Aim of today s presentation Provide an insight into the power of interrai data Help you understand how you can harness that power What interrai Services can offer

What is interrai An international not for profit organisation Collaborative of clinicians and researchers in over 35 countries Responsible for developing comprehensive clinical assessment tools designed to work together A suite of clinical assessment tools, using a common language, common measures and clinical concepts NZ (MoH) has a royalty free licence with interrai International

Where are we now? MoU with MoH Providing a national interrai Service $8.5m annual funding interrai Services a dedicated business unit in TAS 4 main work streams Visual identity and communications established A new web site

Governance interrai NZ Governance Board established 11 members wide representation across the sector David Chrisp, Access Community Health is the HCHA rep interrai NZ Future Direction a 3-year rolling strategic plan To continuously improve health outcomes for NZers as they age and the effectiveness and efficiency of our health system by guiding and leading the use of interrai instruments and the dissemination and use of interrai information

Education and Support Education and support for interrai assessors and those using interrai information One integrated model providing consistent quality services nationally Becoming competent as an interrai assessor Staying competent as an interrai assessor Understanding and using interrai information for managers 609 Home Care assessors 2700 Aged Residential Care assessors

interrai Software Services 8 Single national software platform provided by Momentum Healthware Displays information for decision making and care planning Assessments move with people across care settings Allows the information to be aggregated to provide o clinical decision support tools, o case-mix classification systems, o monitoring and benchmarking measures, o algorithms to identify and target priority groups

The interrai Data Analytics and Reporting Team Established in July 2015 Core function: o Provide interrai data access, analysis, reports and interpretation Vision: o Assist stakeholders in their planning, decision making and quality improvement o Assist to improve health outcomes of New Zealanders as they age 9

What are we aiming to achieve Provide a high level overview from a national perspective on data from interrai assessments Make interrai analysis, data and information available and accessible Help people to understand and interpret interrai assessment data.

Assessment count by type

What we are looking at interrai outcome scales and Clinical Assessment Protocols (CAPs) are outputs from the assessment process Automatically generated using built in algorithms in the assessment Excluded Home Care clients who moved into aged residential care within 6 months of being assessed Outcome Scales o They are usually used to evaluate an older person s current clinical and functional status. CAPs o A triggered CAP is usually an opportunity to facilitate improvement, slow rate of decline or alleviate symptoms.

Method of Assigning Priority Level (MAPLe) 13 The MAPLe score is only used in Home Care assessments, not in Contact assessments or LTCF assessments. It is a priority indicator, with higher scores based on the presence of activities of daily living (ADL) impairment, cognitive impairment, wandering and behaviour problems. The MAPLe score is also a predictor of carer stress. The higher the score, the greater the priority for services to be commenced or increased in the community to prevent hospitalisation or admission into residential care.

Method of Assigning Priority Level (MAPLe) 16% of Home Care assessments were for clients with the highest MAPLe score, compared to 23% last year

Method of Assigning Priority Level (MAPLe) by DHB, 2015/16

Activities of Daily Living (ADL) Self-Performance Hierarchy Scale 16 The ADL Self-Performance Hierarchy Scale is a measure of functional performance grouping activities of daily living according to the stages of the disablement process in which they occur The scale is based on four ADL items showing the level of difficulty in Personal hygiene Locomotion Toilet use Eating.

How to access interrai data? Anyone can apply to access interrai data If you want to access interrai data at the client level, the interrai data access protocols apply They are available to download at Click on link to access an application form For enquiries,

If you are a Home Care Provider To access your data Your interrai assessor will have access to client level data via the interrai operational software Your DHB will have access to interrai data for your clients. Your DHB interrai educator or interrai system clinician is your first point of contact We will soon begin scoping a project to provide a national standard suite of interrai reports to home care and community support providers. An example HealthCare New Zealand We will engage with you through your representatives later in 2016.

The so what questions? Start asking the so what questions. It is those very questions that will ultimately influence our thinking and our decision making to improve health outcomes for New Zealanders as they age

Add value by working together Potential to add and link interrai with other datasets o Population census data o Workforce o Aged residential care quarterly bed survey o Community Pharmacy o Other national health datasets e.g. National Minimum Datasets, Clients Claims Payments System (CCPS) and many others

Where next? More actionable More visual More timely More interactive interrai Data Analysis Annual Report 2015/16 Dec 16 Building a standard suite of reports for ~670 aged residential care facilities Scoping a national standard suite of reports for home care providers interrai data visualisation tool interactive aggregated data

The power of interrai data Collect once use many times
