1 MEMORIAL MEDICAL CENTER HUMAN RESOURCES POLICY MANUAL Policy No. 5-7 SECTION: EMPLOYEE RELATIONS PAGE: 1 of 6 SUBJECT: MMC IMAGE DATE: 08/27/98; 11/1/00; 7/26/02; 7/01/03; 5/01/04; 9/01/04; 4/01/06; 9/01/06; 11/01/09, 7/10/10; 3/1/11 APPROVED: REVISED 7/14/14 Chief Executive Officer EFFECTIVE: 8/1/14 OUTCOME STANDARD We will view ourselves through the eyes of our guests. All team members at MMC must present an appearance, which is clean, neat and appropriate for a professional health care environment. PROCESS STANDARD A. All MMC departments/units at all locations are covered, including contract affiliates. Directors/Managers are authorized to determine additional requirements required by department/unit specific standards or safety and/or infection prevention purposes. B. Team members are expected to comply with this MMC Image policy at all times when performing their job responsibilities. Team members attending department/unit educational programs/meetings sponsored by the Medical Center, or representing the Medical Center at off-site programs, are expected to adhere to the MMC Image. 1. The Medical Center reserves the right to determine appropriate style or colors. 2. No one is authorized to leave the premises while wearing a hospital-owned garment. 3. Only team members providing patient care wear scrubs, scrub jackets, or uniforms. Team members who do not provide patient care wear business attire unless otherwise specified in the department specific section of this policy. C. General MMC Image Requirements 1. Clothing worn by team members must be neat, clean, in good repair, non-revealing, not faded and of the appropriate size. 2. Sleeveless garments and shorts are not part of the MMC Image. 3. Clothing with writing or graphic messages is not appropriate. 4. Skirts, dresses and slacks must be a professional length and not more than three inches above the knee. The top of a skirt slit should also not be more than three inches above the knee.
2 SUBJECT: MMC IMAGE PAGE: 2 of 6 5. Slacks/pants - tailored dress slacks or pants. Professional dress capris must be longer than the knee. Clinging, ankle hugging, calf hugging slacks, leggings, and jeans are not part of the MMC image. 6. Dress shirts, polo shirts, ties, blouses and MMC sponsored clothing are part of the MMC image. Team Members may wear MMC sponsored tee shirts as allowed by the Director and HR during specified times of the year only. Tops, sweaters, blouses must be long enough to cover midriff. 7. Fabrics - spandex, chiffon, metallic, leather, sheer or clinging fabrics are not part of the MMC image. 8. Recreational clothing team members may not wear sweatshirts, tank tops or shorts while at work. 9. Shoes must be clean, polished and kept in good repair, rubber-soled with laces tied for safety. Heels that are elevated should be appropriate for the work setting and no more than three inches for safety reasons. Shoes worn in clinical areas will be closed toe and made out of fluid resistant materials. Non-patient care areas may wear Peep Toe shoes as allowed by the Director. Peep toe shoes show less than half of the toes. Certain departments/units may require specific footwear or shoe covers due to the nature of the job. Novelty shoes and sandals are not part of the MMC Image. 10. Hosiery/socks must be worn. Hosiery must be of a single color to coordinate with clothing with the tops of hosiery not visible. If socks are worn with slacks or pants, coordinating colors should be selected. Socks worn with skirts are not part of the MMC Image. 11. Team members may only wear hats/bandanas when they are part of the uniform specified by the department/unit. 12. Jewelry team members may wear two rings and/or ring sets on each hand unless contra-indicated for infection prevention or safety reasons. No more than two necklaces/bracelets/pins, no more than 2 earrings per ear and no gauge piercings (must have skin colored inserts in ear holes) as they are distracting and not part of the MMC Image. Bolo ties are acceptable. Team members who provide patient care should not wear earrings that dangle or hang low. 13. Body piercing/facial jewelry- no visible body piercing, (including tongue piercing) or ornamentation is allowed other than earrings. 14. Team members are expected to make every possible effort to cover visible tattoos, branding, body modifications and hickies in a professional manner.
3 SUBJECT: MMC IMAGE PAGE: 3 of I.D. badges the wearing of an identification badge is a condition of employment and/or association with MMC. Badges are worn above the waist and not on the sleeve. Badges are worn in an upright visible position at all times. Only two MMC approved pins may be worn on the badge holder clip. 16. Hair a neat, natural hairstyle is an essential part of a well-groomed appearance. Team members should select conservative styles that will not fall forward or over the face and eyes while performing job duties. Teasing for body or shape is kept to a minimum. Extremes in hairstyles such as cutouts or Mohawks or unnatural color are not part of the MMC Image. Team members who provide patient care must tie or contain hair away from the face. Appropriate hair confinement is worn in areas as indicated by safety or infection control requirements. 17. Make-up worn by team members should be complimentary to natural features and create a fresh, natural appearance. Excessive make-up is not part of the MMC Image. 18. Fingernails- must be kept clean and well groomed and do not exceed one-fourth of an inch beyond the fingertip. Team members who have direct patient contact or persons who routinely handle medications, supplies or other patient care items will have natural nails only. These team members may only wear clear or pale flesh tone or pale pink colors of nail polish. Polish must be free of chips or cracks. Multicolored nails, nail ornamentation or extremes in color are not considered part of the MMC Image. Team members who scrub in for procedures may not wear nail polish. 19. Perfume/deodorant due to close contact with others, deodorant or antiperspirant must be worn and team members should not wear colognes, perfumes and scented toiletries. MMC promotes a Fragrance Free environment for the safety of our patients and team members. 20. Sunglasses team members may not wear sunglasses within the Medical Center. 21. Team members may wear shirts or buttons that recognize nationally celebrated health care disciplines in conjunction with the designated time frame. Any other clothing, accessories or buttons, etc. must be previously approved. All other attire must meet MMC Image expectations. 22. Halloween costumes are not part of the MMC Image. Team members may accent attire with holiday scrub tops, warm-up jackets, socks, sweaters or jewelry that meets overall MMC Image requirements.
4 SUBJECT: MMC IMAGE PAGE: 4 of Licensed and unlicensed team members providing patient care should wear scrubs or uniforms in any coordinating combination except those colors designated to a specific department. Acceptable print tops do not include cartoon characters (except as designated in Maternal/Child), camouflage or denim patterns. D. Department Specific Dress Code 1. Engineering a. Team members should wear white/navy pin stripe work shirts with navy pants and appropriate footwear. b. Team members may wear coveralls when working in dirty conditions. c. Painters should wear white coveralls. 2. Environmental Services a. Environmental Service Attendants should wear uniforms (patti tunic in coal pincord with coordinating slacks/pants). b. Supervisors should wear white dress shirts with a silver tie, a black vest and slate gray slacks/pants or skirt. 3. Food and Nutrition Services a. Team members should wear black slip resistant shoes and black, white and/or red polo shirts, and black pants/slacks. b. Directors/Managers/Supervisors and dietitians should wear business attire. c. Cooks may wear cook s white chef coats or the department uniform. 4. Home Health a. Tailored shorts (which reach the knee) may be worn with an appropriate top during the summer months as patients frequently do not have or do not use their air conditioners. 5. Laundry a. Team Members should change into clean MMC provided scrub attire before starting their shift and change back into street clothes at the end of their shift.

Phlebotomy team members should wear Caribbean blue or black colored scrubs and may choose to wear a Caribbean blue, black or coordinating print top with Caribbean blue or black. Acceptable print tops do not include cartoon characters, camouflage or denim patterns. b. Technical team members must meet the general MMC Image requirements or may wear Caribbean blue or black scrubs. 7. Material Management a. Team members issuing and stocking in the Warehouse area should wear dark colored slacks or pants and a navy or dark blue shirt. b. Team members working in the Warehouse area and handling Warehouse items must wear steel-toed shoes. 8. Admissions a. Black skirts or pants per department specific guidelines. b. Blouse/shirts are color coordinated, long-sleeved, button-down with collar per department specific guidelines. c. Team members may wear jackets per department specific guidelines. 9. Security/Guest Relations Representatives a. Team members will wear the selected tan colored shirts and olive colored dress tactical pants. Only approved jackets and sweaters will be worn as approved by department management. 10. Nursing Team Members a. Licensed Nurses in all areas EXCEPT restricted and semi-restricted areas should wear scrubs or uniforms in any combination of white or other color. The color scheme should be coordinated and professional in appearance. Acceptable print tops do not include cartoon characters (except as designated in Maternal/Child), camouflage or denim patterns. Team members may also wear a white lab coat over their scrubs. b. Licensed Nurses on Pediatrics and those providing care to Pediatric patients in the Ambulatory Surgery Unit, should wear the designated attire for RNs at MMC or may choose to wear a cartoon character print top that coordinates with the scrub bottoms.
6 SUBJECT: MMC IMAGE PAGE: 6 of 6 c. The Administrator on Duty (A.O.D.) wears business attire or RN designated attire. 11. Restricted and Semi-Restricted Areas (OR, Ambulatory Surgery, Central Sterile, Cardiovascular Lab, LDR, Neonatal, Laundry and Interventional Radiology). a. All team members and individuals that enter the semi-restricted and restricted areas must wear scrub attire intended for use only within these areas. 12. Behavioral Health a. Team Members in Behavioral Health will not wear black or red scrubs. 13. Emergency Room a. Team members will wear solid colored scrubs with Emergency printed down the pant leg, Memorial Medical Center printed on the arm sleeve, and the MMC logo with their name and title embroidered on the upper left chest pocket area. The color scheme is not title specific nor is it department specific but the colors should be coordinated and appropriate. b. Variations in scrub attire can be according to special events and holidays as described above. Other variations are appropriate if the team member s scrubs are soiled while on the job. In that case, MMC scrub attire may be worn and returned to MMC.
