1 Internships - Student Assessment of Clinical Experiences Student Name: Aja Evertsen Facility: Health South in Tempe Clinical Instructors: Dan Angulo PT Please complete this form and provide a copy to your Clinical Instructor and submit a copy via WebCT. These evaluations are shared with the facility, future students, and clinical education faculty. Thank you. Purpose: To identify CI s who are valuable mentors. To identify developing CI s who may benefit from clinical faculty development through Clinical Education Training. To identify areas in which the Division should supplement or further develop the academic or clinical preparation. Instructions: In the blank following each item, please rate each item according to the following scale: 1- Strongly disagree, needs improvement 2- Disagree, would benefit from some improvement 3- Agree, good meets basic expectations, no improvement necessary 4- Strongly Agree, very good, exceeds expectations 5- Very Strongly Agree, excellent, far exceeds expectations FACILITY Orientation: The policies and procedures of the facility (dress code, hours, documentation, record keeping, tour, staff introductions, location of equipment/supplies) were made available to me upon arrival to the facility. 3 Comments: Orientation of the facility was remotely thorough. General location of equipment and supplies room was shown, then specific location of supplies were found as needed. Orientation: I was instructed on facility standards regarding emergency procedures (fire procedures, disaster plan, patient emergency, security, etc) 3 Comments: I was not oriented with this information in the beginning, yet emergency procedures where covered in a staff meeting 5-6 weeks into my experience. I had the opportunity to be exposed to a wide variety of diagnoses: 4 1
2 Comments: I feel that I had good exposure to many common pathologies and others that were not so common. I was also exposed to a wide variety of individuals. I was given an opportunity to observe and/or perform a wide variety of treatments. 4 Comments: I was able to perform many treatments that I have learned and many that I have not learned. I felt limited in this area due to my lack of knowledge of different treatments. I found it interesting how differently therapists treat their patients. I was given an opportunity to work with other health care professionals. 2 Comments: I was given the opportunity to work with other PT s, but did not have the opportunity to work with a professional outside the PT field. I was given the opportunity to supervise, delegate to and work with supportive personnel 4 Comments: The facility has one tech that helps that I have been able to supervise, delegate to and work with. I was given adequate time and opportunity to prepare for treatments (including chart review, equipment, setup, etc) and to perform quality treatments 4 Comments: The computer system allows for easy access to charts and previous notes. Equipment is easily accessable for set-up. The facility (workload, scheduling, administration, and staff) allowed time, supported and had a positive attitude of regard for clinical education and my experience for learning: 3 Comments: My scheduling was generally spaced out enough to allow for a positive atmosphere for learning. Generally the administration and staff had a positive attitude toward my education and experience for learning. I felt like an aide at times. I greatly appreciated the few times that the staff took the time to work with me and better my learning experience. The facility offered a notable quality and variety of exposure to the clinical world (stimulating clinical environment for patient care type/setting, exposure to professional development opportunities, exposure to appropriate professionalism, role modeling of staff, respect, and positive regard for all patients/families/staff/other health professionals ) 4 Comments: The staff created a professional environment and I was impressed many times of their ability to have positive regard for all. I was introduced to the importance of professional development opportunities through the region staff meetings. 2
3 The facility offered a notable variety of supplementary or unique learning experiences (i.e. exposure to other health professionals, exposure to various aspects of PT, opportunity to observe surgery, home evaluation, etc) 2 Comments: I was not exposed to various aspects of PT. I would have loved the opportunity to work with other health care professionals and especially surgeries, yet the opportunity did not present itself and I did not verbalize this particular interest. My experience was concentrated in the orthopedic clinic setting. I did have the opportunity of observe a portion of a neurological evaluation on a spastic individual by one of the therapists in the clinic. This was my only experience outside the orthopedic setting. Overall this facility offered me a positive clinical education experience 3 Comments: It took me longer than I had expected to feel comfortable with the facility and therapists. I appreciated the extra instruction and help that was given by the other therapists. The facility had adequate equipment for the needs of the clinic. CLINICAL INSTRUCTOR The CI reviewed my background, level of preparation and goals at the beginning of the internship and objectives and goals for the internship were mutually determined: 4 Comments: The CI and I sat down the first day and reviewed my goals for the internship. I also completed a weekly overview of the previous week in conjunction with goals for the next week. Many of the goals carried from week to week and others were easily attainable goals. The CI planned, selected and facilitated learning experiences and opportunities; modified learning experiences based upon my performance and abilities and sequenced learning experiences to allow progression towards my goals. 3 Comments: My CI sequenced experiences well by allowing me to perform more simple evaluations first of LE and then progressing to UE following with spine. I felt that there was a lack of feedback of my performance and how I was doing, along with how I may do something better. Good feedback concerning documentation was given. My CI selected learning experiences by allowing me to work with such pathologies such as adhesive capsulitis, ORIF of tibia/fibula, and radicular pain. The CI presented clear expectations for my performance at the beginning and throughout the internship: 3 Comments: My CI presented expectations for my performance in evaluations and the progression as soon as I felt comfortable. Due to my CI never having a 1 st year intern before, he didn t really know what his expectations were and therefore I feel that he hesitated to give feedback or helpful instruction. 3
4 The CI was accessible and regularly available for questions, feedback, discussion, consultation, observation and/or supervision: 3 Comments: He was available if needed, yet many times I would have to interrupt him with a patient and then I was possibly crossing the line of patient privacy. I was supervised from afar, yet did not receive constructive criticism to know how I was performing. We both tended to be busy with patients that didn t leave a lot of time for discussion and consultation. The level of supervision was appropriate to and adapted to my level of clinical experience throughout the internship 3 Comments: I made the assumption that I must have been performing to appropriate levels due to not requiring complete one on one supervision. I would have appreciated more instruction. My CI was interested in his/her role as a clinical supervisor (i.e. invested in the process, welcomed students, enthusiastic about sharing clinical expertise, etc.) 2 Comments: My CI has many years of great experience and has great knowledge of physical therapy. I would have liked to have the opportunity to learn more of his experiences and knowledge obtained over the years, but for whatever reason I was not able to work side by side with him due to limited opportunity throughout the eight weeks. The attitude of the CI toward patients fit my concept of professional and ethical behavior: 5 Comments: Dan is great with his patients. He has a calming way about him and cares about the well being of his patients and staff. He always presents himself in a professional manner. Dan also shows ethical behavior toward patients and staff. The CI encouraged me to think critically and to problem solve: 2 Comments: An occasional question was addressed to me about a specific problem, but most of my critical thinking was with progressing treatments as needed and coming up with treatment plans for new patients. I was able to critically think through modifications of exercises to meet the needs and demands of the patients. Dan really encouraged me to think about exercise and progression. I would have personally liked more of a challenge by Dan picking my brain and allowing my to test my knowledge by the reasoning behind why we as PT s do what we do. The CI encouraged me to take responsibility for and to take an active role in my own learning experiences: 4 4
5 Comments: For a few patients Dan would suggest that I look at resources available for possible and effective treatments for specific conditions. Indirectly he encouraged me to take responsibility by simply putting my out there to make decisions and treat patients without him breathing over my shoulder and I had to take the responsibility to treat the patients as if they were solely my responsibility. The CI gave me timely, useful, frequent and specific feedback when appropriate, and did so in an appropriate manner (not embarrassing). Time and space were considered and allowed for during this communication: 2 Comments: Aside from feedback on documentation I felt that little feedback was given, even after a specific request at midterms. Dan never embarrassed me and handled suggestions away from the patient, which was appreciated. Feedback given at midterm seemed non-personal and I wasn t quite sure if I was really doing what was expected or not. Evaluation of my performance by the CI was fair and objective; it included the identification of my strengths and areas that need improvement. I received feedback from my CI that was nonthreatening and constructive 4 Comments: I never felt threatened by criticism. Areas for improvement were mentioned and Dan was encouraging about time management and efficiency in reguards to the fact that I can t expect myself to be able to handle a huge patient load and multi-task patients perfectly. I appreciated his encouragement in that those skills will come with time and experience and I can t expect to learn these skills in a few weeks. My clinical performance evaluation process encouraged my reflection, self-assessment and input 4 Comments: The CPI allowed me to reflect on where I stand and my confidence in performing different tasks and skills. I seemed to take the approach of evaluating my weaknesses and what I need to work on, where Dan was a little more encouraging realizing that I have time to work on those skills and I should be proficient yet. The CI demonstrated good time management and organizational skills 5 Comments: Dan did a great job of managing time and organizing patients well. I never once saw him get uptight or high-strung about the pace of the clinic or having several patients at a time. He was always calm and relaxed and seemed to get everything done in a timely manner. I was impressed with his ability to manage his time. The CI was able to effectively and practically manage problems, challenges and conflicts that arose 4 5
6 Comments: Dan stayed calm with any problems or conflicts that arose and was able to handle the situation well. Dan does well with effectively managing problems. The CI solicited my communication and feedback: 3 Comments: Many times I felt that I was not communicating well or making myself understood. It took me quite a while to feel that I had good communication with my CI. The CI was sensitive to my individual learning style, experience and personality 1 Comments: I requested feedback and the experience of him sharing his experiences on several occasions and I didn t feel that he really made the effort. Every once in a while when I would sit in on an evaluation he would have me feel &/or explain what he was doing and why. This only happened a handful of times during the eight weeks. I felt that he didn t put a whole lot of effort into me. I was disappointed not to be able to learn more from him and his experiences, but when I would ask questions I didn t really get an answer, and he would move on to something else he was doing. The CI had effective verbal and non-verbal communication skills and was responsive to my communication 1 Comments: I didn t feel that Dan was responsive to my communication at all. I thought it would get better and I continued to try to modify my communication to adapt to him, but I never found a technique that worked. It was a struggle for me because I have never had difficulty communicating before. Every time I would ask him a question he would look at me like he didn t understand what I was saying. When I would ask if I confused him or if he didn t understand he would say no, I am not confused, and then not really answer my question. I could never figure him out and I was not impressed with his lack of effort to teach me. The CI demonstrated clinical expertise and competency in his/her area of PT practice 4 Comments: My CI was very competent and has much experience. I would have liked to work more side by side with him so I could learn more from him and the experience he has. Overall, I would say that my CI was an effective supervisor of this learning opportunity 1 Comments: Maybe my expectations were too high, but I don t think so. All of my other experiences working with therapists have been positive experiences and I have been able to adapt to the situations at hand. My best experience of the internship is when Dan was away on vacation for a week and I had the opportunity of working with two other therapists that were very helpful and explained techniques and took the time to teach me. I almost feel that I learned more about patient 6
7 care in that one week then I did in the seven others. Dan did teach me a lot about insurance, paper work, different meetings and the legistics of PT which was good things for me to learn. Additional Comments: I think my expectations of this internship 1, were too high, therefore my experience was a little disappointing. Date shared with Clinical Instructor June 27, 2003 ACADEMIC PREP How well did your feel that the academic curriculum prepared you for this clinical experience 5? Comments: I feel that the education that I have received has been more than adequate to prepare me for this clinical experience. I wish I could go back and take many of the courses again now that I have a better background. Estimate the average time in hours that you spent outside clinical hours in prep for the clinical experience: 40 What were the strengths of your academic prep for this internship? Knowledge of the staff and peers at the University of Utah and the staffs ability to instruct effectively. I have been amazed at the teaching ability and knowledge our staff has and what a great opportunity I have to learn from the best. I appreciate the staffs commitment to learning and teaching and the overall willingness to take the time to help use as students understand. I feel that the patient sessions really helped and also the sessions of discussion in groups in MS Eval. What were the areas of needed improvement of your academic prep for this internship? I little more instruction on expectations of the case study. The examples were helpful. More instruction on resources that may be needed, especially for those of us that are going out of state. CLINICAL EDUCATION PREP What were the strengths to prep you clinically for this 3 rd year internship? Musculoskeletal Evaluation, kinesiology, and physical agents were the most helpful, but I think most experiences of my education in the first year were a good preparation. I think having great instructors makes a big difference. What areas of improvement are needed for you for clinical prep for this 3 rd year internship? 7
8 I know there is restriction on amount of one on one time spent with each student, but it would have been helpful to know more about the amount of pressure and techniques of joint mobs with different conditions, but maybe that is to come. Please share any input with regard to the Director of Clinical Education in terms of your preparation for clinical education, communications relative to clinical education, and coordination of your clinical experiences. Any additional comments: Thank you for your time and energy to evaluate the clinical education experience from both preparation and experiential perspectives, your input is invaluable to the Division, clinical faculty and future student affiliations.
