1 POSITION DESCRIPTION Position Details: Title: Registrar Reports to: Clinical Director of CTSU or his nominated Consultant Department: Cardiothoracic Surgery (CTSU) Location: Auckland City Hospital Mercy Ascot Hospital Date: September 2014 Purpose of Position: In this 12 to 24 month placement, the Registrar will facilitate the management of patients care of the Cardiothoracic service and gain training and skills within this surgical specialty. Key Accountabilities: General work closely with the team s House Officer, provide supervision and share responsibilities be responsible for the assessment and admission of acute and elective patients allocated to their team. Undertake clinical responsibilities as directed by the Consultant, organise relevant investigations and ensure the results are followed up, sighted and signed in a timely fashion be responsible for patient referrals and to ward management of patient s ir team s care, consulting with others involved in the care of the patient where appropriate and as directed work closely with medical specialists in the provision of assessment and investigations of new follow-ups in outpatient clinics undertake diagnostic and treatment procedures as appropriate to the sub-specialty maintain a high standard of communication with patients, their families and other staff inform Consultants of the status of their patients, especially if there is an unexpected event attend hand-over, team departmental and interdepartmental multidiciplinary meetings as required. Admitting assess and admit cardiothoracic surgical patients referred from other hospital services or the community

2 On Call be available on long range locator and able to attend the hospital within 15 minutes review patients on the cardiothoracic surgical ward when requested by other medical staff or as necessary, for the assessment and management of all medical and surgical issues related to ADHB patients. provide advice to and liaise with GP s and other hospitals medical staff on matters relating to the cardiothoracic surgical sub-specialty authorise for patients to be transferred to and seen by the cardiothoracic surgical service when appropriate, in consultation with the appropriate Cardiothoracic Surgical Consultant. Inpatients review the patients of their nominated /s at the commencement of each and as necessary, to undertake examination, assess progress, organise investigations and update management plans throughout the complete discharge planning, including instructions for discharge medications and plans for follow-up, prior to the patient s discharge. Dictate a discharge letter for the patient s GP, following discharge assist in the operating theatre the weekly operating theatre schedules and perform cases for senior staff, at both the Auckland City and Mercy Ascot Hospitals, when requested. See pre-operative ensure results of investigations are available prior to surgery, including clinical summaries inform on-duty medical staff about patients whose condition requires monitoring and review assist in the provision of Heart & Lung Transplantation service, as requested / as possible. Note: This may involve travel with the retrieval team to locations outside of Auckland. supervise the work of House Officers responsible for the care of patients admitted under his/her nominated /s Outpatients assess and manage patients referred to outpatient clinics and run the clinics on behalf of Consultant medical staff, where appropriate. Discuss patients seen with the appropriate Consultant before any major management decisions are made communicate with the referring person following the patient s attendance at clinic Administration maintain a satisfactory standard of documentation in the files of patients. Sign all prescriptions and notes with a printed name and locator number, legibly recorded be responsible for certifying death and completing appropriate documentation. Inform the coroner when necessary, after consultation with the appropriate Consultant assist with clinical research at the discretion of the Clinical Director dictate summaries on discharged letters to GPs following outpatient visits, in a timely fashion obtain informed consent for procedures within the framework of Medical Council guidelines as follows: 1. The practitioner who is providing treatment is responsible for obtaining informed consent beforehand for their patient. The Medical Council believes that the responsibility for obtaining consent always lies with the as the one performing the procedure, they must ensure the necessary information is communicated and discussed.

3 2. Council believes that obtaining informed consent is a skill best learned by the house surgeon observing s and experienced registrars in the clinical setting. Probationers should not take informed consent where they do not feel competent to do so. Roster Duty Details Day duty (Mon - Fri) hours Long duty (Mon Fri) (then on-call ) Weekend (1:7) Other Satur and Sun (on duty and oncall) Weekend night (1:7) Satur and Sun nights (on-call) The Registrar is expected to attend the evening ICU ward at 2000 hours (sharp) when on Long duty or Weekend duty. Other Cover Other Resident Cover Explanation of own service roster: Registrars may be required to participate in a 3 month rotation to the MercyAscot private hospital in Auckland, acting as 1 st assistant in operating theatre and providing ward cover, as instructed by the Clinical Director. Registrars may be offered the opportunity to assist in operating theatre for booked weekend contract cases for additional remuneration, as discussed with the Clinical Director.

5 Research Details Registrars are expected to do research with assistance given by the department as necessary. Performance Appraisal Registrar meet with their designated Consultant at the beginning of the run to discuss goals and expectations for the run, review and assessment times and 1:1 teaching time act on a corrective plan of action that is developed in conjunction with their following any assessment that identifies areas for development Service The service will provide: an initial meeting between the Consultant and Registrar to discuss goals and expectations for the run, review and assessment times and 1:1 teaching time an interim assessment report on the Registrar 3 months into the run after consultation with both the Registrar & assigned Consultant the opportunity to discuss identified areas for development. The assigned Consultant will bring these to the Registrar s attention and discuss and implement a plan of action a final assessment report at the end of the run, discussed with the Registrar and a copy of which is to be sighted and signed by the him/her Hours and Salary Category Average Working Hours Basic hours 40 (Mon Fri) Rostered additional hours 19.5 (incl. nights, weekends and long s) All other unrostered hours 3 (incl. Satur am ward s and Sun visitation of pre-operative patients) Total hours per week 62.5 Note: Out of hours worked with the Heart & Lung Transplantation service will be reimbursed at relevant hourly rates, or with time in lieu, as agreed with the Clinical Director and Service Manager Service Commitments The service, together with the RMO Support Unit will be responsible for the preparation of duty rosters Salary The salary for this attachment is Category B of the New Zealand Resident Doctors Association MECA.
