2 Communicator Establishes effective therapeutic relationships with patients and their families, which incorporates racial, cultural, socio-economic and gender considerations Utilizes patient centred interviewing skills and seeks corroborating information as needed Shares information and explanations that are clear, accurate and timely while checking for patient and family understanding Effectively communicates with patient s families and addresses their needs and expectations Communicates clearly and concisely across all domains (charting, letters, reports, transfer of care) and mediums (oral, written, electronic) Collaborator Works effectively with other members of the health care team. Participates effectively in inter-professional team meetings and contributes effectively to the shared psychiatry-fm model of practice Constructively works towards resolving conflict Leader/Manager Manages time and resources effectively Selectively initiates referrals considering likely benefit to the patient and availability of resources Completes patient related paperwork effectively and in a timely fashion Health Advocate Identifies vulnerable or at-risk patients and initiates appropriate interventions when their health care needs are not being met by health care system Is familiar and knows how to access community-based resources for patients Identifies when determinants of health are impacting patients and/or the communities in which they live Page 2 of 5
