3 PERSONAL STATEMENT During my career as a professional, I have extended my services in the field of healthcare as a healthcare provider. My aim is. Further, I intend to expand the services towards the community, increasing the community participation and ensuring the equity of the services delivered. I am dedicated towards the improvement of the healthcare system in Sri Lanka at all levels, including human resources development. I believe that this is achievable at a national level by both the improvement of the standards of healthcare providers as well as a system improvement. In order to achieve this I have been working in innovative national policy development and implementation at the grass root level ensuring their sustainability. Page 2

As part of my duties, I coordinated two modules in the newly introduced undergraduate medical curriculum. 2. I gained experience in curriculum evaluation as under the supervision of the Director, I was assigned the task of analyzing examination performances of students following the newly introduced medical curriculum in comparison with the old curriculum, and to make recommendations for improvement. 8 LEADERSHIP ACHIEVEMENTS AS A MEDICAL OFFICER My most satisfying achievement was being. UNDERGRADUATE LEVEL I was the.. SECONDARY SCHOOL LEVEL I was RESEARCH, the Head Prefect PUBLICATIONS of my college AND and captained CME ACTIVITIES the college quiz and debating teams. I was selected as the best school cadet in TEACHING AND HUMAN RESOURCE DEVELOPMENT IN HEALTH. As a member of the healthcare community, I have always considered dissemination of information and teaching of prime importance.. 1. During my first appointment as a demonstrator at the Faculty of Medicine, Colombo, I was involved in curriculum development activities under the direct supervision of the then Dean, Prof I am involved in the clinical training programmes of medical undergraduates and postgraduates on a regular basis. 3. I have been invited as a visiting lecturer to deliver lectures on development of communication skills, public speaking and public relations at the Faculty of Medicine, University of Colombo. Page 7

9 4. I have been extensively involved in the clinical teaching of nurses, nursing students, physiotherapists and other categories of hospital staff. In addition, I have been a visiting lecturer at the Nurses Training School, School of Physiotherapists and School of Cardiography affiliated with the National Hospital of Sri Lanka. Page 8

10 Contribution To Issues Of National Interest My Services To The Community TSUNAMI RELATED CONTRIBUTION The boxing day tsunami of 2004, resulted in unprecedented demands on the voluntary asnd healthcare sectors in Sri Lanka and other countries in the region. Acutely aware of my social responsibility at a time of national calamity, I took immediate steps to be involved in the rescue and relief activities. On request by the Commander of the Air-Force, within an hour of the disaster, I made a media statement in my capacity as the AS AN ACTIVE MEMBER OF OTHER ORGANIZATIONS... MEDIA AND JOURNALISM.. OTHER INTERESTS I have special interest in sports and have been a keen table tennis and badminton player since my school days. I also have interest in local and classical music as well as in the cinema. Teaching and social work are my hobbies. Page 9
