3 Core Criteria for Stage 1 1. Use CPOE for medication orders directly entered by any licensed healthcare professional who can enter orders into the medical record per state, local and professional guidelines. 2. Implement drug-drug and drugallergy interaction checks. OrderEase has been certified and MDOrder will be certified to meet this : OrderEase is a web-based inpatient and outpatient CPOE solution. MDOrder is a desktop solution designed by physicians and developed with MAGIC sites in mind. It offers fast implementation and streamlines the medication ordering process to help achieve Meaningful Use in 2011/2012. The following solution will not be certified but can assist hospitals in meeting this : Iatric Systems interfaces can connect ONC-ATCB certified CPOE products with Healthcare Information Systems (HIS). EasyConnect Jaguar interface engine can help solve the challenges of exchanging, sharing and retrieving electronic health records across different systems and eliminating point-to-point interfaces. The following solutions will be certified to meet this : OrderEase and MDOrder will include drug-drug and drug-allergy interaction checks. 3. Maintain an up-to-date problem list of current and active diagnoses. OrderEase has been certified and MDOrder will be certified to meet this : OrderEase and MDOrder include a user-friendly Problem List feature, fully integrated with the ordering process. The following solution will not be certified but can assist hospitals in meeting this : Visual SmartBoard enables hospitals to track and flag whether a Clinical Decision Support (CDS) query was answered in order to monitor compliance with this. 4. Maintain active medication list. The following solutions will be certified to meet this : OrderEase and MDOrder will allow users to electronically record, modify and retrieve a patient s active medication list. The following solution will not be certified but can assist hospitals in meeting this : Iatric Systems interfaces can electronically transmit medications from various Emergency Department Information Systems to MEDITECH s RXM application in order to help hospitals maintain active medication lists. EasyConnect Jaguar interface engine can help transmit the messages that contain medication information from one point to another. 5. Maintain active medication allergy list. The following solution will not be certified but can assist hospitals in meeting this : Iatric Systems interfaces can electronically transmit medication allergies from various Emergency Department Information Systems to MEDITECH s RXM application in order to help hospitals maintain active medication allergy lists. EasyConnect Jaguar interface engine can help the transmit messages that contain medication information from one point to another. 6. Record all the following demographics: (A) Preferred language (B) Gender (C) Race (D) Ethnicity (E) Date of birth (F) Date and preliminary cause of death in the event of mortality in the eligible hospital or Critical Access Hospital (CAH) 3

Record and chart changes in the following vital signs: (A) Height (B) Weight (C) Blood pressure (D) Calculate and display body mass index (BMI) (E) Plot and display growth charts for children 2-20 years, including BMI 8. Record smoking for patients 13 years old or older. The following solution is certified to meet this : Visual Flowsheet displays vital signs, growth charts and BMI graphs. Visual Flowsheet also can calculate and display BMI, as well as plot and display growth charts for children 2-20 years old. The following solution will not be certified but can assist hospitals in meeting this : Clinical Monitor Interface electronically transmits vital signs and vent and pump information into MEDITECH systems. The following solution will not be certified but can assist hospitals in meeting this : Visual SmartBoard enables hospitals to track and flag whether a CDS query was answered in order to monitor compliance with this. 9. Report hospital clinical quality measure to CMS (or in the case of Medicaid eligible hospitals or CAHs, the States) hospital clinical quality measure selected by CMS in the manner specified by CMS (or, in the case of Medicaid eligible hospitals or CAHs, the States). The following solution has been certified to meet this : Meaningful Use Manager is a web-based solution that provides an instant view of your organization s compliance with all 24 Stage /2012 Meaningful Use requirements. Easily report progress to your Board and senior management. Clinical Quality Measures provide numerator/denominator values and submission for 15 Meaningful Use quality reports. comply with this Meaningful Use. 10. Implement one clinical decision support rule related to a high priority hospital condition along with the ability to track compliance with that rule. 11. Provide patients with an electronic copy of their health information (including diagnostic test results, problem list, medication lists, medication allergies, discharge summary, procedures), upon request. Sites can use alert s, text messages (SMS messages), sounds, flashing text and pop-up windows with recommendations. The following solutions are certified to meet this : PtAccess allows patients to view electronic health information, including discharge instructions and clinical results in a secured web-based interface. Patients can view diagnostic test results, problem lists, medication lists, allergies, discharge summaries and procedures. Additional capabilities include reviewing billing statements, making payments via the Internet, as well as printing and securely ing their health information. PHR Connect integrates a hospital s electronic medical record (EMR) data with personal health records (PHR) such as Microsoft Health Vault. PHR Connect automatically populates and updates a patient s PHR with the most recent hospital EMR data. This allows patients to access their health information from anywhere and share that information securely with caregivers and family members. Clinical Document Exchange provides the ability to download patient records to a thumb drive/cd and/or provide a hard copy of patient health information. 4
5 Core Criteria for Stage 1 continued 12. Provide patients with an electronic copy of their discharge instructions at time of discharge, upon request. The following solutions are certified to meet this : PtAccess allows patients to view electronic health information, including discharge instructions, and clinical results in a secured web-based interface. Patients can view diagnostic test results, problem lists, medication lists, allergies, discharge summaries and procedures. Additional capabilities include reviewing billing statements, making payments via the Internet, as well as printing and securely ing their health information. Patient Discharge Instructions provides hardcopy and electronic discharge instructions and procedures to patients upon request. 13. Capability to exchange key clinical information (for example, problem list, medication list, medication allergies, and diagnostic test results), among providers of care and patient authorized entities electronically. The following solutions are certified to meet this : PHR Connect integrates a hospital s electronic medical record (EMR) data with personal health records (PHR) such as Microsoft HealthVault. PHR Connect automatically populates and updates a patient s PHR with the most recent hospital EMR data. Clinical Document Exchange enables exchange of key clinical information between systems. 14. Protect electronic health information created or maintained by the EHR technology through the implementation of appropriate technical capabilities. Iatric Systems applications provide unique identifiers, session timeouts, encryption and audit logs that track patients Protected Health Information (PHI) disclosure and verify user identities and access privileges. The following solution will not be certified but can assist hospitals in meeting this : Security Audit Manager allows hospital staff to quickly and easily monitor access to patient data by automating manual audit processes. This helps hospitals quickly identify and report patient privacy violations and comply with HITECH patient privacy breach reporting mandates. Additionally, Security Audit Manager reports help educate hospital employees about new, strict federal breach reporting mandates, and they allow the patient to receive a complete list of who has seen his or her record. 5
6 Menu Criteria for Stage 1 for eligible hospitals or Critical Access Hospitals (CAH): must meet five of the following objectives and associated measures. 1. Implement drug formulary checks. 2. Record advance directives for patients 65 years old or older. The following solutions will be certified to meet this : OrderEase and MDOrder will include drug formulary checks. The following solution will not be certified but can assist hospitals in meeting this : Visual SmartBoard enables hospitals to track and flag whether a CDS query was answered in order to monitor compliance with this. 3. Incorporate clinical lab test results into EHR as structured data. 4. Generate lists of patients by specific conditions to use for quality improvement, reduction of disparities, research, or outreach. 5. Use certified EHR technology to identify patient-specific education resources and provide those resources to the patient if appropriate. 6. The eligible hospital or CAH that receives a patient from another setting of care or provider of care or believes an encounter is relevant should perform medication reconciliation. 7. The eligible hospital or CAH that transitions their patient to another setting of care or provider of care or refers their patient to another provider of care should provide summary care record for each transition of care or referral. 8. Capability to submit electronic data to immunization registries or immunization information systems and actual submission according to applicable law and practice. comply with this Meaningful Use.
