4 Healthcare Associated Infections Increase Morbidity & Mortality (Pain, Suffering and Death) CDC estimates that each year about 2 million patients admitted to acute care hospitals in the U.S. acquire an infection that was not related to the reason for hospitalization These infections result in about 99,000 deaths (one death every 6 minutes!!)

5 How do we protect our patients?? Not all healthcare associated infections are preventable but there are evidence based practice strategies that will reduce the incidence of CAUTI CLABSI VAP Surgical Site Infections

6 PATHOGENS ARE MOST OFTEN PASSED FROM PATIENT TO PATIENT ON THE HANDS OF HEALTHCARE WORKERS.

7 Hand Hygiene is the most important means of preventing and/or controlling infections

14 GEL OR WASH AFTER REMOVING YOUR GLOVES!! 12% OF THE TIME WHATEVER WAS ON THE OUTSIDE OF YOUR GLOVES BEFORE REMOVAL IS ON YOUR HANDS AFTER REMOVAL (and don t wear gloves in the hallway )

16 Use Safe Work Practices to keep yourself safe and limit the spread of contamination Keep hands away from face Perform hand hygiene Limit surfaces touched Change gloves when torn or heavily contaminated

17 CAUTI Assess the need for a foley EVERY DAY Foley Bags off the floor! Foley Bags below the bladder Tubing secured to the leg-no tension or tugging! Good perineal care

18 CLABSI Assess the need for a central line EVERY DAY Check site every shift Change dressing if loose or soiled Avoid the antecubital site for peripheral IV s and the femoral site for central lines Scrub the hub before every access for seconds and allow to dry Change peripherals every 96 hours Change any line put in under adverse conditions within 24 hours

19 VAP Head of bed 30⁰ Oral care every 4 hours Sedation Vacation

20 Surgical Site Infections Pre-Op Antibiotics Appropriate Hair Removal-No Shaving! Glucose and Temperature Control

21 Initiating Precautions Any professional nurse or physician may initiate Precautions If initiated by a nurse, an order is to be obtained that day or the next day Required for known or suspected diseases/organisms Provide education and document in EPIC Use the patient education modules in the IP Information Box or the intranet

22 Use Yellow Charts for Precautions Always put a copy of the actual Precautions sign on the inside front of the chart Clean charts between patients

23 Contact Precautions Private room or cohort Gloves when entering room Gowns when entering room C difficile, MDRO s RSV, viral respiratory infections in infants & toddlers, localized zoster

24 For Patients with Infective Diarrhea C. Difficile diarrhea Norovirus common cause of food poisoning ALWAYS USE SOAP AND WATER!!

25 BEFORE ENTERING AND UPON LEAVING THIS ROOM WASH YOUR HANDS WITH SOAP & WATER ONLY

26 Airborne Precautions (Small Droplet Aerosol) Prevent transmission by small droplet nuclei Negative pressure room N 95 mask TB, measles, smallpox

27 Droplet Precautions (Large Droplet Aerosol) Private room or cohort with same Standard surgical mask H.influenzae, including epiglottitis, influenza, mumps, meningococcal infections, pertussis, rubella

28 Seasonal Influenza Private room or cohorted Surgical mask to enter Gown and gloves if risk of contact with secretions N95 mask and eye protection for aerosol generating procedures Discontinue 7 days after the onset of symptoms Encourage resp. etiquette

40 Educate Your Patient! The IP Information Box has materials on many topics in both English and Spanish! Preventing line infections Preventing surgical site infections Isolation Precautions..and more Vaccine Information Sheets (VIS) are in the IP Information Box and med rooms. Make sure your patient receives one when getting the influenza or pneumococcal vaccine.
