The Future of PRIMARY Care is Local Get involved and help build coordinated, connected and accessible healthcare in your community. 1
WHO WE ARE Your Local Health Integration Network Toronto Central LHIN is responsible for planning, funding and integrating local health services that meet the needs of 1.3 million residents and tens of thousands of others who come to Toronto for care. Our Strategic Plan focuses on working with our residents, patients, caregivers, 170 health service providers, and other partners to build a local health system that is coordinated, easy to navigate and provides timely and equitable access to care. The people we serve across Toronto DEMOGRAPHICS 1.3 million inhabitants 100,000+ travel to Toronto Central LHIN for care 14 % over the age of % immigrants 170 languages spoken 33.6% visible minorities 20% of residents living in low income households (highest in province) HEALTH CONDITIONS 1/3 of Toronto Central LHIN residents over the age of 12 have chronic conditions: Residents over the age of 20: 8% have diabetes 18% have high blood pressure 13% have asthma And 9% of adults 35 and over have COPD 9% have visited their doctor for mental health concerns MOHLTC population estimates; 2011 Census and National Health Survey; Canadian Community Health Survey; Ontario Community Health Profile Partnership
Supporting Primary Care Local Leads Primary Care Clinical Leads have been recruited into leadership positions in five sub-regions within the Toronto Central LHIN. These Leads will partner with primary care providers, other local health service providers and organizations to improve access to care, co-ordination and service integration. Our goal: To build a population-based, person-centred, and integrated health care system, with a focus on primary health care and community care What Happens Next? Primary care physician input is needed to help build the system of primary care that we need. Local primary and community care committees are being formed in each sub-region to guide, develop and implement improvements locally that will improve access and care coordination across the health care system overall. Please share your address with the Toronto Central LHIN so we can keep in touch, seek your input, and advice on improving care for your patients. Connect with us at tclhin.primarycare@lhins.on.ca 2
OUR GEOGRAPHY Patient choice and provider autonomy will be respected in all planning activities by the LHIN. The five sub-regions within Toronto Central LHIN are useful for planning purposes and understanding population health, but are not boundaries. Patients and physicians can move freely across sub-regions and LHINs. Physician Advisor, Primary Care Strategy, Toronto Central LHIN: Dr. Phil Ellison Allen Rd 232, WEST Highest rates of hospitalization for chronic diseases Dr. Donald Smith Islington Ave 305, Dundas St W WEST Gardiner Expy Weston Rd Eglinton Ave W MID-WEST Queen St W MID-WEST Fastest growing population in Toronto Central LHIN Dr. Pauline Pariser pauline.pariser@lhins.on.ca 3 ** Primary Care Physicians are defined by a basket of service methodology developed by Ministry of Health and Long-term Care. These totals are an estimate from January 2016 and include primary care providers that belong to a Physician Enrollment Model (PEM) or satisfied specific billing requirements.
Highway 401 NORTH 199, NORTH Largest population of seniors over 65 years old Dr. Jocelyn Charles and Dr. Yoel Abells Yonge St Don Valley Pkwy EAST Danforth Ave Warden Ave 269, MID-EAST EAST High proportion of new immigrants Dr. Geordie Fallis 143, MID -EAST High proportion of individuals living below the low-income cut off Dr. Curtis Handford Facility Type Hospital Rehab/Complex Continuing Care 2011 Census; Toronto Central LHIN; Ministry of Health Long Term Care 4
OUR SHARED PRIORITIES Through engagement with primary care providers, physician leaders have confirmed five priorities for system improvement: Attachment, access and 1 continuity of care Ensure that all residents of the Toronto Central LHIN have access to a primary care provider and are able to receive care in a timely manner. 2 Access to Inter- Professional Teams Improve access to inter-professional teams for all patients of primary care providers. Access to Specialist 3 Consultations Improve and streamline access to specialists. 5
HOSPITAL DISCHARGE 4 PLANNING Improve the quality and timeliness of discharge summaries and to expand enotification around patients who access emergency or inpatient care in hospital. 5 Secure Communication Expand to secure communication between providers to expedite follow-up, improve care coordination, and reduces duplication of tests. Secure provider/patient communication will also be explored in this work. The Toronto Central LHIN wants to get to know you and work in partnership with primary care providers to improve care for the communities we serve. Your input is critical to the success of this work. Connect with us at tclhin.primarycare@lhins.on.ca Physicians have a unique perspective of the health care system, which gives them the potential to drive integration and coordination of the patient journey. Primary care providers and inter-professional teams are working hard to help patients navigate the health care system, but communications and coordination needs to be enhanced to support their efforts to improve patient care. 6
