2 Grant Request and History of CBJ Support for JAMHI Programs The Juneau Alliance for Mental Health, Inc. ( JAMHI ), is a private, not for profit corporation. Since 1985 JAMHI has provided services and housing to seriously mentally ill adults in Juneau. In 2000 the City and Borough of Juneau asked JAMHI to take over its responsibilities as the Community Mental Health Center, including provision of general mental health and emergency services. Since that time, the Assembly has made an annual grant award to JAMHI to partially defray the cost of providing these services. JAMHI requests that CBJ continue its support at the current grant level of $350,000.

5 Service Components: Emergency and Rehabilitation Emergency 24 hour Emergency Mental Health (ES) provided by masters or doctorate level mental health professional clinicians are short-term, intensive psychiatric services provided to a person during an acute mental health crisis episode. ES Clinicians provide services to assess the mental status and risk of harm to self or others with referral recommendations. Emergency response occurs on-site at the Bartlett Regional Hospital, JAMHI offices and facilities, schools, Juneau Youth, Lemon Creek Correctional Center, and other local social service agencies. Psychiatric and Nursing Three contracted psychiatrists provide psychiatric assessment, medication management, and case consultation. Psychiatrists are available on an on-call basis to the nurse and clinicians for support consultations. An RN provides nursing services to assess client s psychiatric and side effect symptoms intra-muscular medications, education and support, and can screen for other medical and dental needs as required. Case Management A team of case managers and clinicians (8 FTE) provide community support for severely mentally ill clients; on-going outreach services; inter-agency coordination; and support to family members. Case management also provides training in life skills that encourage independent living.

6 Service Components: General Mental Health General Mental Health include individual and group focused/brief therapy and counseling; provided through a grant from the City and Borough of Juneau. Serves approximately 250 general mental health recipients. A portion of the CBJ Grant to JAMHI is passed through to Juneau Youth to provide General Mental Health to children in the amount of $39,630.

7 Service Components: Enhanced Residential Lemon Creek Residential Facilities: Salmonberry House - supportive residential program serving up to five individuals with severe mental illness. Crisis Intervention Program a 3-bed 72 hour supportive residential program serving individuals experiencing a crisis and providing temporary supportive services. Supported Living Apartments - provides support and skill development to 10 clients living in semiindependent transitional apartments. Salmon Creek Residential Facilities: 8 Plex for individuals with Co-occurring Disorders. Provides full consumer support for seriously mentally ill adults, with severe cooccurring substance abuse disorders. 8 Plexfor Seniorswith mental illness and cooccurring disorders. HUD Salmon Creek Housing, Inc. - a supportive residential program serving up to six individuals with mental illness who require more intensive 24/7 support. Independent Housing HUD JAMI Douglas Housing (Douglas Terrace) independent housing for up to 15 residents. Independent Apartments - JAMHI operates over 23 rental units as independent apartments for clients.

8 Service Components: Drop-In Center JAMHI provides a dropin center at our new group home (on the bus line near the hospital). The drop-in center is open from Noon 2:00pm every day, and is partially funded by the CBJ grant. The center provides a daily light lunch to its participants. Interpersonal communication skills, socialization, good nutrition are encouraged and supported at the Center.
