1 ADN RN applicants LVN-RN students who have failed or withdrawn once from any other RN program may apply for admission to the program. If applicant meets the requirements is selected can only be admitted one time to the West Hills College Lemoore ADN program. ADN RN applicants LVN-RN students who have failed or withdrawn twice from any other RN Program(s) will not be considered for admission to the ADN Program 1. Grade point average (GPA) in relevant coursework completed by applicant. Maximum points =50 Coursework Required Documentation to Submit with your application 1 1a. Cumulative GPA 20 points - Minimum 2.5 GPA in cumulative coursework GPA / 4 x.2 x 100 = points 1b. Grade point average in prerequisite required courses 30 points - Minimum 2.5 GPA in anatomy, physiology, microbiology, English (anatomy, physiology, microbiology, English) Prerequisite GPA / 4 x.3 x 100= points. 1

2 2. Repeated classes in any science prerequisite coursework. Repeats include all grades NC, No Pass, or C- C grade or better is considered passing in all coursework. For serious compelling reasons; applicants may request to provide personal statement of explanation for application to be considered for review with approval from the Nursing Director. 2

3 3. Completion of all GE requirements for graduation (Int Algebra or higher; General Chemistry: Nutrition; Intro to Psychology; Intro to Sociology; Oral Communication; Humanities Elective. Maximum points= 10 Completion of GE requirements 3 3a. All GE courses completed 10 points Missing Math or Chemistry: -5 points each Required Documentation to Submit with your application Missing other GE: psychology, sociology, humanities, speech or nutrition -2 points each 3

4 4. Work or volunteer experience in health care by applicant. Maximum points =10 Work Experience or Volunteer Experience 4 4a. Healthcare active certification/license with direct patient care Certified Nurse Assistant Medical Assistant Physical Therapy Aide Licensed Vocational Nurse Psychiatric Technician Veterinarian Technician Emergency Medical Technician Surgical Technician Phlebotomist Other upon review 4b. Healthcare active certification/license with no direct patient care experience Required Documentation to Submit with your application 10 points - Copy of front back of certification/license - Work or Volunteer Experience in Healthcare Verification place of employment, type of facility, job duties. Direct patient care is identified as hs on experience when caring for patients or clients. Vital signs, bathing, assessment, toileting, ambulating/mobility, providing treatments, assisting with procedures, administering medications. 7 points - Copy of front back of certification/license - Work or Volunteer Experience in Healthcare Verification place of employment, type of facility, job duties. 4

5 4c. Direct patient care experience in a health care setting with no license/certification or inactive or expired certification/licensure 4d. Volunteer: direct patient care in healthcare setting >100 hours 4e. Volunteer: non-direct patient care in healthcare setting, >100 hours 5 points - Work or Volunteer Experience in Healthcare Verification place of employment, type of facility, job duties. 3 points - Work or Volunteer Experience in Healthcare Verification - Letter from employer(s)/volunteer agency on company letterhead verifying place of employment, type of facility, job duties.
