1 Job Description for Rotating Internship at Optivet Length of post: 12 months Employment package: Salary to be confirmed Membership of RCVS 25 days holiday + bank holidays Minimum requirements: Membership of the Royal College of Veterinary Surgeons Excellent communication skills Good level of general small animal medical and surgical skills Confident and competent in small animal anaesthesia A high standard of written and spoken English is required. For non-native English speaking candidates, a qualification in English is required (eg IELTS (academic, minimum level of 7 no elements less than 6.5) or TOEFL (internet-based test, minimum score 93 no elements less than 23)). Hours worked: The nature of the post does not lend itself to specific working hours, however, they are expected to be 40 hours per week. The normal working hours of the clinic are 08:30 to 19:30 Monday to Friday and there is no out of hours commitment outside of night cover. While the intern will have a rota, the expectation is that if there is activity in the clinic and the intern is able to attend that they take every opportunity to attend the clinic to see cases regardless of their rota commitments to maximise the benefit of their internship. The more the intern puts in to this year the more they will get out. Whilst every effort will be made to allow time for study and journal clubs during the working week it is expected that the intern will do additional study/research/reading in their own time. CPD: CPD allowance is 5 days A further 10 days may be taken as unpaid leave should the intern wish to pursue other training opportunities or attend other conferences. Responsibilities of the intern: To support the team in the care of our in-patients. This will involve supporting both the vet team and the veterinary nursing team. The intern will be expected to contribute to all aspects of patient care.
2 Structure of internship: Rota: Week Monday Tuesday Wednesday Thursday Friday 1 Surgery Medicine Surgery/ Ophthalmology Ophthalmology Medicine (Dependent on if any surgeries) 2 Medicine Ophthalmology Ophthalmology Surgery/ Medicine Medicine (Dependent on if any surgeries) 3 Night Night Night Night Night Rota will be subject to change and will include a week of nights in rotation with the other interns/night team. This is expected to be a 1/3 and the hours will be 11pm-8am, Monday- Sunday. During nights, days will not be worked. A study afternoon will be decided with the intern on an individual basis. This will allow time for work on case studies, journal clubs, certificates etc Further study will be expected out of work hours. Clinical Outcomes Please see the attached appendix for clinical outcomes in each service. Please note that the extent the intern will be able to perform these procedures/diagnostics will be directed by the clinician in charge and that adequate levels of clinical progress will need to be displayed to complete the outcomes lists. Completion of the outcomes lists will be a collaboration between clinicians and the intern with both parties needing to work together to maximise the clinical achievements. Medicine- appendix A Ophthalmology- appendix B Surgery- appendix C Non-Clinical Outcomes The intern will be expected to achieve the following outcomes: Attend journal club o The intern will be expected to attend journal clubs when happening o Presentation at journal club will be expected and decided with the senior clinician for that service Present at grand rounds o Grand rounds will be take place monthly, the intern will be expected to present a topic or case study (1 per service). This will outline the case presentation, diagnostics performed, treatment, outcome and discussion of literature. o The intern will be expected to do this in a 1/3 pattern, or every 3 months. o Over the 12 months one case from each service will be presented with the fourth and final case being the intern s choice. The intern will be expected to perform two of the following during their internship: o Talk at a conference (national/international) in an area of their choice o Publish a case study or article in a peer reviewed journal
3 o Present at a CPD event for referring vets/vet nurses o Produce a non-peer reviewed article Certificate (CertAVP) o Interns enrolled or wanting to be enrolled on a certificate will be supported in their continued study Management Tree The interns line manager will be the practice manager. For holidays, time off, sickness and CPD all enquiries should be directed to HR. The internship programme director is Matthew Best, medicine clinician. For queries regarding the internship, contact Matt in the first instance. Matt is also the interns contact point for medicinebased queries, however, when not on clinics Kit Sturgess will be their point of contact. The ophthalmology point of contact for the intern is Rob Lowe. The surgery point of contact for the intern is Jerry O Riordan, note that he is not on-site day-today, so queries should be brought up when he is in clinics. Intern Integration The intern will be moved around services as necessary in times of need by the senior clinician of that service. The practice manager will also be responsible with the day-to-day movements of the intern. Assistance will be provided to the nurses and VCAs when needed however priority should be given to the service the intern is on at that time. Probation Period There will be a 3-month probationary period. During this period the internship may be terminated by either party with a one week notice period. After the probationary period is complete, the intern will undergo a probationary meeting with Rob Lowe and Matt Best to discuss strengths, weaknesses and points to improve. Each service will have given feedback prior to this meeting. Progress meetings The intern will undergo 3 monthly meetings going forward. These will be with Matt Best. The intern will be able to use these meetings to gain feedback from individual services, get help on talks/publications and provide feedback to the services about the intern s time spent with them. Feedback sheets should be completed prior to these meetings and copies are provided in Appendix D.
4 Appendix A Medicine Clinical Outcomes Core The intern will be expected to be confident in the following key skills by completion of the internship: Take full, relevant history from owners on all medicine cases Perform a full clinical examination of a cat/dog Doppler ultrasound and oscillometric blood pressure Perform cystocentesis Perform a basic abdominal ultrasound and be able to interpret the findings Perform a basic echocardiogram and be able to take basic measurements Administer chemotherapy Take a bone marrow aspirate FNAs of structures in an anesthetised patient Thoracocentesis Abdominocentesis ECG- acquisition and interpretation of basic electrical abnormalities Run a basic blood profile in the lab o PCV/TP o Haematology and biochemistry on the in-house analysers o CRP o Prepare and read blood smears o Coags o Blood gases Initiate and monitor blood transfusion Interpret blood results Run basic urinalysis Interpret urinalysis results Interpret results of CSF, histology samples and any other results from external laboratories Perform and interpret a neurological exam in cat and dog Prepare a dog for endoscopy and colonoscopy Perform an endoscopy and colonoscopy Urethral catheterisation Desirable If case presentation allows and the intern displays the relevant skills required, the following are goals that may be achieved in addition to the core outcomes: Pericardiocentesis CSF tap Frontal sinus trephination Core biopsies Gall bladder aspiration
5 Appendix B Ophthalmology Clinical Outcomes Core The intern will be expected to be confident in the following by completion of the internship: Taking full, relevant history from owners on all ophthalmology cases Full ophthalmology examination of a cat/dog Ocular ultrasound and image interpretation Routine tonometry Tear testing Corneal staining Direct and indirect ophthalmoscopy Fundoscopy Slit lamp biomicroscopy Electroretinography Gonioscopy Neuromuscular blockade of ophthalmic patients under anaesthesia Setup of operating microscope o On site microscope o Portable microscope Care of ophthalmic instrumentation o Ophthalmic instrumentation names and functions Ophthalmic photography Basic lesion identification Basic ophthalmology examination of a horse Desirable If case presentation allows and the intern displays the relevant skills required, the following are goals that may be achieved in addition to the core outcomes: Basic corneal surgery o Attendance of corneal cadaver surgical session Lid margin suturing Basic peri-ocular surgery o Attendance of cadaver surgical session Enucleation techniques
