4 CONNECTING THE TRIPLE AIM & QUALITY IN THE CENTRAL WEST LHIN Achieving Transformational Change Strategic Quality Framework o Safe o Patient- Centred o Equitable o Effective o Timely o Efficient Governance & Leadership Provincial/Regional Alignment Regional Engagement ENABLERS Quality Governance Oversight is aligned with Provincial Framework through: - Central West LHIN Board of Directors - Regional Quality Table - Sub Regions Quality Improvement work is aligned with the Minister s Mandate Letter, H S F R and other provincial priorities, and deployed to the sub region level enabled by rigorous application of quality improvement science, analytics and informed by the voice of the patient and their families in their own health care planning and delivery Continuous quality improvement is conducted at all levels within the Central West LHIN leveraging community expertise and local connections between primary care providers, inter-professional health teams, hospitals and public health to promote consistency in care. Building QI Capacity & Capability Improvement work will be enabled through creating a culture of quality that includes the development of skills and knowledge in the use of quality improvement tools. Helping Professionals & Caregivers Thrive Staff and Caregivers will thrive in an environment that supports and promotes workplace engagement, wellness and excellence in patient care. Road Map of how we will achieve a Quality System in the Central West LHIN Quality Monitoring and Reporting Healthcare Improvement Scotland, 2010 Quality Improvement work is enabled by a structure which aligns a wide range of measurement across the LHIN that helps drive progress towards the Triple AIM and demonstrate improvement at the Institutional, Care Community, LHIN and Provincial levels. 4
