The Department shall determine continued facility compliance with the nurse aide training program requirements through on-site reviews and their trainees competency examination performance 3

10 Improving Your Prometric testing The actual clinical skills portion is one that the Primary Instructor will likely never see. Spend time talking with your CNA s after to see how you can make your mock test more like the real one Talk to the evaluator 10

11 Withdrawal of Approval Failure to comply with all federal and state regulations or requirements pertaining to the training and/or competency testing of nursing home nurse aides may result in the withdrawal of approval for the nurse aide training program. 11

12 Withdrawal continued Pursuant to federal statute, the nurse aide training and/or testing may not be provided in any nursing home that, within the previous 24 consecutive months, has been subject to any of the following surveillance or enforcement occurrences. Extended or partial extended survey; Denial of payment for new admissions; Denial of payment for all individuals; Assessed civil money penalty of $5,000 or greater; or Imposition of a temporary manager. 12

13 If a Program is in progress If a state-approved nurse aide training program is in progress in the nursing home when the 24-month ban is imposed, the training program shall contact the Department immediately to develop a plan to complete the NATP as expeditiously as possible without penalizing the trainees. In some rare situations a waiver could be applied for, but it is not likely. 13

14 Training Records The facility must maintain training records for each individual completing the NATP, including a copy of their completed skills checklist, and performance evaluation records. Training records must be maintained and made available to Department staff upon request. 14

17 ADA packet A completed Accommodation Request Packet includes the Testing Application, Candidate Accommodation Request Form, the Professional Evaluation Form and any additional verification required. A complete packet will allow Prometric to assist the candidate in arranging the best accommodation possible for the situation. They are unable to process incomplete Accommodation Requests. 17

18 ADA continued You MUST either have an appropriate professional (an internist, for example, is not appropriate to diagnose a mental disorder or reading disability) complete the Professional Evaluation Form OR provide existing documentation from the person(s) who granted you the same or similar accommodation you are now requesting in another formal testing environment (the Professional Evaluation Form must still be returned with the appropriate box marked). 18

19 ADA continued Any documentation provided in support of the processing of your application, shall not be dated back more than one (1) year from your accommodation request date. This means they will likely have to get a new one 19

Prometric is unable to accommodate a request for unlimited time. If extra time is needed, please specify the amount. 21

22 ADA accommodations An oral administration of the written test is available. administered by registered professional nurses through the state s nurse aide testing and registry contractor. DOH has contracted with Prometric for nurse aide certification or competency testing and management of the NYS Nursing Home Nurse Aide Registry 22

23 Not passing Individuals that are not successful in passing either the clinical skills test or the written/oral test shall receive an official score report that includes diagnostic information by knowledge or content area They will only need to repeat the portion that they missed (either written or clinical), but the whole section 23

24 Reasons for not passing Recording-They may have covered up the recording paper Pulse or respirations different from acceptable variation Too numerous small mistakes Large safety error Wrong side of the actor/mannequin s body 24

25 3 Attempts If the individual has not passed the certification examination within the three attempts and/or within 120 days of their first day of training or employment, the individual may no longer work as a nurse aide trainee in the nursing home. The facility may assign the individual to nonresident contact duties. 25

26 Training and Passing Well-trained students make small mistakes Poorly trained students make big mistakes 26

27 Re-testing If you did not pass an exam, you must re-register in order to retest. To re-register, you must complete another application or you can apply online at after you receive your official score report. If you will be re-testing at a Regional Exam Site, send the application to Prometric along with the appropriate exam fees. If you are re-testing through a nursing home or training program, give the application to the person who scheduled your initial exam. 27

28 Re-tests Remember you must have 4 to schedule a test Try not to let too much time elapse If you wait for another class, you may want to do refresher 28

29 Re-training If you applied through Routes 1 or 7 and do not pass both exams within the required timeframe, you must retrain through a different NYS-approved nurse aide training program before reapplying. 29

30 Lost Certificates You may request a duplicate NYS Nursing Home Nurse Aide Certificate by completing the Service Request Form available online at There is a $15 fee for the duplicate certificate. Keep copies of certificates 30

31 Re-certifying CNA;s NYS Nursing Home Nurse Aide Certificate is valid for 24 months. It will expire two years from the last day of the month in which you were certified. You will be mailed a reminder notice approximately 45 days before your certification expiration. The notice will go to your home address currently listed on the Registry To be eligible to recertify, you must have worked for pay as a nurse aide in a NYS nursing home or at a NYS-approved nurse aide employer for at least seven hours within the previous 24-month period. 31

32 Name or Address Change It is your responsibility to keep your information correct and current with the NYS Nursing Home Nurse Aide Registry. Address Change If your address changes after you have been certified, you must send a completed Service Request form to Prometric. The form is available online at There is no charge for updating your address with the NAR. Name Change If your name changes after you have been certified as a NYS Nurse Aide, you must send legal documentation of the name change. Mail your legal documentation and Service Request form to Prometric. The form is available online at Acceptable forms of legal documentation include a copy of your marriage certificate, divorce decree or legal name change decree. 32
