1 JOB DESCRIPTION Job Title: Location: Salary: Hours: Type of Contract: Accountable/Responsible to: Staff Accountable to Postholder: Senior Community Dietitian Enteral Feeding Manchester 29,000 to 35,000 (pro rata for part time) 40 hours (part time applicants welcome) Permanent Dietetic Service Lead Community Dietitians Job Purpose: To join a newly formed community nutrition support and home enteral feeding team of dietitians and assistant practitioners. Supporting patients in clinics, their own homes or across care and nursing homes in Manchester. In line with ABL s holistic approach to health care, we require an experienced community dietitian to join our team. The post holder will provide clinical leadership to junior dietetic colleagues as well as holding a caseload of complex patients in home enteral feeding. Additional to this the role will contribute to service development and professional training programmes. ABL is passionate about supporting and training staff and continuing professional development, and the post holder will receive and contribute to regular training and supervision. You will be joining a small but expanding team of passionate and friendly dietitians and assistant practitioners and will be based with a wider team of friendly and supportive health and community professionals. 2 Duties and Responsibilities To be responsible for planning and prioritising own clinical nutrition support and HEF caseload. Patients may be referred for a variety of conditions (e.g. gastroenterology, oncology, cardiology, diabetology, neurology and stroke, terminally ill and palliative care patients). To provide dietetic assessment and treatment at a highly developed level, using specialist skills and knowledge to manage nutrition support and HEF caseload. To provide high quality, nutritional and dietetic advice contributing to the care of patients, enabling them to make informed choices about their health and related behaviours. This will involve calculation of nutritional requirements, assessment of oral and non-oral intake, lifestyle and barriers to change, interpretation of biochemical and anthropometric measurements, and consideration of social and cultural needs. The outcome of the dietetic consultation will be an agreed individualised patient nutrition action/care plan. To make clinical decisions, liaising with community and hospital nutrition support dietitians and feed company as necessary, about the appropriate and safe administration of enteral nutrition to the patient; making changes to the enteral regimen based on nutritional status, clinical condition, biochemical, fluid status monitoring, and taking into account the social situation and cost effective prescribing. To contribute to clinical decisions regarding nutritional appropriateness of intensive nutritional treatments, e.g. when dealing with referral for HEF for patients with Stroke, Neurological and Dementia. To make clinical decisions concerning necessary changes to patients dietary intake and oral nutritional supplement prescriptions, and communicate these decisions to other healthcare professionals as appropriate. To liaise with other health professionals regarding the transfer of patients from home to hospital requiring dietetic follow-up, including malnourished and clinically ill patients requiring nutrition support. To liaise with Acute Dietitians and out of area Dietitians to facilitate the safe transfer of appropriate patients onto the HEF caseload for continued monitoring and support by the Community Nutrition Support and HEF Dietitians. To correspond with clinicians and GP s regarding appropriate nutritional treatment plans, prescriptions and investigations following dietetic assessment. To be responsible for facilitating the ordering of enteral feeds and enteral feeding ancillaries in accordance with the agreed protocol for patients receiving HEF. To monitor the provision of appropriate HEF equipment to patients To work with the dietetic service lead and other dietetic team members to produce dietetic resources. To provide specialist advise to GPs and other prescribers on the appropriate prescribing of nutritional supplements (including energy, protein, vitamin and

3 mineral supplements) and medications to improve enteral feed tolerance, as well as advising on potential drug-nutrient interactions. To be responsible for obtaining informed consent from patients prior to the commencement of dietetic intervention and treatment. To provide supervision and support to dietetic and other staff within ABL To continue the development of training and teaching sessions concerning the identification, treatment and monitoring of malnutrition for health and community care staff. To achieve and demonstrate agreed standards of personal and professional development within agreed timescales. To establish links with Community Nutrition Support Dietitians in other trusts to promote sharing of good practice and evidence based research. To attend and actively participate in Community Nutrition Support and HEF Dietitians Team meetings. To record and submit clinical and professional data according to ABL Health standards. To promote and maintain efficient and safe use of equipment used within the service. To work within the framework of Clinical Governance in order to maintain and improve patient care, quality and efficiency. To demonstrate personal and professional development in line with the ethos of lifelong learning. Be able to travel across Manchester to deliver a dietetic service to patients in their own homes, care homes, day centres, clinics and other community settings. 4 The postholder will be responsible for maintaining the clinical and/or corporate records that fall within the remit of this role to the standards in ABL s records management policies, and data quality processes and standards.
