It is the 5 th IIM to be established by the Govt of India in collaboration with the Govt of Kerala in The Institute campus is spread over a 100-acre lush green scenic Malabar hills at Kunnamamgalam in Kozhikode near the national highway to Wayanad. It has about 800 students in campus with about 200 faculty and staff. About 100 families also reside in the campus. IIMK is on a high growth trajectory today, offering widest range of academic programs in the field of management education. Being one of the premier management institutes in the country set up by Ministry of HRD, Govt. of India, it is contributing to management education and developing human resources to meet requirements of India s economy and industries. IIMK is desirous of running/operating a small dispensary/clinic for examination, consultation, prescription to outpatients including dispensing of medicines in its campus to meet the day to day health care requirements of community consisting of the students, faculty & staff and their families and guests. The dispensary shall be run by a doctor with at least an MBBS qualification along with a Staff Nurse and a Pharmacist on daily basis, less Sundays and other public holidays in the afternoon for a time period of three hours. Services may also be requisitioned for special occasions such as central functions, VIP visits etc. for which separate charges shall be payable. Request for Proposals (RFP) Expression of Interest (EOI) is invited from eligible and qualified doctors / hospitals for running a Dispensary at IIMK Campus for an initial period of two years. The period of contract could, however, be extended for further periods on mutual consent. Notice Inviting EoI for Running a Dispensary at IIM Kozhikode Campus Page 1

2 Scope of Services: The dispensary will provide examination, consultation, prescription to outpatients including dispensing of medicines by a general practitioner/ specialist doctor. Medical examination and Consultation: To examine patients, provide consultation and prescribe medicines for common ailments. Advice/refer for treatment requiring specialist attention. Prescribe routine medical tests when required. Medical Dressing: Carry out routine dressing and treatment of wounds, administering injections etc. Pharmacy Services: Dispersing Medicines as per prescriptions by the doctor. Advisory Services: Provide advice on preventive measures to be taken against contagious diseases and related matters. Special Services: providing special medical services for central events like convocation etc and during VIP visits. Scope of services is not all exclusive and could be modified by IIMK and the doctor/hospital/nursing home from time to time. Responsibilities of the Doctor/Hospital/Nursing Home To provide medical services as per Clinical Establishments (Registration and Regulation) Act, 2010 and The Clinical Establishments (Central Government) Rules, 2012 and other medical rules and regulations as applicable to our country and state. The doctor and staff of the dispensary will observe courteous behavior towards the patients especially towards female patients. Conveyance for the medical staff back and forth to the dispensary will be the responsibility of the doctor/hospital. Conveyance of patients to a hospital on advice of the doctor will be arranged or provided by IIMK. In case the doctor or any other medical staff is unable to attend the clinic at the appointed time, a suitable replacement will be provided by the service provider. All required statutory compliances and licenses required for running the dispensary will be the responsibility of the service provider. IIMK will provide all possible help as required for the same. Responsibilities of IIMK: IIMK shall provide space for running the dispensary. Space as existing may be examined and modifications, if required, will be carried out to the extent feasible by IIMK. IIMK will procure stores and equipment as required for running the dispensary on receipt of a written requisition and advice from the doctor/hospital. 3 Technical Qualifying Criteria: The doctor will at least be an MBBS with at least one-year experience; the nursing staff should be a qualified staff nurse, with at least one year experience and the pharmacist having at least a diploma in pharmacy with at least one year s experience. The doctor/ hospital should have at least one-year s previous experience in running a medical dispensary or clinic. Submission of Proposal(Expression of Interest): Sealed envelope containing completely filled and duly signed Proposal with all supporting documents shall be submitted by the vendor/service provider. The envelope shall be super scribed as Expression of Interest for Running a Dispensary at Indian Institute of Management Kozhikode. EOI No (03)/2015-IIMK.GA Date of Opening 29 December Please refer to Appendix for Performa. The Expression of Interest documents in the abovementioned envelope may be sent by post or personally deposited in the Tender Box of The Institute at the reception. The Expression of Interest should be submitted latest by 3:00 PM on 29 December Opening of Expression of Interest: The Expression of Interest will be opened at 16:00 hours on 29 December 2016 at IIMK in the presence of nominated representatives of IIM Kozhikode and the Service Providers or their representatives they choose to be present. In the event of the specified date of bid opening being declared a holiday for IIMK, the bids shall be opened at the appointed time and location on the next working day. The bids so received will be evaluated by a duly constituted committee and recommendations put up for approval to the competent authority. Evaluation of Expression of Interest: IIM Kozhikode will determine the substantial responsiveness of each proposal with reference to EOI terms and conditions. For this purpose, a substantially responsive proposal is one, which conforms to all the terms and conditions of the EOI documents without material deviations. The evaluation shall normally but not necessarily be based on L1 basis for the entire scope of work. The scope besides cost will include the profile of the tenderer and past record, ability to provide the desired services and a presentation to be made by the EOIer, if required. However, IIMK does not pledge itself to accept the lowest or any Bid and reserves to itself the right of accepting the whole or any part or portion of the Bid or cancel the EOI without assigning any reason whatsoever. Performance Guarantee: Within 15 (fifteen) days of the receipt of notification of award/letter of Intent, the successful service provider shall furnish a performance guarantee equivalent to an amount which is 5 % of the annual value of the quoted amount, in the form of Bank Guarantee from any scheduled Bank or a Fixed Deposit or Demand Draft or any other acceptable form of payment. The Performance Guarantee shall be kept valid for a period of 2 years plus 2 months. The Performance Guarantee amount shall be payable without any condition whatsoever and this guarantee shall be irrevocable. 4 performance in the entire tenure of the agreement between IIMK and the Service Provider. The Performance Guarantee/Security Deposit shall be released only on satisfactory performance of the terms of agreement between the Service Provider and IIMK for the entire period of the agreed time frame plus two months. The deposit will not bear any interest. The Security Deposit is liable to be forfeited, if the Service Provider fails to execute the work as per the terms and conditions of the agreement and to the satisfaction of IIMK or on account of any breach of the agreement. The selected service provider shall execute an agreement on non-judicial stamp paper of appropriate value within two weeks from the date of acceptance of the order with IIMK at Kozhikode in a format to be mutually agreed by both parties. The successful service provider shall commence operation from the date mentioned at the award of the Contract. Guidelines for submitting EOI: 1. Service providers should furnish the required details in their own letter-heads. In case desired documents/proof is not enclosed, the EOI may be rejected and no clarification/enquiry in this regard will be entertained. 2. The EOI must be sent in sealed cover so as to reach the address mentioned below on or before 3:00 PM on 29 December IIMK shall not take responsibility if the EOI is not received at the below mentioned place within due date and time and the EOI received after the date and time stipulated for receipt of EOI is liable to be rejected. No request for extension of time for submission of EOI shall be considered on the plea of delay by Post/Courier, etc. 3. The last date and time of receiving the EOI shall be adhered strictly. EOI received after the last date and time will not be opened and no further action will be taken on such EOI. However, if the last date is declared as holiday, the next working day on which office is opened will be treated as the last date of receiving the EOI. 4.
