Welcome, Introductions and Apologies, and Declarations of Interest, Quorum and Matters Arising Welcome, introductions and apologies were completed. Apologies were noted as above. Declaration of Interest/Conflict of Interest The Chair reviewed the subset of interests relevant to the committee membership and then invited members to declare any further interests relevant to the agenda. No new declarations were received. 2 Minutes of Patient and Public Engagement Meeting 7 th March 2017 and Action Log resulting with Matters Arising The group agreed they were an accurate record of the meeting held on the 7 th March Action Log - Review of Items not Completed Item 118 Better Births Programme Project has moved forward and the project managers are now looking at engagement events. Action: LP to ask Better Births Programme Manager to include Terri Walters from National Childbirth Trust in engagement. LP 26/05/17 Item 124 Equality Seminars Four Lunch & Learn sessions will be arranged for 2017/18 (planning first one to take place in June) to develop awareness amongst CCG staff of equality and diversity considerations. Opportunity for group members to take part. More information to follow. Action Closed 2

3 Item 126 New Carer s Representative. NB has forwarded contact details for Carers Advisor for Adult Social Care to LP. Action: SF to invite the carers advisor for Adult Social Care to join the PPE group as Carer s Representative. Action Closed Item 127 Signposting for Mental Health. Action: To be included in remit. Mental Health clinical commissioning team to coordinate. Action Closed 3 Updates from Stakeholders Godalming Dementia Action Alliance Surrey County Council has funded the Alzheimer s Society to help establish local Dementia Alliances and Dementia Friendly communities across Surrey. The Godalming Dementia Action Alliance is one of these. The launch event is scheduled for Wednesday 17 th May at the Wilfred Noyce Centre in Godalming from 6.00pm-9.00pm to raise awareness of the work and to establish a Dementia Friendly zone in Godalming. Dementia awareness week is from Sunday 14 th May through to Saturday 20 th May GJ commented that all Godalming leisure centre staff have received dementia awareness training. SJ advised that all GP practice staff have also been offered the training from the Alzheimer s Society. JC asked if the Government Disability Confidence Scheme was involved as it can support employers to employ people with disabilities : LM suggested that the Godalming Hub for SILC may be able to help promote the services. The Dementia Action Alliance is currently working very closely with Binscombe Medical Centre and The Mill Medical Practice. It was noted that neither practice yet has a Patient Participation Group Chair, which could be a useful contact for this project. SF LP 16/05/17 16/05/17 Action: SF to liaise with Binscombe Medical Centre and The Mill Surgery to discuss whether PPG Chair could be suggested. SF 16/05/17 3

4 Milford and Witley PPG Chair The surgery is trialling a new way of working whereby a doctor and nurse are available between 8.00am-8.30am when calls are coming into the surgery from patients. They will triage the patients as the calls come in. The trial will start mid-may. SJ commented that this is a good initiative; some patients will like the idea of being able to speak to a doctor on the phone and being triaged immediately. JG has joined the Royal Surrey County Hospital Patient Panel, which now has 3 PPG reps from the Guildford and Waverley area. Diocese of Guildford NB has been involved with the Dementia Action Alliance in Ash. Action: NB to send SF dates of the upcoming events so that SF can circulate to group. NB has created a leaflet called What s-on to cover local initiatives. Horsley Surgery liked it and their PPG has now created their own booklet called Living and Aging Well in Horsley, which has been delivered to all homes in the area. Action: NB to let LP know when the booklet is online. Governors of Royal Surrey County Hospital Three new non-executive directors have been appointed. Changes have also been made to the clinical structure. Christopher Tibbs, the current Medical Director will also be the Deputy Chief Executive. The A&E Department is being upgraded alongside the urology department. Changes have been made to the car park due to building work and the disabled bays have been moved. There are also issues with the new payment machines which are being addressed. Action: LP to Alf Turner for clarification to the car park changes, once received SF to circulate to group to confirm to their members for wider circulation. National Childbirth Trust TW explained how the loss of volunteers could lead to the potential closure of the local branch. The nearest branch if it did close would be Dorking. They are meeting tonight (02/05/17) to discuss options. Women will still be able to access NHS antenatal classes. There is concern about support after birth. Haslemere PPG Paula Head, Chief Executive is due to attend the Haslemere Health NB NB LP SP 16/05/17 27/06/17 31/05/17 04/07/17 4

5 Group Meeting on the 15 th May 2017 and the new Chair will attend the Haslemere League of Friends meeting on the 17 th May 2017.Volunteer car services are challenged. They would like to explore whether more outpatient clinics can be established in Haslemere. Shere PPG A shortage of volunteers is also affecting organisations in Shere. The pre-school group is closing as they do not have any volunteers to sit on the organising committee. Shere First School is in negotiation to take over the group and site it at the school. Holding a public meeting tonight (02/05/17) to advise relatives on mental health issues affecting young people and the support available. Plans have been submitted, with support from parish councils, for a generator to support the practice in case of power failure. Surrey Independent Living Council LM gave a general update on hubs in Redhill and Epsom. Volunteering SILC has a list of people wanting to volunteer some of whom have moved into other volunteering positions or employment. Advised that the national volunteering centre is a helpful source of advice: Catalyst A buddy system is established with Voluntary Action South West Surrey (VASWS) which is progressing well. FT confirmed that funding for the Safe Haven has been agreed from 2017 to Aldershot, Woking and Guildford are recruiting team leaders. Catalyst has appointed a Communications Assistant who will be responsible for updating the website. From 1 st April 2017 Catalyst is funded for two years to support work with High Impact Drinkers. This is a Surrey-wide Service. An evaluation will take place at the end of the first year. A team will be based in Woking. Voluntary Action South West Surrey Volunteers week takes place during the first week of June VASWS is promoting this on the High Street and at Farmers Market in Guildford.CD volunteered to promote any literature other group members have regarding volunteer opportunities. 5

6 VASWS is supporting The Great Get Together (street parties in commemoration of Jo Cox) with other businesses in Chapel Street, Guildford. Buddy project as mentioned by FT is going well. Godalming Leisure Centre They are working towards all Leisure Centres in Waverley becoming Dementia Friendly. Rehabilitation groups already available for those recovering from stroke and heart attacks. Health walks are also available. Information on all services is available at the leisure centres. Healthwatch Surrey JD provided the update below: Amplifying the voice of Care Home Residents: Healthwatch Surrey's report My Way Every Day ay_-_a_look_at_activities_across_25_care_homes_in_surrey.pdf was published summarising the results of 25 visits to Care Homes which led us to make a number of recommendations. These were shared with commissioners, providers, the regulator and Skills for Care. Healthwatch Surrey have asked for formal responses which will be collated as part of an Impact Report to be published later on in the year. The findings were used in a Health and Wellbeing Board workshop which took an in-depth look at care homes and domiciliary care in Surrey. Early intervention in Mental Health: The organisation visited Surrey s seven Safe Haven cafes, talking in depth to 25 users and published a report Keeping the light on. ens_a4_pdf._2.pdf ey_health_report_ _final.pdf Making it easier to make a GP Appointment: 120 participants from seldom heard groups took part in the project, talking about their patient journey when accessing their GP. Common frustrations were identified such as waiting on phone, getting an appointment quickly enough, parking, intrusive receptionists, and continuity of care for long term conditions. Common themes for the groups we spoke to included: translation and language support, specialist care, continuity of care, PPGs, 6

7 accessibility both in terms of physical and communication. The report is currently being written up with recommendations based on the findings. Due to Purdah the report will be available in June 2017 (rather than May). Improving the experience of Hospital Discharge: In March 2017, Healthwatch Surrey contributed to live and pre-recorded case studies to the recent launch event of the KSSAHN Safe Discharge and Transfers project, to help participants reflect on the impact of poor discharge on patients, their families and carers. Healthwatch Surrey volunteers Patient Leaders are also involved in the project. Healthwatch are in the early stages of developing a project with Epsom Hospital to look at how the new Epsom at Home service including the Epsom Community Assessment and Diagnostics Unit (CADU) (designed to reduce hospital admissions of elderly patients) and accelerated discharge processes are working. A summary report on what we have learned so far on hospital discharge will, again due to Purdah, be published in June 2017 (rather than May). 4 Presentation by Children & Family Health Surrey CK provided an overview of the new integrated Children s service, a Surrey Wide Service provided by a partnership of organisations: CSH Surrey, First Community Health and Care and Surrey and Borders Partnership NHS Foundation Trust. Action: SF to circulate the slides of Clare s presentation to the group. More information is available on their website: SF 26/05/17 JC asked Clare how easy it would be for patients and families to access the system to receive the appropriate care. How do families gain access the system? CK advised that referrals can be made by a range of health care professionals including school nurses and health visitors. Details are on their website. TW asked whether antenatal visits will be provided throughout Surrey. CK advised that the health visitors should be providing this service across the county. The aim is to have the antenatal appointment at 28 weeks for each new mum in a format that will work for them. 7

8 PB suggested that we invite CK or her colleagues back in 6-9months. Action: SF to set diary date to make arrangements for CK or another representative from Children & Family Health Surrey to attend PPE meeting in 6-9months. SF 24/10/17 5 Service Change Updates Sexual Health From 1 st April 2017, Central and North West London NHS Foundation Trust (CNWL) began the delivery of sexual health and HIV services in Surrey. For more information please refer to the Healthy Surrey website:- Phone: CNWL on (staffed Monday to Friday, 9am to 5pm) sexualhealth.cnwl@nhs.net Visit: Non-Emergency Patient Transport Services (NEPTS) From 1 st April 2017 South Central Ambulance Service have been awarded the contract for Non-Emergency Patient Transport Services. Transport bookings can be made online 24/7 through the (Non- Emergency Patient Transport Service) NEPTS Patient Zone (see the Managing your NEPTS booking section), or by calling the Central Booking Line on between 08:00 and 18:00, Monday to Friday. or by going direct to:- Leaflet-Mar-2017.pdf If you have any questions about the Surrey (Non-Emergency Transport Service) NEPTS contract, please at the following address: PTSSurreyQ&A@scas.nhs.uk Update on clinical commissioning GP Advice Services run by the Citizens Advice Bureau has been funded for a further year from April 2017 following a successful pilot. The CCG is taking part in the NHS National Diabetes Prevention Programme, a joint initiative between NHS England, Public Health England and Diabetes UK, which aims to deliver at a 8

9 large scale services for people already identified with nondiabetic hyperglycaemia, and who are therefore at high risk of developing Type 2 diabetes. 13 group based sessions, spread across a minimum of 9 months; with approximately 15 participants per session. Referred participants will set and achieve goals and make positive changes to their lifestyle through weight loss, improved diet and increased levels of physical activity. Sessions will be delivered predominantly in groups and will be face-to-face unless there is a strong rationale for an alternative approach. Following procurement by NHS England for the Kent, Surrey and Sussex areas, the successful bidder appointed for this area was Ingeus. The service will launch at the beginning of May Venues and course educators have been arranged in both Guildford and Waverley locations. Venues for the course include Guildford Library; Park Barn Centre, Guildford; The Wilfred Noyce Centre, Godalming; Cranleigh Medical Practice and The Villages Medical Centre, Send. 6 Stroke Consultation Update Consultation period ended at midnight on the 30 th April The NHS Transformation Unit in Manchester has been appointed to carry out the analysis of all feedback. Their report will be ready mid- June and a public meeting of the two CCGs will be convened early July to consider the report and make decisions regarding next steps. 7 Quality Updates Helen Collins, Associate Director for Quality and Improvement, updated PPE Group members on performance for the following three organisations: Royal Surrey County Hospital NHS Foundation Trust A&E department is flowing much better and waiting times have improved. Surrey & Borders Partnership NHS Foundation Trust A deep dive review of suicides and unexpected deaths has been carried out. Awaiting an Annual Suicide Report from the organisation. A contract notice has been issued regarding long waiting times for certain services in the CAMHS contract. South East Coast Ambulance Service NHS Foundation Trust assurance has been provided that work on targeting response times in Guildford and Waverley Area is being carried out. 9

10 Action: Members were encouraged to promote Community First Responders. The CCG is working with GPs and Providers to raise awareness of the project. Contact Sam Farr if you require more information. 8 QIPP (Quality, Innovation, Productivity and Prevention) GR explained the work that has been carried out so far to improve care for patients with inflammatory bowel disease.gr then outlined other services that the CCG is investigating for improvements in care and productivity:- Childhood hearing loss surgical treatment is 25% higher than comparator CCGs. Rhinosinusitis Medical and surgical intervention is higher than comparator CCGs as is re-admittance following surgery. Gallstones longer length of stay following surgery than would be expected. Reflux higher prevalence of surgical intervention than anticipated. Hernias higher rate of surgical intervention than compartor CCGs. Carpal tunnel syndrome again, a higher rate of surgical intervention than comparator CCGs GR asked the group whether the CCG should be considering other conditions. It was suggested that babies be included in the Reflux data, and also pregnant women can be prone to Carpel Tunnel during their pregnancy but the condition does normally disappear. GR was invited to attend the PPE group in future to provide an update on progress against these QIPP projects. Action: SF to table QIPP on the agenda for PPE group May Action: SF to circulate copy of slides to group.
