recommendation: One from a Minister, Employer, Teacher, or Professor, and one from the Applicant s Choice. (ALLOW AMPLE TIME FOR MAIL DELIVERY) APPLICATIONS RECEIVED AFTER APRIL 1, 2019 WILL BE REFUSED. HELPFUL HINTS POWERFUL SUGGESTIONS A. Obtain copy of your high school transcript from the Guidance Office. Make sufficient copies of the transcript to accompany all scholarship applications you plan to submit in the future. B. Get the letter(s) of recommendation needed. See if the letter writer would permit you to make copies for use with other scholarships. DON T wait until the recommendation is due before you ask the person to write one for you. Give the writer at least two weeks notice. C. Be sure to include a picture where it is called for. Here is where an inexpensive, billfold size photo would come in handy.

2 LEESBURG LIONS CLUB SCHOLARSHIP AWARD 1. Three Grants of $ 1, ($ per semester) will be awarded. 2. Factors considered in awarding the scholarship include: A. Academic Achievement = 30 B. Community, church, and school involvement = 35 C. Essay = This award will be presented to a student who: A. Attended Leesburg Elementary for at least 2 years and graduated from Warsaw Community High School. B. Or whose parent is a member of the Leesburg Lions club. 4. A copy of the applicant s transcript and scholarship fact sheet must be submitted along with two letters of recommendation from: A. Minister, Employer, Teacher or Professor. B. Applicant s choice 5. This award may be presented to students pursuing a college, vocational, or technical degree. The award recipients will not be eligible to apply for future awards. 6. The recipients will be selected by a Selection Panel of two individuals from the Leesburg Community not affiliated with the Lions Club and one Lions Club member. The panel reserves the right to refuse any or all applicants. Panel also reserves the right to interview any or all applicants. 7. The recipient will be notified at the annual awards program unless the recipients currently in college or vocational school. In that case, the recipient will receive written Notification. 8. Applications are available at the High School Guidance Office, Leesburg Office of Farmers State Bank and the Kosciusko County Community Foundation.
