As part of this effort, Burleson Hospital focuses on improving in the following areas: Organizational Effectiveness: As a Catholic healthcare ministry, caring for the poor and vulnerable is central to our Mission and values, we track our service to the poor and vulnerable according to the amount of uncompensated care we provide. Culture of Satisfaction: We track patient satisfaction through the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), to assess everything from how well our doctors and nurses communicate and responsiveness of staff to pain management and hospital cleanliness. Clinical Performance: We are dedicated to providing compassionate, quality patient care. We track patient safety and ongoing health by looking at falls and the number of patients who are readmitted to our care. Finance: We are committed to responsible stewardship of our resources, so we track financial targets. Each month, we will share our progress in these areas as seen in the table below. Working in partnership with these providers, our Health Resource Center connects our local residents to agencies providing services ranging from basic shelter, food and clothing to more complex behavioral health counseling and other social needs. For more information on our Health Resource Center, please visit The following chart, designed to show the impact and reach of our center, includes the following: Client encounters, reflecting face-to-face service coordination, walk-ins and other interactions with the agencies located in our Resource Center Education events, including community discussions, financial stability workshops and self-administering medicine at home. Senior meals, including home deliveries, senior bag distributions and group meals. Information and referral, including phone calls, service coordination and walk-ins. Transportation provided to clients for medical care and other needs WIC clients, including women and children receiving food voucher support, nutrition counseling and other resources. With the expertise and hard work from our Plant Operations Management team and donations from the community, we have increased the size of the Gift Shop from 55 to 235 square feet. The Ladies Auxiliary has done an incredible job organizing and bringing in new items to offer the staff, as well as the community. It is definitely a must stop when shopping for gifts as well as food/drink items and books. We would like to thank those who made donations to the renovations, including: 2

CHI St. Joseph Health Skilled Nursing and Rehabilitation Update CHI St. Joseph Health Skilled Nursing and Rehabilitation Burleson Welcomes Dr. Delbert Price CHI St. Joseph Health Skilled Nursing and Rehabilitation welcomes Dr. Delbert Price of Deer Oaks Psychological Services. He will provide psychological services and support to our guests. Dr. Price has nearly 40 years of experience in various educational, therapeutic and administrative positions in treatment service systems, serving individuals with mental health and educational needs. He most recently served as Director of Health and Psychological Services for Brazos County Department of Juvenile Services. He holds his Doctoral degree in Educational Psychology from Texas A&M University, his Master s in Speech Communication from the University of Houston and Bachelor s in English and Speech from Houston Baptist University. Chaplain Heidi Returns to Provide Spiritual Services Chaplain Heidi has returned to CHI St. Joseph Health Skilled Nursing and Rehab. in Burleson to provide spiritual services to guests on Monday mornings. Resident Art Sale Ongoing Our resident art sale is currently ongoing with all proceeds going to Caldwell s Make a Christmas Happen. We sell resident-made artwork and décor to give underprivileged children an enjoyable Christmas. To date, more than $2300 has been collected. CHI St. Joseph Health to Celebrate Anniversary Milestones with Service, Community Event As part of our ongoing celebration of our two milestone anniversaries the 80 th anniversary of our founding and the 100 th anniversary of The Sisters of St. Francis of Sylvania, Ohio we are hosting a full calendar month of community service and a Christmas event for our community. We have more than 20 teams signed up for our Random Acts of Kindness Month throughout the month of November, commemorating the Sisters decades of service. Follow us on the CHI St. Joseph Health Facebook page to keep up with our month of giving, and participate in the conversation at hashtag #actsofkindness. To cap off our anniversary celebration, we will host the Magic of Christmas community event Sat., Dec. 10 at the Brazos County Expo Center, located at 5827 Leonard Road in Bryan. The community event will be open to the public from 10 a.m. to 2 p.m. This free event for our team members and the community will feature crafts, refreshments, a strolling magician, bounce houses, train rides, face painting and a silent auction, with proceeds benefiting the CHI St. Joseph Health Foundation Employee Campaign. This campaign supports training, scholarships and a crisis fund for our more than 3,000 team members. 3

4 Catholic Health Initiatives Supports Rural Access to Healthcare With 30 critical access and rural hospitals across its nation-wide network of more than 100 healthcare facilities, Catholic Health Initiatives is setting a high standard for rural access to healthcare in the communities they serve including yours. Critical access hospitals like CHI St. Joseph Health Burleson Hospital have a critical role to play in the overall health of Burleson County and surrounding area residents. Providing 24-hour access to emergency care services seven days per week, critical access hospitals stabilize patients and then, if needed, transfer them to a higher level of care, such as CHI St. Joseph Health Regional Hospital in Bryan. At CHI St. Joseph Health, we believe it s equally important to provide safe, quality health care in rural communities, as well as in urban ones. It s an extension and fundamental element of our Mission to serve the poor and vulnerable, because critical access hospitals serve many areas that might otherwise be unreached. In 2014 we chose to align with a much larger organization which shares in this service Mission: Catholic Health Initiatives. The country s third-largest nonprofit health system with total assets of $23 billion, CHI s vision is to create healthier communities and serve as a voice for our nation s most vulnerable. Year after year, CHI s advocacy priorities include preserving and promoting access to high-quality health care services in rural areas. Catholic Health Initiatives, Dignity Health Explore Aligning Ministries to Strengthen Catholic Health Care Catholic Health Initiatives and Dignity Health have signed a non-binding letter of intent to explore aligning our organizations and expanding our mission of service in communities across the nation. CHI s board and sponsors are evaluating the potential alignment to strengthen their leadership role in transforming health care through increased access and enhanced clinical excellence. The organizations complement one another in many other important ways. CHI brings a diverse geographic footprint with proven clinical service lines and home-health capabilities, as well as successful partnerships in research and education. Dignity Health has a proven operating model that has successfully scaled enterprise-wide initiatives to ensure consistent practices across the system, and is well known for its work with innovative, diversified care-delivery partnerships. Both systems include Catholic and other-than-catholic health care affiliates that share a commitment to care for all, including those who are poor and underserved. CHI St. Joseph Health Burleson Hospital A critical access hospital in Caldwell, CHI St. Joseph Health Burleson Hospital offers a Level IV Trauma Center with 24-hour emergency care, inpatient, outpatient, wellness, imaging, laboratory services and occupational, speech 4

5 and physical therapy. It is also home to a Senior Renewal Program, designed to enhance the emotional health of senior adults, providing group psychotherapy and individual and family therapy. Burleson Hospital, stroke accredited by the Texas Department of State Health Services, also provides cardiopulmonary services, providing oxygen therapy and EKGs. CHI St. Joseph Health Skilled Nursing and Rehabilitation CHI St. Joseph Health Skilled Nursing and Rehabilitation in Burleson County is a skilled and intermediate nursing care facility certified by The Joint Commission to provide long-term care.
