2 Development Portfolio 2 What is a Clinical Nurse Leader? As the healthcare system continues to evolve with improving ones quality of life, a clinical nurse leader (CNL) entails an umbrella of factors. These factors direct one focus on the quality of care on an individual and community based perspective. Throughout the eyes of the CNL he/she will be equipped to assess a population at the microsystem level with a theoretical and evidence based practice approach. The focus of care is within a holistic realm that enables the clinician and the patient to collaborate with effective behavior changed models to improve health outcomes and alleviate pain. A CNL practices as an advocate to improve an individual s healthcare concerns and issues related to the nurse s profession with the importance of autonomy with a focus on the use of health policy attributes. Information gathered amongst the microsystem level will enable the CNL to improve team management with technology use, which will enhance the healthcare system within a cost-effective approach. A CNL is tailored to coordinate a design program which will lead to a request of grants to help increase health promotion programs within a systematic analysis with a prospective for the benefit of the environment. Lastly, a CNL is capable to create an environment of safety for fellow peers, as they continue to explore their roles and which allows them to network among new relationships and improve ways of working together to implement health promotion strategies. What is my personal Philosophy? My focal point of nursing is caring with a distance and delivering therapeutic remedies that will enhance a patient and the family s quality of life. With the goal to alleviate pain, restore physiological systems, and provide education. As a clinical nurse leader my involvement of the professional role is shared governance with evidence based research practice of standards of care and outcomes. I will continue to contribute to being a lifelong learner accountable for practice and participation in interdisciplinary activities. Alter patient behaviors with theory based models that are beneficial to lessen the health risk and further complications. I will direct patient centered care from a cultural sensitive approach with knowledge based on target population beliefs. I will grant the ability to always approach patients and fellow peers with an interpersonal skill and an outlook on leadership. Furthermore, I will be the guiding light and the force to provide unconditional care.

Reflection journal of weekly events during clinical rotation enabled retrospective factors that could have streamlined assessment in relation to nursing diagnosis. Communication a. Demonstrate communication skills during assessment, intervention, evaluation, and teaching b. Express oneself effectively using a variety of media in a variety of contexts c. Establish and maintain effective working relationships within an interdisciplinary team d. Communicate confidently and effectively with other health care workers both in collegial and subordinate positions e. Produce clear, accurate, and relevant writing. N254A/B Theoretical Foundations helped with experience to teach fellow peers the importance of basic skills during fundamental lab session Completion of classes centered around collaborative and team building ideas with the use of power point presentations, APA style reports, and video presentations Utilized effective situation, background, assessment and recommendation (SBAR) to translate patient s reports to clinical preceptor and peers during clinical internship. N252 Health Promotion 2012 consisted of a 25 minute power point amongst a collaborative group setting regarding implications on Type 2 Diabetes within compromised communities N260 Secondary Health Promotion enabled the use of creating an American Psychology Association (APA) clinical case with essential data on evidence based practice related to the following screening preventions: Hyperlipidemia, Type II DM, Hypertension, and Osteoporosis

Provide effective health teaching using appropriate materials, technologies, resources, and formats b. Evaluate effectiveness of health teaching by self and others Health promotion and risk reduction and Disease Illness and Disease Management 2013 Thai health fair demonstrated effective communication regarding education on hypertension prevention amongst individuals within the community Care Harbor Tools and resources produced from PubMed, Sin-hal and U.S Prevention Task Force (USPTF) Experience with care-connect during clinical internship to access and evaluate patient s medical records. N252 Health Promotion, N250 Ethics consist of completed group assignments which resulted to work in a timely manner while facilitating and delegating task collaborate (USPTF) screening tools for prevalent diagnosis related to diabetes, hypertension, osteoporosis and hyperlipidemia with the use of an I-pod during clinical rounds Health Belief Model (HBM) to measure risk and adherence of 50+ African American women regarding breast cancer screenings. N260 Secondary Prevention to promote wellness amongst the elderly with evidence based practice Informed and educated patients on the benefits of medications and adverse effects in the hospital settings. Monitor lab work for abnormal findings during clinical rotation in relation to pathophysiological mechanism Information and Health Care Technologies Care connect has been used within the clinical setting to assess patients record, labs and pre-admission data.

Articulate the significance of the CNL and other nursing roles to policy makers, health care d. Incorporate knowledge of cost factors in delivering care N204 Research Design and Critique, Biomedical library search engines for clinical research pertaining to reduced caloric intake and effects on aging N250 Portrays professional and valuable behaviors amongst colleagues, clinical staff and patients Focus on cultural diversity and approach with sensitivity and a holistic nursing care Approach individuals needs based on cultural and spiritual perspective in the clinical setting and community during clinical internship and volunteerism opportunities.
