Knowledge, Competencies & Skills Essential Demonstrated ability to work as part of a multidisciplinary team. Demonstrated skills in area of speciality (aged care). Effective communication and interpersonal skills. Computer skills. Commitment to Continuous Quality Improvement Activities and an understanding of the National Standards Accreditation Process. Experience in development and review of Standards of Practice. Ability to translate evidence/research findings into best practice. Ability to deliver presentations and educational sessions. Desirable Previous experience working on an ACAT or in an aged care setting. Education, Qualifications & Special Training Essential Current registration with AHPRA Appropriate qualifications and minimum of 12 months experience in relevant clinical field. Work Experience Minimum 12 mths registration experience in specialty area. Experience in supervision and support of other staff and students in the work place setting. Undertaking ACAT assessments as per Aged Care Guidelines and WNSWLHD procedures and protocols. Contributing to the Aged Care Health services delivered by SVOS. Communication and networking with internal and external service partners. Provision and management of holistic multidisciplinary care in partnership with other health professionals. Provides a management, advocacy and co-ordination role to clients, actioning any problems or incidents that may occur. Reflects and recognizes professional competence identifying limitations and develop a personal development plan to remain updated and increase knowledge base. Committed to the personal development of other health professionals and students. Contributes to the ongoing review of clinical practice, protocols and policies. Recognises the value of research in contributing to developments in aged care, improved standards of care and has ability to contribute to, and utilise research. Contributes to the maintenance of a safe, clean environment for patients, staff and visitors. Ensure effective communication with all staff, clients and relatives/carers. Awareness of appropriate legislation and the impact on the role of the health professionals. Maintain standards of documentation. Initiate and Participate in Quality and Continuous Improvement activities. Demonstrate commitment to own personal development and ongoing education. All client care is provided in a case management, advocacy and coordination role. Constantly reviews personal development needs and actions accordingly. Displays a sound knowledge of specialty area. 5 JOBS DEMANDS CHECKLIST The purpose of this form is to manage the risk associated with the position in relation to the occupant. It may be used to provide information to a Health professional to perform a preemployment health assessment if required. Identification of possible risk can also assist with the development of a training plan for the occupant to ensure the risks are minimised. Frequency Definitions O = Occasional - activity exists up to 1/3 of the time when performing the task F = Frequent - activity exists between 1/3 and 2/3 of the time when performing the task C = Constant - activity exists for more than 2/3 of the time when performing the task R = Repetitive - activity involves repetitive movements N = Not Applicable - activity is not required to perform the job Demands Description Frequency Physical Demands Of Job Tasks Kneeling/Squatting Tasks involve flexion/bending at the knees and ankle, possibly at the waist in order to work at low levels. Leg/Foot Movement Tasks involve use of the leg and or foot to operate machinery Hand/Arm Movement Tasks involve use of hands/arms - eg stacking, reaching, typing, mopping, sweeping, sorting, inspecting. Bending/Twisting Tasks involve forward or backward bending or twisting at the waist. Standing Tasks involve standing in an upright position without moving about. Driving Tasks involve operating any motor powered vehicle Sitting Tasks involve remaining in a seated position during task performance. Reaching Tasks involve reaching overhead with arms raised above shoulder height or forward reaching with arms extended. Walking/Running Tasks involve walking or running on even surfaces. Walking/Running Tasks involve walking on uneven surfaces. Walking/Running Tasks involve walking up steep slopes. Walking/Running Tasks involve walking down steep slopes. Walking/Running Tasks involve walking whilst pushing/pulling objects. Climbing Tasks involve climbing up or down stairs, ladders, scaffolding, platforms, trees. Working at heights Tasks involve making use of ladders, foot stools, scaffolding, etc. anything where the person stands on an object other than the ground. Lifting/Carrying Tasks involve raising/lowering or moving objects from one level/position to another, usually holding an object within the hands/arms 1. Light lifting/carrying (0-9 Kg) 2. Heavy lifting/carrying (16 Kg and above) Restraining Tasks involve restraining patients/clients/others Pushing/Pulling Tasks involve pushing/pulling objects away from or towards the body. Also includes striking or jerking. Grasping Tasks involve gripping, holding, clasping with fingers or hands. Manual Dexterity Tasks involve fine finger movements - ie keyboard operation, writing. 6 Sensory Demands Of Job Tasks Sight Tasks involve use of eyes (sight) an as integral part of task performance - ie looking at screen/keyboard in computer operation, working in dark environment, interpreting x-ray film, etc. Hearing Tasks involve working in a noisy area - eg boiler room, workshop and/or operation of noisy machinery/equipment. Smell Tasks involve the use of the smell senses as an integral part of the task performance - eg working with chemicals. Taste Tasks involve use of taste as an integral part of task performance. Touch Tasks involve use of touch as integral part of task performance. Psychosocial Demands Tasks involve interacting with distressed people Tasks involve viewing/ handling deceased and/ or mutilated bodies Tasks involve interacting with people who as part of their lives may be aggressive, verbally or sexually uninhibited Exposure To Chemical Hazards Dust Tasks involve working with dust - eg sawdust Gases Tasks involve working with gases Fumes Tasks involve working with fumes - ie which may cause problems to health if inhaled.
