1 JOB DESCRIPTION POSITION: DEPARTMENT: LOCATION: REPORTING: Resident Care Team Leader (Registered Nurse - RN or Licensed Practical Nurse- LPN) Resident Care AVENS A Community for Seniors Resident Care Supervisor PURPOSE The Resident Care Team Leader (RCTL) is primarily responsible for ensuring all resident care requirements are carried out and met in a safe, timely and effective manner within the parameters of AVENS A Community for Seniors policies and procedures, legislated requirements and accreditation and professional standards of care. This includes supervision of Resident Care staff and reporting regularly to the Resident Care Supervisor and/or Director of Care. SCOPE Registered Nurse (RN) - Applies knowledge and skills within the RN Scope of Practice as defined by the Registered Nurses Association of the Northwest Territories and Nunavut (RNANT/NU). Licensed Practical Nurse (LPN) - Applies knowledge and skills within the LPN Scope of Practice as defined by the Government of the Northwest Territories. The RCTL operates based on the Supportive Pathways Model of Care as well as the: AVENS Mission, Vision and Values AVENS Vision Seniors have a safe and caring community for life. Within these principles, and under the direction of the Resident Care Supervisor (RCS) or the Director of Care (DOC), the incumbent carries out resident care requirements within the facility while supervising the activities of Resident Care Team Members. This position liaises closely with Recreation Therapy and Dietary/Housekeeping programs as part of the overall care team. RESPONSIBILITIES 1) Supervise the Resident Care Aides (RCAs) to ensure the provision of quality care in accordance with established policies and procedures and standards of care by: Assigning scheduled activities to the Resident Care Team Members during shifts Encourages and adheres to a holistic philosophy of the Supportive Pathways Model of Care Completing shift reports Monitoring assignments and recurring activities of the care team against established procedures Recommending action to the RCS/DOC to improve and/or recognize the work of care team members Assessing the need to address unusual circumstances related to resident care or facilities security and maintenance and recommending changes to policies and procedures to address practical problems or opportunities for efficiencies Providing on the job training to Resident Care Team Members to improve performance and/or when procedures change Researching and implementing best practices in terms of long term resident care Ensuring the cultural and spiritual values of the residents are respected Mentoring, coaching and role modelling Supportive Pathways concepts of care. 2) Lead and provide resident care and nursing services as identified in the Care Plan by: Providing and mentoring others in the provision of resident care Contributing to the development of, and ongoing revisions to, care plans Providing nursing services in accordance with care plans Taking specimens (including blood) for the purpose of obtaining the results of diagnostic tests Distributing, administering and recording the use of pharmaceuticals in accordance with doctors' prescriptions and control procedures Performing personal care tasks such as bathing Contributing to residents mental, psychological and social well-being by providing companionship and stimulation Providing on-going programming as follow up to Occupational and Physiotherapy recommendations Performing duties in a manner which provides for a safe environment for residents by adhering to safety procedures and by remaining current in the field of care for RESIDENT CARE TEAM LEADER RN/LPN August 2014 Page 2 of 5
3 the aged. 3) Report on resident care and communicate with staff and co-workers to ensure staff, supervisors and family members are informed of the progress of residents by: Consulting with and requests direction from the RCS/DOC or the resident s physician in the event a care situation is outside the scope of practice or competency level of the individual RN or LPN Liaising with the RCS/DOC for clinical support or direction as needed Communicating to the RCS/ DOC in a timely manner regarding the need for a resident reassessment or when the level of care required exceeds that which is appropriate for the LTC facility Providing on going direction and feedback to the RCAs when on shift to ensure all assigned tasks and duties are completed according to the residents care plans and the established standards Reporting unsafe or unusual incidents to the RCS/ DOC by way of completing and documenting the applicable Unusual Occurrence Reports Responding to or referring inquiries from family members and friends about a resident s condition to the RCS/ DOC when required Preparing written shift reports Logging all incidents such as arguments, difficulties and stress experienced by residents Advising the RCS/RCM and staff of other shifts on changes in resident s behaviour and any special instructions to be carried forward into the next shift 4) Participate in quality control responsibilities and professional development activities by: Participating, in person or by written submission, in the planning program development and evaluation work of committees such as the Infection Control, Quality Management, Emergency Response and Occupational Health and Safety Committees Performing, or receiving, assigned in-service training events. 5) Assist with other related duties as required. QUALIFICATIONS Registered Nurse (RN) The incumbent must be a graduate of a recognized program for Registered Nurses and must be eligible for full practicing licensure with the RNANT/NU. Experience working with an elderly population and individuals that have dementia is an asset. An acceptable criminal records check is required and must be maintained. Licensed Practical Nurse (LPN) The incumbent must be a graduate of a recognized program for Practical Nurses and must be eligible for full practicing licensure with the Government of the Northwest Territories. NOTE: A strong candidate for this opportunity must possess 2 year(s) or more of experience in a similar role. Work schedules will be regularly reviewed and may be changed as needed in accordance to operational requirements. The incumbent will be RESIDENT CARE TEAM LEADER RN/LPN August 2014 Page 3 of 5
4 required to work various shifts including weekends, day, evening and night shifts. KNOWLEDGE, SKILLS AND ABILITIES The preferred incumbent will have the following knowledge, skills and abilities: Takes personal responsibility for maintaining current and up to date knowledge of changes in or new developments in gerontological practises with an aim to continuously improve service and care delivery Knowledge of the aging process and the ability to apply this in a professional manner in a care situation Demonstrated nursing skills Strong health assessment Knowledge and experience with Quality and Risk Management processes and systems in a care setting Proven team leadership and human resource management skills in a care setting Knowledge of and ability to research and implement best practices in terms of long term and dementia care Excellent reading and writing skills Computer skills to maintain charts and prepare activity reports Problem solving skills Excellent interpersonal skills including compassion and understanding for residents Good listening skills. EQUIPMENT USED The incumbent must be familiar with the operation of the following equipment: Computer Cell phone Fax machine Nursing equipment PHYSICAL REQUIREMENTS The incumbent must be able to: Stand for extended periods during the shift Walk for extended periods during the shift Bend forward and in various directions to assist residents and complete duties Kneel for periods of time Lift and carry loads up to 25 kg in weight Assist residents who need are dependent for support with transfers and mobility Regularly engage in physical exertion Complete repetitive motion, and Operate in awkward positions.
