2 Dear Fellow Texan: is honored to serve the Greater Dallas and surrounding communities. The hospital s mission is to operate an integrated heart and vascular health care delivery system, founded as a Christian ministry of healing that exists to serve people by offering a continuum of quality service committed to quality care and patient safety, medical education, research and community service. As this community benefit report demonstrates, Baylor Hamilton Heart and Vascular Hospital provides quality care that includes subsidized services, medical education, and research. We also provide health education and screenings to the community. Considering our specialty, the community service area of East Dallas and South Dallas, and using data from the Dallas County Needs Assessment, Baylor Hamilton Heart and Vascular Hospital focuses our efforts in the community primarily on cardiovascular and peripheral vascular disease. We deliver these benefits utilizing multiple tactics, including community education forums, health screenings, health fairs, and television broadcasts. Baylor Hamilton Heart and Vascular Hospital also partners with Baylor University Medical Center to provide cardiovascular support and education at community events. I encourage you to give me feedback for this report. Please address comments to me, in care of Jennifer Coleman, Senior Vice President, Public Affairs, Baylor Health Care System, and 3500 Gaston Avenue, Dallas, Texas Sincerely, Nancy Vish, RN, PhD, NEA-BC, FACHE Page 2

The funds were used to produce several community education groups: LINK Connecting Cardiovascular Health and Wellness Across Generations, Wired For Life and Leap For Life. LINK, is a venue for the community to attend educational seminars on attaining and maintaining heart health. Wired for Life, an education class for patients with newly-implanted devices to tell their stories and receive detailed information regarding their ICD, cardiac disease, medications and lifestyle changes. Leap For Life is an prevention education program. Topics include nutrition, exercise, cardiovascular procedures, medications and stress management. This event is free to the public and offered twice during the month. Research shows that stress, and the unhealthy behaviors people use to manage it, contribute to some of our country s biggest health problems such as obesity, heart disease and diabetes. So it s imperative that people take steps to address issues like holiday stress in healthier ways. Physicians on staff at the Baylor Jack and Jane Hamilton Heart and Vascular Hospital appeared live on Good Morning Texas in multiple educational segments to share tips on how to recognize and manage symptoms and harmful signs relative to heart health. 1.
