1 May 2014 TITLE: RESPONSIBLE TO: LOCATION: FUNCTIONAL RELATIONSHIPS: VISION HEARING TECHNICIAN Team Leader, Child and Family Waitemata DHB sites as required Parents/Caregivers Public Health Nurses Ear Nurses Medical Officers Schools/Pre Schools General Practitioners Optometrists/ Ophthalmologists ENT consultants PRIMARY OBJECTIVES: To provide a vision and hearing screening service for children in the Waitemata District Health Board area for early detection and referral of vision, hearing and colour vision defects according to National guidelines and protocols. 2 KEY TASKS Screening Screening tests of vision, audiometry and tympanometry are carried out for children according to National Protocols Hearing and vision tests are carried out as part of the Well Child/Tamariki Ora B4 School checks framework New entrants who have not received the B4 School Check and children new to New Zealand are screened for vision and hearing defects at schools Youth at alternative education centres and year 9 students in low decile colleges are screened for vision and hearing defects Vision screening tests are carried out for all year 7 children Colour vision testing is carried out for all year 7 boys. EXPECTED OUTCOMES National guidelines for screening tests are adhered to. The parents of all eligible children receive information allowing them to give informed consent All children with written parental consent receive screening as appropriate, with effective follow up and referral to appropriate agencies of suspected defects. For all children tested: Screening results and referral recommendations are communicated to parents/caregivers in writing, with contact information given if more detailed information is required. B4SC targets will be met (i.e. 90% of 4 year old cohort will be tested) in conjunction with B4SC contract holder (Plunket). This may involve coclinics or home visiting for hard to reach children. 3 Administration Maintenance of accurate records and documentation. Test results are recorded at the time of testing. using recommended data collection forms Accurate daily recording of relevant data documentation (including referral letters) completed within 24 hours. B4School Check hearing and vision test results are entered onto the Ministry of Health B4SC Data Base as soon as possible after testing is completed Itineraries. Preparation for School Visit Communication Itineraries are planned and implemented in the appropriate timeframe. Establishments are visited 2-3 times per year at week intervals. Obtain school lists from Ministry of Education data base (Enrol) and then reference with B4SC data base to determine which children to be tested Good communication, negotiating skills. Parents/caregivers given clear and appropriate information by letter, by phone or in person. Competent with for communication within WDHB. Waitemata District Health Board -JOB DESCRIPTION - Page 3

4 Maintenance of Equipment Audiometers and tympanometers are calibrated annually. Vision charts and letter matching cards are clean and in good condition Disinfecting of equipment is maintained as per protocol. Equipment has current and update stickers displayed. Accurate results are maintained when all equipment is kept to required standards. Clinics Audiometry threshold testing is available to children aged 4 years and school children. Vision screening from aged 4 years is available in clinics All testing is carried out and recorded in accordance with guidelines. Parents/Caregivers are given appropriate information as to follow up from results. Health education and information is provided to parents and caregivers. Health and Safety Participate in health and safety training. Identification and reporting of hazards and incidents or accidents, recognizing individual responsibility for workplace health and safety under the Health and Safety in Employment Act 1992 Attendance at training courses and reporting of hazards as per Waitemata DHB policies. Company health and safety policies are read and understood and relevant procedures applied to their own work activities Can identify health and safety representative for area Health Education Respond to all opportunities of health education promotion in eye and ear health. Training and Development Undertake in-service training and maintain appropriate current knowledge base. Clinical performance reviews to be conducted on all Vision/Hearing Technicians according to current policy. Careerforce NZQA qualification National Certificate in Community Support (Vision and Hearing Screening) (Level 3) expected to be undertaken Correct and appropriate health information and available health services are given to children, their parents, teachers and other stakeholders. Participation in the multi-disciplinary team sessions within Child & Family. Relationships are maintained with other health providers and stakeholders. Training and development needs are identified and communicated to Team Leader. 5 Quality Processes and protocols are adhered to in accordance with national guidelines and policies. Medical, nursing and diagnostic advice must not be offered. Participate in activities in the team to identify process and quality improvement. Changes to policy and procedures receive prior approval. All queries are referred to the appropriate professional.
